Clopivas Tablets by Cipla (Generic Clopidogrel Bisulfate Tablets)

Clopivas (Generic Clopidogrel Bisulfate Tablets) is a
platelet inhibitor and prevent blood clots in
coronary artery disease, peripheral vascular disease,
and cerebrovascular disease. It used to treat patients
with Acute coronary syndrome (unstable angina
(UA)/non-ST-segment elevation ACS (NSTEMI)&
ST-elevation myocardial infarction (STEMI) ), recent
myocardial infarction (MI), recent stroke, or
established peripheral arterial disease.

Acute coronary syndrome: In case of patients with non-ST-segment
elevation ACS [unstable angina (UA)/non-ST-elevation myocardial infarction
(NSTEMI)] including patients who are to be managed medically and those
who are to be managed with coronary revascularization, Clopivas has been
observed to decrease the rate of a combined endpoint of cardiovascular
death, myocardial infarction (MI), or stroke as well as the rate of a combined
endpoint of cardiovascular death, MI, stroke, or refractory ischemia. In case
of patients with ST-elevation myocardial infarction (STEMI), Clopidogrel
tablets have been observed to reduce the rate of death from any cause and
the rate of a combined endpoint of death, re-infarction, or stroke. The benefit
for patients who undergo primary PCI is unknown.
Recent myocardial infarction (MI), recent stroke, or established peripheral
arterial disease:
Clopivas (Clopidogrel Bisulfate tablets) has been observed to reduce the
combined endpoint of new ischemic stroke (fatal or not), new MI (fatal or
not), and other vascular death.

Active Ingredient
The active ingredient contained in Clopivas
tablets is Clopidogrel Bisulfate.
Each film-coated Clopivas tablet contains 75
mg or 300 mg of Clopidogrel Bisulfate.

Clopidogrel Bisulfate is available under
various drug trade names that include
Plavix, Clopivas, Ceruvin, Clavix, Clopilet,
Deplatt and Plagril.

Uses of Clopivas Tablets
Clopivas (Generic Clopidogrel Bisulfate Tablets) is used alone or
together with aspirin to lessen the chance of a heart attack or stroke. It is
given to patients with acute coronary syndrome (ACS), to patients who
have already had a heart attack, severe chest pain, or a stroke, or to
patients with other circulation problems (eg, peripheral arterial disease)
that could cause a stroke or heart attack. Acute coronary syndrome is a
condition where the blood flow to the heart is blocked.
A heart attack or stroke may occur when a blood vessel is blocked by a
blood clot. Clopidogrel is a platelet inhibitor. It reduces the chance that a
harmful blood clot will form by preventing platelets from clumping
together in the blood. Clopidogrel may also increase the chance of
serious bleeding in some people.

Clopivas Tablets Dosage
The dosage of Clopivas tablets (Generic Clopidogrel
Bisulfate Tablets) is given below:
Acute Coronary Syndrome:
Clopivas tablets can be administered with or without food.
For patients with non-ST-elevation ACS (UA/NSTEMI), initiate with a
single oral loading dose of Clopivas 300 mg and then continue at
Clopivas 75 mg once daily. Initiate Aspirin (75 to 325 mg once daily) and
continue in combination with Clopivas tablets.
For patients with STEMI, the recommended dose is Clopivas 75 mg
once daily orally, administered in combination with Aspirin (75 to 325
mg once daily), with or without thrombolytics. Clopivas may be initiated
with or without a loading dose.

Clopivas Tablets Dosage
The dosage of Clopivas tablets (Generic
Clopidogrel Bisulfate Tablets) is given below:
Recent MI, Recent Stroke or Established PAD:
The recommended dose is Clopivas 75 mg once daily
orally, with or without food.

Clopivas Tablets Contraindications
Clopivas tablets are contraindicated in patients with a
hypersensitivity to Clopidogrel Bisulfate or any other
component of this medication.
Clopivas is contraindicated in patients with active pathological
bleeding such as peptic ulcer or intracranial hemorrhage.
Clopivas is also contraindicated in patients with severe liver
impairment.

Storage Instructions
Clopivas (Generic Clopidogrel Bisulfate
Tablets) have to be stored at controlled
room temperature i.e. from 20°C to 25°C
(68°F to 77°F) with excursions permitted
between 15° to 30°C (59° to 86°F).
Keep this as well as all other medicines
away from children and pets.

