2. PP: เด็กชายไทย
อายุ15ปี
Chief complaint: หลังผิดรูป 5 ปี
Present illness:
-5 ปีก่อน ผู้ป่วยสังเกตุว่าตอนนั่งมีหลังผิดรูป
หลังโก่งมากกว่าปกติ ไม่มีประวัติอุบัติเหตุ
กระทบกระแทก หลังค่อยๆผิดรูปมากขึ้น ไม่
มีปวดหลัง ปัสสาวะอุจจาระได้ปกติ ไม่เหนื่อย
ไม่มีอ่อนแรง ไม่มีเบื่ออาหารน้ำหนักลด

3. Past history
No underlying disease
No history of trauma
No prenatal/perinatal complication

4. Personal history
No drug/food allergy
No alcohol drinking
No smoking

5. Family history
No family of tumor
No family history of spine disease
No TB contract

6. Physical examination
V/S : T37.2, BP 110/78 mmHg, HR 86 bpm, RR 12/min
HEEENT : no pale conjunctiva, anicteric sclera
Heart : full regular pulse, normal s1s2 no murmur
Lung : clear equal both lungs
Abdomen : soft, not tender
Extremities : no defomity

7. Physical examination
Neuro :
good consciousness
pupil 3mm RTLBE, no facial palsy
motor grade 5 all extremities
DTR 2+ all
Sensory intact
cerebellar intact

17. Adolescent idiopathic scoliosis
Definition : Idiopathic scoliosis in children 10-18 years
Epidemiology : Incidence
3% for curve 10-20 degrees (F:M 10:1)
0.3% for curve >30 degrees F:M 1:1)
Right thoracic curve (most common)
Left thoracic curves are rare -> MRI R/O cyst or syrinx

Curve Progression
1. Curve magnitude
2. Remaining skeletal growth - younger age, Tanner
stage, Risser stage, peak growth velocity
3. Curve type - thoracic>lumbar, double curves>single
curve

Whole spine Standing PA,lat
Cobb angle : >10 degree = scoliosis
MRI indication
1. atypical curve pattern e.g. left thoracic curve
2. rapid progression
3. excessive kyphosis
4. structural abnormalities
5. foot deformities
6. asymmetric abdominal reflex (a/w syrinx)

22. Treatment
Non operative
Obeservation - cobb angle <25 + serial xray monitoring
Bracing - cobb angle 25-45 (to stop progression)

23. Treatment
Operative
Posterior spinal fusion - cobb angle > 45, gold standard
for thoracic/double major curve
Anterior spinal fusion - for thoracolumbar and lumbar
cases with normal saggital profile
Anterior/Posterior spinal fusion - for large curve (cobb
angle >75) , yonger age (to prevent asymmetrical growth
between anterior and posterior of spine)

Psudoarthrosis
