Chronic diarrhea as a result of colonic fistulas -two case reports with different origin. When it comes to chronic diarrhea symptom, the first thing
one thinks of is never a surgical cause, but an infectious disease. The aim of this paper is to show 2 different cases of chronic diarrhea, resulting from
benign surgical causes - colonic fistula. The first case is a result of cholecystocolic fistula, while the second is the result of gastrojejunocolic fistula.
Colonic fistulas originate from different causes: malignancy, NSAID, diverticulosis of the colon, cholecystitis, pancreatitis, lymphoma, or after radiation
therapy. They can also result from a trauma, which can be post-surgical.
Introduction:
Cholecystocolic fistula occurs as a result of the inflammation of the gallbladder. It arises from existing adhesions. The incidence rate
is not high, but the complication is not a rarity per se. It is less frequent complication than cholecystoduodenal fistula. The main symptoms are secretory
diarrhea, vitamin K malabsorption and weight loss, and thus suspicion of malignancy is usual. The treatment is surgical removal of the gallbladder,
fistula and part of the colon en bloc.
Case report:
A 73-year old male patient was admitted to the department after 5 months of medical treatment. Laboratory tests, coproculture,
colonoscopy, abdominal ultrasonography, and gastroduodenoscopy were performed - the diagnosis was not established. The diagnosis was made by
means of irrigography and short and narrow cholecystocolic fistula was confirmed. The possibility of malignant disease was not completely excluded.
The patient underwent surgery after parental nutrition-adhesions, gallbladder, and the prepared fistula were removed as well as the longitudinal part
of the transverse colon, which was simultaneously repaired. Ex-tempore diagnosis-the surgical specimen originated from inflammation, not from
malignancy. The post-operative course was uneventful. The first post-operative stool was normal. The patient gained some weight after a few months.
Conclusion:
Along with the contemporary diagnostics methods, contrast examination plays an important diagnostic role. When infection is
excluded as the cause of chronic diarrhea, cholecystocolic fistula should be considered. Malignant disease should be excluded before the surgery, or it
may be diagnosed during the surgery, which would determine the course of the treatment. The treatment of benign cholecystocolic fistula is surgical
en bloc procedure.
Introduction: Diffuse Large B Cell Lymphoma (DLBCL), as a set of heterogeneous aggressive lymphoma, most commonly originated in the germinal center B lymphocytes. Rare cases were from peripheral blood B cell (outside germinal center) or from the inert lymphoma development and transformation. And such tumor with originality outside germinal center was seldom seen in the literature. Even the tumor in combination with chronic gastritis has not been reported before.

Introduction
Cutaneous metastases from abdominal malignancies are rare and have been reported in less than 5% of patients [1]. Furthermore, metastases in patients suffering from colorectal neoplasia are even rarer entity. Tan et al, among 2538 of the new cases of colorectal cancer over the period of 6 years, reported only 3 cases (0.1%) with cutaneous deposits [1]. Presentation varies from cutaneous or subcutaneous small nodules, rash or large fungating lesions [1-4]. Inevitably, their presence implies the disease progression, and poor prognosis with the reported survival between 1 to 34 months [2,3,5].
We report a case of an elderly patient who initially was thought to present with a simple skin infection. Subsequently, the patient was diagnosed with the moderately differentiated mucinous adenocarcinoma and required the right hemicolectomy. This case highlights that a high index of suspicion is recommended in an unresolving skin erythema.

Case 409: ECTOPIC FASCIOLIASIS, Dr LÊ ĐÌNH VĨNH PHÚC
1. CASE 409: ECTOPIC FASCIOLIASIS, LÊ ĐÌNH VĨNH PHÚC,
PHẠM CHÍ TOÀN, VÕ NGUYỄN THÀNH NHÂN, MEDIC
MEDICAL CENTER, HCMC, VIETNAM
Female patient, 31 year-old, accountant, in Gia Lai province. Onset 2 months with
scattered bodyitching, no skin lesions, no fever, no abdominal pain. A private
clinic in Gia Lai, with blood tests, diagnosed Toxocara sp infection, and gave her
albendazole 800mg/day x 21 days. No itching, she went down to an another
private clinic in Quy Nhon province, and with another blood tests, she
was diagnosed infected Cysticercosis, treated with albendazole 800mg/day x 10
days. Then appearing right abdominal pain, ultrasound suspected liver damage
caused by Fasciola spp. She went to Institute of Parasitology and Entomology in
Quy Nhon province and then came to Medic Hoa Hoa in Ho Chi Minh city.
Ultrasound detected right liver lesion with mixed echo, d = 5 cm, clear border,
within a few hypoechoic nodules, and no liver tissue edema around. (Fig 1, 2).
Further ultrasound detected colon wall thickness at liver region, hypoechoic, not
lumen narrowing. (Fig 3).
Endoscopyshowed transverse colitis.
2. Blood tests: WBC 14,500 cells/mm3 (Neutrophil 61.9%, Eosinophil 15.8%),
hsCRP 14.53 mg/L. HBsAg (-), antiHCV (-), AFP (-), CEA (-), Fasciola sp IgG
(+), stoolexam (-).
Biopsy tissue in colon lesion was done and microscopic report was eosinophil
mucosa colitis.
MSCT CE presented liver lesion d = 4x6cm and transverse colon lesion with wall
thickness d = 20mm (Fig 5, 6).
3. We diagnosed: liver abscess and pseudotumor colitis by Fasciola spp (Ectopic
Fascioliasis), treated with Triclabendazol 10mg/kg/day x 2 days.
Re-examination 4 weeks later, WBC 8,800 cells/mm3 (Eosinophil 2.5%), hsCRP
1.3 mg/L.
Liver lesion in ultrasound and MSCT, wall thickness d = 8mm in MSCT (Fig 8, 9).
4. We represented an ectopic Fascioliasis case with hepatic and transverse colon
lesions and an undifferential serodiagnosis. Endoscopic biopsyresult helped ruling
out a colon tumor. But based on ultrasound findings of liver and colon lesions
which were confirmed by MSCT we could chosed a concordantdiagnosis for this
case.
