YAWS
Yaws is a chronic infection that affects mainly the skin, bone and cartilage.
The disease occurs mainly in poor communities in warm, humid, tropical areas of Africa, Asia and Latin America.
The causative organism is a bacterium called Treponema pertenue, a subspecies of Treponema pallidum that causes venereal syphilis. However, yaws is a non-venereal infection.
About 75% of people affected are children under 15 years of age (peak incidence occurs in children aged 6-10 years); males and females are affected and no race is exempt.
Yaws is transmitted mainly through direct skin contact with an infected person.
A single skin lesion develops at the point of entry of the bacterium after 2–4 weeks. Without treatment, multiple lesions appear all over the body.
Overcrowding, poor personal hygiene and poor sanitation facilitate the spread of the disease.
The disease is rarely fatal; however, it can lead to chronic disfigurement and disability. Yaws can be treated with a single dose of a cheap and effective antibiotic: Benzathine Penicillin injection cures the disease.

2. SYPHILIS
• Syphilis is sexually transmitted disease caused spirochete called
treponema pallidum.
• The organism is very small with a length of 6 to 20 μm and a diameter
range of 18-20 μm.
• World health organisation estimated that more than 90% of the cases
are occurring in developing countries.
• Recently syphilis has re-emerged in several developed countries like
Russia, the united states, Canada, and England due to unsafe sex
practices.
3. Mode of spread
• Sexual contact- the organism usually enters the body through broken
skin or through the mucous membranes of the mouth, genitalia or
rectum during sexual contact. It can also be transmitted by kissing or
intimate contact with an infected person.
• Non sexual contact- in rare occasions syphilis can be acquired by
accidental inoculation or blood transfusion from an infected person.
The disease can also be transmitted from an infected pregnant female
to the foetus.
4. PATHOGENESIS
• The incubation period varies from 10-90 days(usually 21 days) before
the manifestation of the initial stages of syphilis. Syphilis is mainly
divided into three stages depending upon the period after which the
lesions appear, these are primary, secondary and tertiary.
• PRIMARY SYPHILIS- the typical lesion of primary syphilis is the
appearance of an ulcer called “chancre” usually associated with
regional lymph node enlargement. The ulcer is usually single, painless,
round and firm. The ulcer can be seen on ano-genital sites or at extra-
genital sites like mouth and lips.
• The chancre lasts 3 to 6 weeks , and it heals without treatment.
However, if adequate treatment is not administered, the infection
progresses to the secondary stage.
5. • SECONDARY SYPHILIS- this stage of syphilis
usually develops after 2-3 months following primary
syphilis. Secondary syphilis is the result of
inadequately or untreated primary syphilis. The
patients usually develop generalised lymph node
enlargement along with cutaneous lesions and skin
eruptions all over the body. The lesions are symmetric,
pale, red or pink, non pruritic , discrete, round that
measure 5 to 10 mm in diameter and are distributed on
the trunk and proximal extremities. Antibody tests are
always positive at this stage.
6. • TERTIARY SYPHILIS- this stage of syphilis usually
develops after 2-3 years following secondary syphilis.
The lesions of the tertiary syphilis are less infective
and the organisms can not be detected very easily. The
complications of tertiary syphilis are cardiovascular
syphilis, neuro syphilis etc.
