This document discusses the concepts of projects and project management. It begins with definitions of a project as a temporary endeavor undertaken to create a unique product or service. Key aspects of projects mentioned include them being limited in time, goal-centric, and impacting existing organizations. Examples of projects like building a house or developing a new product are provided. The document then discusses what constitutes a project team and management. It provides insights into evaluating different project models and highlights factors like control, discipline, and organization type that determine the best model. The rest of the document offers perspectives on project failures, principles of high reliability organizations, and practical tools for project planning like the work breakdown structure, Gantt chart, PERT planning and managing scope.

Project failure tends to be embedded in a project from the start. There is a spectrum of failures from complete collapse to a range of lesser failures associated with behind schedule and over budget. The reasons are all too well known. Yet the lessons from project failures are not being learned and the behaviours that give rise to failures continue to persist. Project failures will continue to occur until the reasons and behaviours are explicitly understood, acknowledged and addressed.
The reasons for project failure across project phases include:
Requirements
• Poor initial requirement definition
• Poor requirements validation
• Poor management of requirements
• Requirements not linked to business benefits
Solution Design
• Solution design not validated
• Solution design not linked to business needs
• Solution design too complex
• Solution design does not capture necessary complexity
• Solution design based on unproven technology
• Solution not implementable
• Underlying business processes not defined adequately
Estimation
• Errors due to limitations in estimating procedures
• Failure to understand and account for technical risks
• Deliberate underestimation/misrepresentation of costs
• Poor inflation estimates
• Top down pressure to reduce estimates
• Lack of valid independent cost estimates
Project Management
• Lack of program management expertise
• Mismanagement/human error
• Over optimism
• Schedule concurrency
• Program stretch outs to keep production lines open
• Lack of communication
• Poor management of change and scope creep
Development and Implementation
• Lack of competition when selecting suppliers, poor supplier selection process
• Poor supplier engagement
• Poor contract design
• Inconsistent contract management/administration procedures, too much or too little oversight
• Waste
• Excess profits by supplier, supplier overstaffed
• Supplier indirect costs unreasonable
• Inadequate resource allocation and prioritisation
• Organisation cannot handle change
Finance and Budgeting
• Business case incomplete
• Funding instabilities caused by trying to fund too many projects
• Funding instabilities caused by management decisions
• Inefficient production rates due to stretching out programmes
• Failure to fund for contingency
• Failure to fund projects at realistic cost
Presentation to ISACA Governance Event 11 Sept 2009 on factors affecting Project Success/Failure referencing ISO38500, Project Management, Standish and the CHAOS Report, also Sauer et all, Cadbury and drawing on Peter Salmon's varied and extnesive personal experience
Studies show that many projects either fail outright or fail to meet most of their objectives. There are a myriad of possible reasons why this might be the case. Very often, organizations go looking for a culprit and sometimes blame the project manager or even the very concept of project management itself. Sometimes they decide to “fix” the problem by getting all the project managers certified. Or they decide to standardize on a certain tool. And while certification and standardization are laudable things, they do not necessarily address the central problem or problems. This presentation will discuss the top ten reasons why projects fail and briefly discuss solutions to each problem. We will see how such areas as estimates, scope and “the accidental project manager” contribute to the problem.
Steve Jobs' LEAN STARTUP PROJECT MANAGEMENT: How Steve Jobs Planned, Organize...Rod King, Ph.D.
Steve Jobs' approach to creating extraordinarily successful products is shrouded in mystery. This presentation introduces a new framework, "Lean Startup Project Management", which can be used to explain Steve Jobs' innovation methodology which was largely intuitive. As the name implies, Lean Startup Project Management focuses on translating into reality ideas and principles from Eric Ries's bestselling book, "The Lean Startup." The core tool of Lean Startup Project Management is the One-Page Lean Startup which is a multilevel dashboard. Whereas existing approaches focus on using a tactical dashboard for Lean Startup Project Management, the One-Page Lean Startup uses 3 integrated dashboards: visionary, strategic, and tactical One-Page Lean Startup. Users end up saving time, money, energy, and lots of other resources. Experiment with the One-Page Lean Startup and provide us with your feedback.
Presentation for Agile Australia Conference 2013. Introducing Lean Startup concepts in a way accessible to people used to usual project management methods. With lean startup you don't assume you know the end state required, (as you do with a project), you assume you need to focus on learning to discover the end state to solve the problem you area you looking at.
Project – a [temporary] sequence of unique, complex, and connected activities having one goal or purpose and that must be completed by specific time, within budget, and according to specification.
Project management – the process of scoping, planning, staffing, organizing, directing, and controlling the development of an acceptable system at a minimum cost within a specified time frame.
Process management – the activity of documenting, managing, and continually improving the process of systems development.
Any project involves planning, scheduling and controlling a number of interrelated activities with use of limited resources, namely, men, machines, materials, money and time.
A project is a temporary endeavor undertaken to create a unique product or service.
It is
performed by people
constrained by limited resources
planned, executed and controlled
It is required that managers must have a dynamic planning and scheduling system to produce the best possible results and also to react immediately to the changing conditions and make necessary changes in the plan and schedule.
Projects differ in size, scope, cost and time, but all have the following characteristics:
solves a real and existing problem, has well-defined start and end points.
A life cycle involving a series of phases in between the beginning and end
A budget, Specific goals and conditions, Defined responsibilities
A set of activities which are sequential, unique and non-repetitive
Use of resources which may require coordinating
Unique: All projects are unique in themselves, no two projects are exactly similar.
Defined roles and relationships for participants in the project
Fixed set of objectives: A project has a set of objectives or a mission. Once the objectives are achieved the project is treated as completed.
Tenure: Project is never a continuous activity, it has to come to an end. Its life span is fixed.
Team work: : Project is a team work and it normally consists of diverse areas. It needs a team to accomplish various activities.
A project requires to have systematic flow where all the elements interact and cohere to deliver the desired goal. There are many aspects involved in the project process but one of the most oblivious I importance is the definition of the objectives.
Defining objectives guides the direction that the process will take.
It also helps identify the resources that are necessary to achieve the said objectives. As such, it is paramount that the objectives of the project always remain in sight at all times.
Management : is the process of using organizational resources to achieve organizational goals through Planning, Organizing, Controlling and leading.
It is thus, a set of activities directed at an organization’s resources with the aim of achieving organizational goals in an efficient and effective manner.
A brief presentation to entire project team has been given with the objective to improve awareness regarding project management as a subject and how, different processes and knowledge areas plays a vital role in managing projects efficiently. Presentation also focused on importance of communication and the traits of a successful project manager.
A full course I developed based on about ten years of experience in project management in IT projects in the Netherlands and non-profit media projects in Kenya.

