jednjaka na tipičnim mjestima.
• Javljaju se gotovo isključivo u odraslih.
• Kongenitalne divertikule se svrstava pravilnije u
duplikature jednjaka.
Tri su tipa divertikula:
• faringoezofagealni pulzijski divertikul
• epifrenički pulzijski divertikul
• parabronhijalni trakcijski divertikul
3. Uvod
• Pulzijski divertikuli nastaju kao posljedica
hernijacije sluznice jednjaka kroz slabiji dio
okolnih mišića.
• Trakcijski divertikul nastaje izvlačenjem stijenke
jednjaka što je posljedica upalnih promjena
izvan jednjaka.
• Najčešći upalni proces koji uzrokuje trakcijski
divertikul je upala traheobronhalnih limfnih
čvorova.
• Trakcijski divertikul se sastoji od svih slojeva
stijenke jednjaka.
4. Faringoezofagealni (Zenkerov)
divertikul
• Ludlow, 1769. g.; Zenker, 1874. g.
• Teorija: neusklađenost kontrakcije ždrijela i
relaksacije gornjeg dijela ezofagealnog sfinktera.
• Krikofaringealni mišić se zatvara prije nego što se
bolus protisne aboralno.
• Zbog kontrakcije konstriktora ždrijela stvara se
povišeni tlak u hipofarinksu.
• Povišeni se tlak prenosi na stijenku hipofarinksa.
• Sluznica hipofarinksa protrudira iznad
krikofaringealnog sfinktera straga u središnjoj liniji.
5. Zenkerov divertikul
• Postupnim rastom divertikul se spušta između
kralježnice i vratnog jednjaka prema dolje te pritišće
jednjak.
• Javljaju se simptomi visoke vratne disfagije uz
subjektivni osjećaj stranog tijela u grlu.
• Karakteristična je česta regurgitacija svježe, upravo
progutane hrane.
• Kod hijatalne hernije postoji regurgitacija već
probavljene hrane iz želuca.
• Česta je aspiracija hrane i tekućine.
• Prisutne su rekurentne ili kronične upalne promjene
na plućima.
6. Zenkerov divertikul
• Dijelovi hrane mogu dulje zaostajati zbog
čega postoji neugodan zadah iz usta.
• Disfagičke tegobe se postupno pojačavaju
tijekom godina.
• Na kraju postoji gotovo potpuna opstrukcija
prolaza hrane i tekućine.
• U početku ne dolazi do gubitka TT.
• Kako disfagičke tegobe napreduju, gubitak na
TT može biti jače izražen.

9. Dijagnoza
Postavlja se na temelju RTG snimke
jednjaka.
Endoskopski se pregled obavlja uvijek
poslije RTG snimke.
Endoskopija se obavlja radi isključivanja
karcinoma jednjaka.
10. Liječenje
• Liječenje je kirurško.
Pristupom kroz lijevu cervikotomiju se izvodi:
• resekcija divertikula ili divertikulektomija
• presijecanje mišićnih niti krikofaringealnog mišića i
mišića proksimalnog jednjaka (krikofaringealna
miotomija)
Kod manjih se divertikula može napraviti samo
divertikulopeksija za prevertebralnu fasciju uz
miotomiju.
• Kod divertikula manjih od 1 cm dovoljna je samo
krikofaringealna miotomija.
11. Liječenje
• U starijih osoba lošeg općeg stanja može se
izvesti endoskopska faringoezofagotomija.
• Uvodi se specijalni faringoskop u divertikul,
presijecaju se prednja stijenka divertikula i
zadnja stijenka jednjaka.
• Na taj način se stvara zajednička šupljina
divertikula i jednjaka koja onemogućava
nakupljanje hrane.
• Presijecanje se izvodi dijatermijom ili laserom.

14. Epifrenični divertikuli
• Smješteni su u distalnom dijelu jednjaka, češće s
desne strane, obično 5 do 10 cm iznad DES.
• Ako se nalaze u abdominalnom dijelu jednjaka,
onda obično polaze s prednje stijenke.
• 2 puta su češći u muškaraca.
• U više od 50% bolesnika postoje i ostale
pridružene abnormalnosti jednjaka, najčešće
ahalazija, difuzni spazam ili drugi poremećaji
motiliteta, hijatalna hernija i refluksni ezofagitis.
• Mnogi bolesnici nemaju nikakve simptome ili su
blago izraženi.
DES-donji ezofagealni sfinkter
15. Epifrenični divertikuli
• Najčešći simptomi su disfagija, bolovi u prsnom košu i
regurgitacija.
• Ponekad je teško razlučiti jesu li simptomi posljedica
divertikula ili udruženih lezija.
• Aspiracija i plućne komplikacije javljaju se rjeđe nego
kod faringoezofagealnih divertikula.
• Moguće je naći krvarenje i perforaciju, zatim kroničnu
infekciju zbog dugotrajnog zadržavanja i raspadanja
hrane u divertikulu.
• Dijagnoza se postavlja na temelju karakterističnog
RTG nalaza kojim se utvrđuju veličina i mjesto
divertikula.
16. Epifrenični divertikuli
• Endoskopijom se isključuje maligna bolest.
• U dijagnostici su važne ezofagomanometrija i pehametrija
jer utječu na izbor kirurškog zahvata.
• Liječenje je kirurško.
• Sastoji se od divertikulektomije lijevim transtorakalnim
pristupom.
• Sastavni dio operacije je i duga miotomija na suprotnoj
strani od vrata divertikula koja treba zahvatiti cijelu dužinu
tijela jednjaka u kojoj je manometrijski ustanovljen
poremećaj motiliteta.
• Ako postoji kila i refluksni ezofagitis, potrebno je napraviti
djelomičnu fundoplikaciju Belseyjevom metodom.
18. Parabronhijalni divertikuli
• Trakcijski divertikuli koji nastaju kao posljedica
upalnih promjena u regionalnim
traheobronhalnim limfnim čvorovima, najčešće
zbog TBC i histoplazmoze.
• Lezija se obično nalazi u visini karine dušnika.
• Otkrivaju se rutinskim RTG pregledima jednjaka
kontrastom.
• Prije toga obično nema nikakvih simptoma.
• Najvažnija komplikacija parabronhijalnih
divertikula je ezofagorespiratorna fistula,
najčešće s desnim bronhom ili dušnikom.
19. Parabronhijalni divertikuli
• U bolesnika s fistulom dolazi do paroksizmalnog kašlja
prilikom gutanja, posebno poslije uzimanja tekućine.
• Ezofagoskopija je obavezna.
• Ezofagoskopijom se isključuje karcinom kao najčešći
uzrok fistule.
• Parabronhijalni divertikuli ne zadržavaju hranu.
• Liječenje nije nužno, osim ako se ne pojavi fistula.
• Kirurško liječenje se sastoji od razdvajanja fistule,
resekcije divertikula te zatvaranja otvora na jednjaku i
bronhu odnosno dušniku.
