2. Patient Profile
• ผู้ป่วยเด็กหญิง อายุ 7 ปี
• ภูมิลาเนา อาเภอ จังหวัดนครราชสีมา
• สัญชาติไทย นับถือศาสนาพุทธ
• Cause of injury : ถูกผลักล้ม 1 ชั่วโมงก่อนมา รพ.

3. CC : ปวดข้อศอกขวา 1 ชั่วโมงก่อนมา รพ.
PI : 1 ชั่วโมงก่อนมา รพ. ขณะเล่นกับเพื่อน ถูกผลัก
ล้มแขนขวากระแทกพื้น ไม่มีศีรษะกระแทก ไม่สลบ
จาเหตุการณ์ได้ มีอาการปวดข้อศอกขวาทันที ขยับ
ข้อศอกขวาไม่ได้ มีข้อศอกขวาบวม ไม่มีชา

4. Primary survey
• A: can speak ,can move neck, not tender along
C spine
• B: no accessory muscle use, trachea in
midline, clear and equal breath sound,
CCT Negative
• C: BP 113/66 mmHg, PR 90 /min ,not seen
external bleeding, no gross hematuria
• D:E4V5M6, pupil 3mm RTLBES
• E: Right elbow swelling and tender

5. Secondary survey
• A: ปฏิเสธประวัติแพ้ยาแพ้อาหาร
• M:ปฏิเสธใช้ยาประจา
• P:ปฏิเสธโรคประจาตัว
• L: NPO 13.00 น.
• E: ถูกผลักล้มแขนขวากระแทกพื้น มีอาการเจ็บบริเวณข้อศอกขวา

6. Physical exam
• Head & maxillofacial :
no facial deformity, no wound
• Cervical spine and Neck :
can move neck, c-spine not tender
• Chest :
trachea in midline, normal breath sounds
equal both lung, CCT negative

7. • Abdomen :
soft, not tender
• Perineum :
no ecchymoses at pelvic
• Musculoskeletal :Right elbow swelling, tender at
lateral side of right elbow, limit ROM due to
pain,radial pulse 2+,cap refill <2sec,pinprick
sensation intact,can do Great-OK-ByeBye sign
• Neurological :
E4V5M6, pupils 3 mm RTLBE, motor grade V all

8. Investigation
• Film elbow AP,lat

10. Diagnosis
• CFx supra condylar right humerus
(Gartland III)

11. management
• Control pain
• Closed reduction with k-wire and posterior
long arm slab

• แบ่งตาม mechanism of injury เป็น 2 type
❖1. Flexion type พบ 1-2%
❖2. Extension type

normal 64-81 degrees

20. Treatment
• Grade I : Long arm slab 3 weeks
• Grade II: Close reduction and long arm slab
3-4 weeks ± percutaneous pinning
• Grade III: Close reduction and percutaneous
pining

21. Complication
• Brachial artery compression
• Compartment syndrome
• Nerve injury (anterior interosseous,radial)
• stiffness
• malunion
