Lipril (Lisinopril Dihydrate Tablets) is an
angiotensin converting enzyme (ACE)
inhibitor used for treatment of hypertension
in adults and pediatric patients 6 years of
age or more, adjunctive therapy for heart
failure and treatment of acute myocardial
infarction.

➢Hypertension: The initial adult dosage is 10
mg (monotherapy) or 5 mg (on a diuretic) once
daily. Titrate up to 40 mg daily based on
response.
➢Heart Failure: The initial adult dosage is 5
mg once daily. Increase dose as tolerated to
40 mg daily
➢Acute Myocardial Infarction(MI): Administer 5
mg within 24 hours of MI followed by 5 mg after
24 hours, then 10 mg once daily

to be stored at controlled room
temperature i.e. from 20°C to 25°C (68°F
to 77°F).
Protect from moisture, freezing and
excessive heat.

Angioedema of the face, extremities, lips, tongue,
glottis and/or larynx, including some fatal reactions,
have occurred in patients treated with angiotensin
converting enzyme inhibitors, including Lipril, at any
time during treatment.
Intestinal angioedema has occurred in patients
treated with ACE inhibitors. These patients
presented with abdominal pain (with or without
nausea or vomiting)
Two patients undergoing desensitizing treatment
with hymenoptera venom while receiving ACE
inhibitors sustained life-threatening anaphylactoid
reactions.

Sudden and potentially life-threatening
anaphylactoid reactions have been reported in
some patients dialyzed with high-flux
membranes and treated concomitantly with an
ACE inhibitor. In such patients, dialysis must be
stopped immediately, and aggressive therapy
for anaphylactoid reactions be started.
As with all vasodilators, Lisinopril should be
given with caution to patients with obstruction in
the outflow tract of the left ventricle.

Serum potassium must be monitored periodically in
patients receiving Lipril (Lisinopril Dihydrate Tablets).
Drugs that inhibit the renin-angiotensin system can
cause hyperkalemia. Risk factors for the development of
hyperkalemia include renal insufficiency, diabetes
mellitus, and the concomitant use of potassium-sparing
diuretics, potassium supplements and/or potassium-
containing salt substitutes.
In patients undergoing major surgery or during
anesthesia with agents that produce hypotension,
Lisinopril may block angiotensin II formation secondary
to compensatory renin release.
Monitor renal function periodically in patients treated
with Lipril Tablets. Changes in renal function including
acute renal failure can be caused by drugs that inhibit
the renin-angiotensin system.

Patients at risk of excessive hypotension,
sometimes associated with oliguria and/or
progressive azotemia, and rarely with acute renal
failure and/or death, include those with the following
conditions or characteristics: heart failure with
systolic blood pressure below 100 mmHg,
hyponatremia, high-dose diuretic therapy, recent
intensive diuresis or increase in diuretic dose, renal
dialysis, or severe volume and/or salt depletion of
any etiology.
ACE inhibitors have been associated with a
syndrome that starts with cholestatic jaundice or
hepatitis and progresses to fulminant hepatic
necrosis and sometimes death.

Contraindications
Lipril is contraindicated in patients with a
hypersensitivity to Lisinopril Dihydrate or any
other component of this medication.
It is also contraindicated in patients who have a
history of angioedema (hereditary or idiopathic) or
who have diabetes and are taking aliskiren.

➢Patients taking Lisinopril for the treatment of
hypertension may experience the following side effects:
➢Headache (3.8%), Dizziness (3.5%), Cough
(2.5%), Difficulty swallowing or breathing (signs of
angioedema), allergic reaction (anaphylaxis),
Hyperkalemia (2.2% in adult clinical trials), Fatigue
(1% or more), Diarrhea (1% or more).
➢ Some severe skin reactions have been reported
rarely, including toxic epidermal necrolysis and
Stevens-Johnson syndrome; causal relationship has not
been established.
➢People taking Lipril (Lisinopril Dihydrate Tablets) for
the treatment of myocardial infarction may experience
the following side effects: Hypotension (5.3%),
Renal dysfunction (1.3%)

for the treatment of heart failure may experience the
following side effects: Hypotension (3.8%),
Dizziness (12% at low dose – 19% at high dose),
Chest pain (2.1%), Fainting (5-7%), Hyperkalemia
(3.5% at low dose – 6.4% at high dose), Difficulty
swallowing or breathing (signs of angioedema),
allergic reaction (anaphylaxis), Fatigue (1% or
more), Diarrhea (1% or more)
➢Some severe skin reactions have been reported
rarely

16. Lisinopril Overdose

The most likely symptoms of Lisinopril
overdosage would be hypotension, for
which the usual treatment would be
intravenous infusion of normal saline
solution.
Lisinopril can be removed by hemodialysis

Lisinopril (Lipril tablets) has been classified by the
US FDA as Pregnancy Category D.
Use of medicines that act on the renin-angiotensin
system during the second and third trimesters of
pregnancy reduces fetal renal function and
increases fetal and neonatal morbidity and death.
Resulting oligohydramnios can be associated with
fetal lung hypoplasia and skeletal deformations.
Potential neonatal adverse effects include skull
hypoplasia, anuria, hypotension, renal failure, and
death.
When pregnancy is detected, discontinue Lisinopril
as soon as possible
