Membranous Nephropathy and Lupus Nephritis Class V involve thickening of the peripheral capillary loops with basement membrane 'spikes' and 'craters' visible on staining. Immunofluorescence shows deposition of IgG and C3 along the peripheral capillary loops. Electron microscopy reveals extensive foot process effacement and subepithelial electron-dense deposits that vary in appearance based on disease stage. The glomerular basement membrane may be attenuated or thickened. Lupus Nephritis Class V additionally shows mesangial expansion and a 'full house' immunofluorescence pattern with proliferative features sometimes present.