Enterprise Excellence is Inclusive Excellence.pdfKaiNexus
Enterprise excellence and inclusive excellence are closely linked, and real-world challenges have shown that both are essential to the success of any organization. To achieve enterprise excellence, organizations must focus on improving their operations and processes while creating an inclusive environment that engages everyone. In this interactive session, the facilitator will highlight commonly established business practices and how they limit our ability to engage everyone every day. More importantly, though, participants will likely gain increased awareness of what we can do differently to maximize enterprise excellence through deliberate inclusion.
What is Enterprise Excellence?
Enterprise Excellence is a holistic approach that's aimed at achieving world-class performance across all aspects of the organization.
What might I learn?
A way to engage all in creating Inclusive Excellence. Lessons from the US military and their parallels to the story of Harry Potter. How belt systems and CI teams can destroy inclusive practices. How leadership language invites people to the party. There are three things leaders can do to engage everyone every day: maximizing psychological safety to create environments where folks learn, contribute, and challenge the status quo.
Who might benefit? Anyone and everyone leading folks from the shop floor to top floor.

What is a project?
The tunnel boring machines were
specially designed for
excavating the chalk marl rock which
geological surveys had shown to lie
beneath the seabed along the
proposed tunnel route. However,
several factors combined to make
this a unique challenge for
the machines:
•The length of the tunnel to be
excavated undersea ( in
excess of 20 km ).
•The high rate of advance
required to meet the
construction programme
requirements.
•The possibility of unexpected
ground conditions - such as
an unsurveyed borehole.
At completion, it was estimated that the whole project
cost around £10 billion.
Work stated on both sides in 1987, and the fixed link was
opened in 1994 - nearly 2 years late, and way over
budget.

What is succes in project management?
Answer: do not fail !
But failure is the outcome of a human process
All failures are “expectation failures” = the
failure to meet the expectations of the
stakeholders
27. pag. 27Jan Devos
Organizing for High Reliability: Processes of
Collective Mindfulness (Weick, 1999)
• Preoccupation with failure (“Failure is not an option”)
• Reluctance to simplify interpretation
(beware of ‘frameworks’, ‘models’, ‘mindsets’, …)
• Sensitivity to operations (“situational awareness”)
• Commitment to resilience (“continuous management
of fluctuations”)
• Under specification of structures (“no bureaucracy”)
28. pag. 28Jan Devos
Lessons from HROs
• The expectation of surprise is an organizational resource
because it promotes attentiveness and discovery
• Anomalous events should be treated as outcomes rather
than as accidents, to encourage search for sources and
causes
• Errors should be made as conspicuous as possible to
undermine self-deception and concealment
• Reliability requires diversity, duplication, overlap, and a varied
response repertoire, whereas efficiency requires
homogeneity, specialization, non-redundancy and
standardization
• Interpersonal skills are just as important in HROs as are
technical skills
32. pag. 32Jan Devos
Work Breakdown Structure (WBS)
• Break your assignment down in manageable tasks
• Assign a deliverable to each task
• Define each task with clear objectives
• Assign a execution time to each task
• Assign a cost to each task
• Assign resources to each task
• Tasks who are not in the WBS are out-of-scope
Do a stepwise refinement of some tasks in the WBS
33. pag. 33Jan Devos
Define the goals according to the SMART principle
S Specific / Significant / Simple
M Measurable / Manageable
A Achievable / Appropriate / Attainable
R Realistic / Relevant
T Timely / Trackable

GANTT chart (H.L. Gantt (1861-1919) – assistant of F. Taylor)
• WBS = set of all tasks
• Presentation of all tasks on a time line
• Beginning and Ending time
• Linking of tasks (concurrent tasks are possible)
PERT planning (Program Evaluation and Review Technique)
• Origin: US DoD (1958)
• Extra: TOTAL time needed to finished the project
• Critical path = the longest path of planned tasks to the
end of the project, and the earliest and latest that each task
can start and finish without making the project longer
