2. Patient Profile
• ผู้ป่วยชายไทย อายุ 50 ปี ภูมิลำเนา จ.นครราชสีมา
Chief complaint
• ปวดหัวเข่าขวา 16 ชั่วโมงก่อนมาโรงพยาบาล

3. Primary Survey
• A: patent airway, no tender along C-spine
• B: normal breath sound, equal on both lungs,
CCT negative
• C: BP 111/62 mmHg PR 102 /min, no active
bleeding
• D: E4M6V5, pupil 2 mm RTLBE
• E: - Right knee swelling, limit ROM, ballotment
positive, stepping at patella
- Abrasion wound at right elbow

4. Secondary survey
• S -เข่าขวาปวด บวม พอลุกเดินได้
• A -ไม่แพ้ยา ไม่แพ้อาหาร
• M -รับยาต่อเนื่องที่โรงพยาบาลจิตเวชประมาณ 3 ปี ไม่ได้นำยามา
• P -มีโรคประจำตัวทางจิตเวช
• L -last meal 14.00 น. 29/5/61
• E -15 ชั่วโมงก่อนมารพ. ขณะเดินไปเข้าห้องน้ำ ลื่นล้มเข่าขวา
กระแทกพื้น หลังจากนั้นลุกยืนได้ ยังไม่ปวด ต่อมาประมาณ 1
ชั่วโมงมีอาการปวดเข่าขวา สังเกตเข่าขวาบวมขึ้น เริ่มเดินได้
ลำบาก

5. • V/S : BP 111/62 P 102 RR24 T 36.7
• GA: A Thai man, good consciousness
• HEENT: not pale, no jaundice
• HEART: normal S1S2, no murmur
• LUNGS: Clear both lungs
• ABDOMEN: soft, no tender
• EXTREMITIES: Right knee swelling with
stepping at patella and marked tender, limit ROM
of right knee,

8. Management at ER
• On Cylindrical slab right leg
• Admit

9. Patella Fracture

• Film X-ray Knee AP/LAT
• MRI

13. Treatment
• Nonoperative = immobilization. This usually
involves placing the affected extremity in a
cylinder cast for 4-6 weeks.
▫ intact extensor mechanism (patient able to perform
straight leg raise)
▫ nondisplaced or minimally displaced fracture.

14. • Operative = ORIF with tension band construct,
partial patellectomy (severe comminuted lower
pole of patella), total patellectomy
▫ extensor mechanism failure (unable to perform
straight leg raise)
▫ open fractures
▫ fracture articular displacement >2mm

15. Complications
• Hardware prominence
• Stiffness
• Infection
• nonunion
