2. Case History
ผู้ป่วยชายไทย โสด อายุ17 ปี HN 1901991
เชื้อชาติไทย สัญชาติไทย
ภูมิลาเนาอาเภอเมือง จังหวัดนครราชสีมา
อาชีพ นักศึกษา
เวลาเกิดเหตุ 8.50 น. 22/11/60
เวลาถึงโรงพยาบาล 8.53 น. 22/11/60

3. Chief complaint
 MC ล้มเอง 30 นาทีก่อนมา

4. Present illness
30 minPTA สุนัขวิ่งตัดหน้า หักหลบ MC ล้ม ศีรษะกระแทกพื้น ไม่
สลบ จาเหตุการณ์ได้ไม่ปวดศีรษะ ไม่ N/V แขนขวากระแทกพื้น ไม่มี
แผลเปิด ปวดบวม แขนผิดรูป ขยับแขนไม่ได้ จึงมาโรงพยาบาล

5. Primary survey
 A : can speak , can flex neck , not tender along c-spine
 B : equal chest movement , normal breath sound
 C : BP 141/88 PR 76 bpm , PCT negative
 D : E4V5M6 , pupil 3 mm RTLBE
 E : Abrasion wound at Lt forearm , no open wound

6. Secondary survey
 A : no drug allergy
 M : no current medication
 P : no underlying disease , no surgical history
 L : last meal 7.00 am
 E : Motorcycle accident

7. Physical examination
 V/S BT 37oc BP 141/88 mmHg PR 76 bpm RR 20/min
 GA : A Thai man good consciousness , well co-operative
 HEENT : not pale , no jaundice
 CVS : normal S1S2 , no murmur
 Lung : normal breath sound , equal both lung
 Abdomen : soft not tender , no hepatosplenomegaly

8.  Extremities : deformity at Rt forearm and mild swelling at distal part
dorsal side of forearm with limited ROM of wrist due to pain , can
palpable stepping at dorsal side with marked tender , sensory intact ,
radial pulse 2+ , radial, ulnar and median nerve is intact

9. Investigation

11. Diagnosis
 Close fracture of Rt distal end redius (Smith’s fracture)

12. Management
 Control pain with MO 4 mg IV stat
 Reduction
 On long arm AP slap with arm sling
 D/C

16. Distal Radius fracture
 Most common orthopaedic injury
 younger patients - high energy trauma
 older patients - low energy / falls / osteoporosis
 Associated injuries
 DRUJ injuries
 radial styloid fx
 soft tissue injuries
 TFCC injury
 scapholunate ligament injury
 lunotriquetral ligament injury

17. Extra-articular distal radius fracture
 Stable fracture
โอกาสที่ Fracture site เคลื่อนหรือ loss reduction มีน้อย เช่น Incomplete fracture (หัก
เป็นรอยร้าว) , Impact or Torus Fracture (การย่นของกระดูกจากแรงอัด) และ non-
displace or minimal displacement fracture
 Displace fracture
กระดูกหักที่ fracture site displace หรือ angulate มากจาเป็นต้องจัด reduction ก่อนการใส่
เฝือก และหลังจากใส่เฝือกแล้วยังมีโอกาสเคลื่อนได้อีก ได้แก่ Colle’s fracture และSmith’s
fracture

18. Smith’s fracture
 Named of Robert William Smith , Irish
 ลักษณะการหักของ distal radius ที่มี Volar displacement และ Volar angulation ของ distal
part of fracture ตรงข้ามกับ Colle’s fracture
 Colle’s fracture จะพบได้บ่อยกว่าและเป็นลักษณะตรงข้ามกันคือ มี dorsal displacement และ Volar
angulation ของส่วน distal part of fracture โดย periostrium ด้าน dorsal intact

21. Complications
 Median nerve neuropathy (CTS)
 most frequent neurologic complication
 prevent by avoiding immobilization in excessive wrist flexion and ulnar deviation (Cotton-
Loder Position)
 Ulnar nerve neuropathy (seen with DRUJ injuries)
 EPL rupture
 Radiocarpal arthrosis
 Malunion and Nonunion
 ECU or EDM entrapment (entrapment in DRUJ injury)
 Compartment syndrome
 RSD/CRPS ( Reflex sympathetic dystrophy / Complex Regional Pain Syndrome )

22. management
 Nonoperative
 closed reduction and cast immobilization
 indications
• extra-articular
• <5mm radial shortening
• dorsal angulation <5° or within 20° of contralateral distal radius

23.  Reduction
 1. Traction
 2. extension
 3. supination
 Cast
 Long arm cast with three point fixation
 Position is elbow flex 90o + full supination + slight dorsiflexion wrist and
ulna deviation
 Duration 3-4 weeks
 Complication
 Acute carpal tunnel syndrome
 EPL rupture

24.  Operative
 surgical fixation (CRPP, External Fixation, ORIF)
 Indications : radiographic findings indicating instability (pre-reduction radiographs best
predictor of stability)
 displaced intra-articular fx ( >1mm )
 volar or dorsal comminution
 articular margins fxs
 severe osteoporosis
 dorsal angulation >5° or >20° of contralateral distal radius
 >5mm radial shortening
 comminuted and displaced extra-articular fxs (Smith's fx)
 progressive loss of volar tilt and loss of radial length following closed reduction and casting
 associated ulnar styloid fractures do not require fixation
