De officiële definitie van osteoporose luidt: Osteoporose is een aandoening van het skelet gekarakteriseerd door verminderde botsterkte, waardoor iemand een verhoogd risico heeft op een fractuur.
Op deze afbeeldingen kunt u zien dat het gezonde, normale, bot bestaat uit dikke, met elkaar in verbinding staande trabeculae. Dit in tegenstelling tot het osteoporotische bot, dat dun is en waarvan een aantal verbindingen verbroken is.
Van belang is dat de ziekte niet langer beschouwd wordt als louter een probleem van de botdichtheid. Er zijn meer factoren die de botsterkte – en daarmee het fractuurrisico – bepalen: zoals botresorptie en de architectuur van het bot.
U kunt dit vergelijken met een stalen brug. Niet alleen de hoeveelheid staal maar juist ook de constructie ervan bepaalt de kwaliteit en de sterkte van de brug. Hetzelfde kan men zeggen over BMD en botarchitectuur.
Het is bij osteoporose vooral belangrijk om de gevolgen te bekijken: fracturen die snel achter elkaar kunnen optreden. Deze opeenvolging van fracturen is typerend voor osteoporose. Vanaf het optreden van de vroege, eerste perifere fractuur tot de eerste wervelfractuur en dan uiteindelijk de heupfractuur. De progressie van de ziekte vanaf het optreden van de eerste wervelfractuur kan snel gaan.

Het totale overzicht mbt effectiviteit
Patiënten die nieuw werden ingesteld op medicatie zijn gedurende een jaar gevolgd om de therapietrouw te kunnen bestuderen. Hier staat de persistentie (de patiënten die medicatie blijven ophalen) afgebeeld.
Na een jaar gebruikt minder dan de helft van de patiënten die nieuw is ingesteld op medicatie nog medicatie.
Abstract studie Netelenbos
Summary We analyzed 12-month compliance for all ten oral osteoporosis drugs in the Netherlands by medication possession ratio (MPR≥80%) in 105,506 patients, and persistence in 8,626 starters indicated high MPR (91%), low persistence (43%), and no restart in 78% of the stoppers after 18 months. Introduction We studied compliance and persistence for all available oral osteoporosis medications on a national scale in the Netherlands. Methods We analyzed the IMS Health’s longitudinal prescription database, which represents 73% of all pharmacies in the Netherlands. Twelve-month compliance was measured by medication possession ratio (MPR) in a cross-sectional cohort of 105,506 patients who received at least three prescriptions. Twelve-month persistence (no gap in refills for &gt;6 months) was measured in all 8,626 consecutive patients starting therapy, with a further follow-up in nonpersistent patients during an additional 18 months for evaluation of switching, restart, or definitive stopping oral medication. Multivariate logistic regression analysis was used to analyze the odds ratios (ORs) with 95% confidence intervals (CI) of characteristics of non-persistence. Results MPR of ≥80% was found in 91% of patients. Persistence was 43% (range, 29–52%). Persistence was related to age &gt;60 years (ORs, 1.41 to 1.64), pharmacy outside very dense urban area (ORs, 1.39 to 1.44), additional use of calcium and/or vitamin D supplementation (OR, 1.26 and CI, 1.13, 1.39) and use of glucocorticoids (OR, 0.65 and CI, 0.59, 0.72) or cardiovascular medication (OR, 0.88 and CI, 0.79, 0.97). Of non-persistent patients, 22% restarted within 18 months with oral osteoporosis drugs. Conclusions One-year compliance for all available oralosteoporosis medications was high, but 1-year persistence was low. Most stoppers did not restart or switch during an additional 18-month follow-up. These data indicate a major failure to adequately treat patients at high risk for fracturesin daily practice
Alendronate significantly decreased both NTx (1 month) and PINP (3 months). The suppression of bone turnover with alendronate was maintained through month 12. Conversely, teriparatide significantly increased both markers of bone remodeling. The increases of the bone formation marker, PINP, were vigorous and rapid (113% above baseline at 1 month), peaking at 6 months of treatment (251% increase above baseline. The increases in the bone resorption marker, NTx, were of far smaller magnitude (58% at peak), reached significance only after 3 months of treatment, and lagged behind those of PINP. Thus, the different effects of the two drugs on bone remodeling were evident after 1 month of treatment, and there were significant differences (P&lt;0.001) between the treatment groups in each marker at each time point (months 1,3,6,12).

There are several sources of variability in the meaurement of the markers: the rhythm is circadian, as with several other biomakers, there is a day to day variation, the measurement can be influence by diet, and by immobilisation.
As a consequence of that, morning and fasting urine samples are necessary, to control for the circadian rhythm and the influence of diet, and more than one sample to control for day to day variation.
It is obvious that this is one of the serious limitations of the markers: it is not easy to get repeated measurements of fasting urine samples from elderly osteoporotic women. The best way to control for that is of course to have age-and gender specific reference ranges.
Other sources of variability are uncontrollable: growth, age and sex, recent fractures, and diseases, eg RA or ankylosing spondylitis, and drugs, eg the use of glucocorticoids.
In 2005, Odvina was the first one to publish about atypical fractures in patients on longterm bisphosphonate therapy. Patient may fracture their femur by just rising up from a chair, that is impressive. Sometimes there are warning signals: pain, which may be difficult to interprete, because there are several causes of pain around the hip in the lederly, and a cortical thickening

Treatment with odanacatib 50 mg once weekly resulted in a progressive increase of almost 12% at 5 years in lumbar spine BMD. Discontinuation of treatment after 2 years resulted in BMD decreases, with values remaining slightly above their baseline values at Year 3 and reaching approximately their baseline level at Year 5. The rate of bone loss appeared to be faster in the initial 6 months following discontinuation of treatment.
