COMPARISON & FINDINGS OF BOTH CASES
 Refrigeration system to cool MIC was shutdown to
save cost. (Poor Decision)
 Leaking gas could be detoxified but scrubber
system was turned off due to maintenance
issue.(Poor preventive maintenance)
 Flare tower designed to burn off gases was turned
off due to some connecting pipes were removed for
maintenance and never fitted again. (Poor
maintenance)
 The MIC leak detection system was put off as it was
supposedly generating false alarms. Actually it was
signalling true alarms.(Poor knowledge)
 Water curtain was not high enough to quench the
MIC gas. (Poor design)
COMPARISON & FINDINGS OF BOTH CASES
 A 143 years old bridge was opened for public after
renovation without conducting the safety audit. (Poor
safety)
 The renovation was undertaken by an agency which
was not expert in performing such jobs. (Poor
decision)
 Lot of additional materials were added to the bridge
for strengthening without accessing its load bearing.
(Poor designing)
 The vital structure of bridge ‘Ropes’ which were
corroded was only painted. (Poor maintenance)
 No body knew how many people this bridge could
host at any given time. (Poor logistics)
SO WHAT IS BEHAVIOUR BASED SAFETY(BBS)?
Behavioural safety is the name given to a
number of types of programmes that aim to
improve safety by changing the behaviour
of workers.
Behavior Based Safety is a program
designed to influence employee actions
toward safer outcomes, ideally by
preventing an accident or injury before it
occurs. Implementing a behavior based safety
program is the most comprehensive way for
companies to promote safety, eliminate
hazards and prevent injuries.

 There is a 7-step process that can help in
improving Behavior-Based Safety in your
