2. Patient profile: ผู้ป่วยหญิงไทย อายุ 40 ปี
อาชีพ ทำไร่อ้อย สิทธิ์ ประกันสุขภาพถ้วนหน้า
Chief complaint: ปวดหลัง 1 เดือน

3. Present illness
1 เดือน ก่อนมาโรงพยาบาล มีอาการปวดหลัง
บริเวณเอว ลักษณะอาการปวดแปล๊บ ร้าวลงขาและ
น่องด้านขวา ปวดขึ้นมาทันทีขณะโน้มตัวไปด้านหน้า
และยกของหนัก นอนพักแล้วอาการปวดดีขึ้น รักษา
โดยการใช้น้ำมันนวดบริเวณเอว อาการปวดไม่ดีขึ้น
ระดับความปวด 5/10 เดินได้ปกติ ไม่มีปวดกลางคืน
ไม่มีเบื่ออาหารน้ำหนักลด ปัสสาวะหรืออุจจาระปกติ

Vital signs: BP 122/84 mmHg P 80 bpm
T 37 c RR 20 bpm
GA: an active Thai woman
HEENT: not pale, no jaundice
Heart: normal S1 S2 no murmur
Lungs: normal breath sound no adventitious sound
Abdomen: soft not tender

9. Physical examinations
Extremities: right side antalgic gait,
no Trenderlenburg gait
Back: loss of lumbar lordosis
no mark pain on percussion, no trigger point
Limitation of flexion at 50 degree
SLR test positive at 50 degree, Laseque’s test positive
