เด็กชายอายุ 6 ปี
อาชีพนักเรียน
ภูมิลาเนา อาเภอชุมพวง จังหวัดนครราชสีมา

3. Chief complaint
แขนซ้ายผิดรูป 4 ชั่วโมงก่อนมาโรงพยาบาล

4. Present illness
4 ชั่วโมงก่อนมาโรงพยาบาล ผู้ป่วยให้ประวัติลื่นตกจาก
เก้าอี้เอามือซ้ายยันพื้นไว้ จากนั้นมีแขนซ้ายผิดรูป ปวด
บวม ขยับไม่ได้ ไม่มีชาปลายมือ ไม่มีแผล

5. Past history
ปฏิเสธโรคประจาตัว
ปฏิเสธแพ้ยาแพ้อาหาร
ปฏิเสธประวัติผ่าตัด

- GA : good consciousness
- HEENT : not pale conjunctivae,
anicteric sclerae
- Heart : normal s1 s2
- Lungs : clear
- Abdomen : soft not tender

7. Physical examination

Deformity,tenderness,swelling
dimple sign
Limit ROM due to pain
Capillary refill <2 sec
Radial a. Pulse full
Neuro : intact

12. Diagnosis
Close fracture Left supracondyle

13. Supracondylar fracture
พบได้บ่อยที่สุดของกระดูกบริเวณข้อศอก มักพบช่วง
อายุ 5-10 ปี ส่วนใหญ่เกิดจากการใช้มือยันพื้น เป็นกระดูกหักที่
พบภาวะผิดรูป คือ cubitus varus ได้บ่อย แบ่งเป็น 2 ชนิด

14. 1) Extension type พบได้ 96% กระดูกจะหักในท่า
ข้อศอกเหยียด ชิ้นหักส่วนปลายมักเคลื่อนไปด้านหลัง
2) Flexion type พบน้อย กระดูกหักท่าข้อศอกส่วนหลัง
กระแทกพื้น ชิ้นหักส่วนปลายมักเคลื่อนที่ไปด้านหน้า
