2. Case
• ผู้ป่วยเด็กชายไทยอายุ7 ปี no U/D
• ภูมิลาเนา อาเภอ ปักธงชัย จังหวัด นครราชสีมา
• อาชีพ นักเรียน
• สัญชาติ ไทย นับถือศาสนา พุทธ
• รับไว้ในโรงพยาบาลครั้งที่ 1

3. Chief complaint
• ปวดศอกซ้าย 2 ชั่วโมงก่อนมาโรงพยาบาล

4. Primary survey
• A: can speak ,can move neck, not tender along C spine
• B: no accessory muscle use, trachea in midline, clear and
equal breath sound, CCT -Neg
• C: BP 113/57 mmHg, PR 99 /min ,not seen external
bleeding, no gross hematuria
• D:E4V5M6, pupil 3mm RTLBES
• E: left elbow swelling and tender

5. Secondary survey
• Allergies: no history of drug or food allergy
• Medications: FENAC 25 mg 1 tab per oral stat (จาก
รพช.)
• Past illnesses: no U/D
• Last meal: 18.00 22/08/2560
• Event: ปั่นจักรยานล้ม 20นาที ก่อนมาโรงพยาบาล(รพช.)
หลังล้มเจ็บข้อศอกซ้าย เยียดแขนไม่ได้

6. Physical examination
• Head: no fracture, no external wound
• Maxillofacial: no deformity ,no bony crepitus
• Cervical spine/Neck : C spine not tender, no sign of airway
obstruction
• Chest: trachea in midline, clear and equal breath sound
• Abdomen:normoactive bowel sound ,soft, not tender
• Musculo skeletal: left elbow swelling, tender at lateral side of left
elbow, can’t flex elbow, can’t extend elbow, can supination, can
pronate
• Neurological:grossly intact

10. Diagnosis
•Cfx lateral condyle of left elbow

11. Management at ER
• NPO
• 5%DN/3 1000 ml iv 80 ml/hr
• CBC, anti – HIV
• CXR
• Set OR for ORIF with K wire

12. Post operative

•Regular diet
•Control pain
•Long arm slab

14. LATERAL CONDYLE FRACTURE

20. treatment
• Non operative
long arm casting 6 wks
• Operative

21. complication
• Stiffness
• Nonunion
• AVN
• Malunion
• Tardy ulnar nerve palsy
• Lateral over growth/prominence(spurring)
• Growth arrest
• Late elbow presentation or deformity