Warnings
The annual incidence of hemorrhage may be increased by the administration
of Aspirin along with Clopivas tablets. Hence patients must not use aspirin
when undergoing Clopivas therapy. Use of non-steroidal anti-inflammatory
drugs is discouraged in patients taking Clopidogrel due to increased risk of
digestive tract hemorrhage.
The other precautions are
CYP2C19 inhibitors: Avoid concomitant use of Omeprazole or Esomeprazole.
Bleeding: Clopivas (Generic Clopidogrel Bisulfate Tablets) increases risk of
bleeding.
Discontinuation: Premature discontinuation increases risk of cardiovascular
events. Discontinue 5 days prior to elective surgery that has a major risk of
bleeding.

Warnings
Thrombotic thrombocytopenic purpura (TTP) has been reported.
Cross-reactivity among thienopyridines has been reported. Thienopyridines,
including Clopidogrel (Clopivas tablets), increase the risk of bleeding. In case
a patient has to undergo surgery and an antiplatelet effect is not desired,
discontinue Clopivas (Clopidogrel Bisulfate Tablets) five days prior to surgery
that has a major risk of bleeding.
In cases of confirmed isolated activated partial thromboplastin time (aPTT)
prolongation with or without bleeding, acquired hemophilia should be
considered.
Hypersensitivity, including rash, angioedema or hematologic reaction, has
been reported in patients receiving Clopidogrel Bisulfate (Clopivas tablets),
including patients with a history of hypersensitivity or hematologic reaction to
other thienopyridines.

Clopivas Tablets and Alcohol
The main side effect of Clopidogrel (Clopivas tablets) is bleeding
more easily than normal. You may have nosebleeds, heavier
periods, bleeding gums or bruising. You can drink alcohol with
Clopidogrel. But do not drink too much while taking Clopivas
tablets.
Alcohol doesn't affect Clopidogrel itself, but just be aware that
drinking alcohol can irritate your stomach lining. This could
increase the risk of getting a bleeding in the stomach.

Clopivas Tablets Side Effects
Bleeding, including life-threatening and fatal bleeding, is the most
commonly reported Clopidogrel Bisulfate (Clopivas tablets) side
effect. Another serious side effect is Thrombotic thrombocytopenic
purpura.
Some other side effects that were reported include
thrombocytopenia, leucopenia, neutropenia (including severe
neutropenia), granulocytopenia, anemia, cross-reactive drug
hypersensitivity among thienopyridines (such as ticlopidine,
prasugrel), paresthesia, dizziness, vertigo, abdominal pain, dyspepsia,
gastritis, vomiting, nausea, constipation, flatulence, gynecomastia,
glomerulonephritis, hematuria.

Clopivas Tablets Overdose
Platelet inhibition by Clopidogrel Bisulfate (Clopivas tablets) is
irreversible and will last for the life of the platelet. Overdose following
Clopidogrel administration can result in bleeding complications.
A single oral dose of Clopidogrel at 1,500 or 2,000 mg/kg was lethal
to mice and to rats and at 3,000 mg/kg to baboons. Symptoms of
acute toxicity observed in animals were vomiting, prostration,
difficult breathing, and gastrointestinal hemorrhage.
Based on biological plausibility, platelet transfusion may restore
clotting ability.

Clopivas Tablets During Pregnancy
Clopidogrel Bisulfate (Clopivas tablets) has been classified by
the US FDA as Pregnancy Category B.
Reproduction studies performed in rats and rabbits at doses up
to 500 and 300 mg/kg/day, respectively (65 and 78 times the
recommended daily human dose, respectively, on a mg/m2
basis), revealed no evidence of impaired fertility or fetotoxicity
due to Clopidogrel Bisulfate.
There are, however, no adequate and well-controlled studies
with clopidogrel in pregnant women. Because animal
reproduction studies are not always predictive of a human
response, Clopivas (Generic Clopidogrel Bisulfate Tablets)
should be used during pregnancy only if clearly needed.

Clopivas Tablets During Nursing
Studies in rats have shown that Clopidogrel (Clopivas tablets)
and/or its metabolites are excreted in the milk. It is not known
whether this drug is excreted in human milk.
Because many drugs are excreted in human milk and because of
the potential for serious adverse reactions in nursing infants from
Clopidogrel, a decision should be made whether to discontinue
nursing or to discontinue the Clopivas tablets, taking into account
the importance of the medicine to the mother.
