! ชายไทย อายุ 17 ปี HN : 1977014

3. ! CC : ปวดแขนขวา 2 ชั่วโมง
! PI : 2 hr. PTA ขับรถจักรยานยนต์ชนรถจักรยานยนต์อีก
คัน ไม่ล้ม ไม่มีศีรษะกระแทก คันเร่งจักรยานยนต์สะบัด
กระแทกแขนขวา ปวดแขนขวาทันที ไม่มีบาดแผลภาย
นอก ขยับข้อศอกและไหล่ขวาไม่ได้ กำมือได้ ขยับนิ้วมือ
ได้ ไม่ชา

4. Primary survey
! A : can speak, no tenderness along C-spine, can active
neck movement
! B : equal breath sound both lungs ,CCT negative
! C : BP 145/84mmHg.,PR 86 bpm
! D : E4V5M6 , pupils 3 mm. RTLBE
! E : Rt. Arm deformity ,tender , no external wound

5. Secondary survey
! Allergy : None
! Medication : None
! Past history : No underlying disease
! Last meal : 7 pm
! Event : as in present illness

6. Physical examination
! GA : A Thai man , Alert
! V/S : BP 145/84mmHg.,PR 86 bpm ,T 36.6 C, RR
18/min
! Heart : normal S1S2 , no murmur
! Lung : clear, equal both lungs
! Abdomen : Soft , not tender

7. Physical examination
! Extremities : Rt.arm deformity, swelling , no external
wound or ecchymosis , can not elevate Rt. Arm and flex
elbow due to pain,Rt.wrist not tender ,full ROM,no
wrist drop,motor : can extend thumb against resistance,
OK sign normal,can abduct fingers against
resistance,Froment’s sign normal ,sensory intact ,pulse
full, capillary refill < 2 secs

! Incidence3-5% of all fractures
! bimodal age distribution
! young patients with high-energy trauma
! elderly, osteopenic patients with low-energy injuries

16. Mechanism of injury
! typically result from trauma such as a direct blow
or bending force to the humerus
! less commonly, from a fall on an outstretched
hand or elbow

17. Fracture pattern

18. Holstein-Lewis fracture
spiral fracture of the distal one-third of the humeral shaft

19. Acceptable alignment
! Less than 20 degrees of anterior or posterior
angulation
! Less than 30 degrees of varus angulation
! Less than 3 cm of shortening

20. Complication
! Non union
! Malunion
! Radial n. palsy : mostly neurapraxia can recover
spontaneously over 7 weeks to 6 months

! Non-operative : U-slab , hanging cast ,functional
brace
! Operative

Iatrogenic radial n. palsy
ipsilateral forearm fracture
compartment syndrome

22. Follow-up
! นัดมาเปลี่ยน slab 2-3 wk , follow up film
! Isometric exercise
! ใส่ slab นาน 6-8 สัปดาห์
