Trigonella foenum-graecum, commonly known as fenugreek, is an annual plant in the legume family. It has long, trifoliate leaves and white flowers. The dried seeds are yellow in color and contain steroidal sapogenins and various glycosides. Fenugreek has a variety of traditional medicinal uses including as an analgesic, to relieve constipation and stimulate breast milk production. Modern research has found it to have antioxidant, antitumor and blood sugar lowering effects. However, in large amounts it can cause side effects like diarrhea.

There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

- 4. has long stalked leaves up to 5 cm long stipules triangular, lanceolate, leaflets about 2.5 cm long, obovate , lighter serrate margin. - 5. The plant contains a number of steroidal sapogenins specially diosgenin. Glycosides like furastanol glycosides , hederagin glycosides also appear. - 6. Analgesic effects : In a rat study, Trigonella foenum-graecum extract showed analgesic activity. Antiadhesive properties: anti-adhesive properties against Helicobacter pylori have been studied. The anti-adhesive property was assessed by the inhibition of binding of H. pylori to stomach sections. - 7. Antioxidant activity: In rat study, an aqueous extract of fenugreek seeds prevented the rise in lipid peroxidation. Exercise recovery effects: In trained male cyclists, 4- hydroxyisoleucine isolated from fenugreek seeds significantly increased muscle glycogen concentration to make normal body function. Hepatoprotective activity: In an vitro study using liver cells treated with ethanol, a polyphenolic extract of fenugreek seeds significantly increased cell viability by reducing oxidation. 7sanjay kumar yadav 6th batch1/18/2020

Insulin sensitization effects: In rats study, 4- hydroxyisoleucine extracted from fenugreek seeds increased peripheral glucose utilization and decreased hepatic glucose production, thereby improving insulin resistance. Chronic ingestion of 4- hydroxyisoleucine significantly reduced insulinemia. Lipid-lowering effects: In animal studies, saponins extracted from fenugreek seeds lower the total cholesterol, and low density lipoprotein (LDL) levels. - 9. Amounts larger than normally found in food for up to 6 months. Side effects include diarrhea, stomach diseases, gas, and a “maple syrup” odor in urine. Fenugreek can cause nasal congestion, coughing,, facial swelling, and severe allergic reactions in hypersensitive people. Fenugreek can affect blood sugar levels in people with diabetes. Watch for signs of low blood sugar (hypoglycemia) and monitor your blood sugar carefully if you have diabetes and use fenugreek. 9sanjay kumar yadav 6th batch1/18/2020
