Definisi
• Chronic Obstructive Pulmonary Disease (COPD), a
common preventable and treatable disease, is
characterized by persistent airflow limitation that is
usually progressive and associated with an enhanced
chronic inflammatory response in the airways and the
lung to noxious particles or gases.

severity in individual patients.

Diagnosis
• A clinical diagnosis of COPD should be considered
in any patient who has dyspnea, chronic cough or
sputum production, and/or a history of exposure to
risk factors for the disease.
• Spirometry is required to make the diagnosis; the
presence of a post-bronchodilator FEV1/FVC <
0.70 confirms the presence of persistent airflow
limitation and thus of COPD.

Chronic Systemic Inflammation Syndrome
To be diagnosed with CSIS (chronic systemic inflammatory
syndrome), a patient would need to have three of the
following diagnostic components: aged over 40 years;
smoking for more than 10 pack-years; symptoms and

heart failure; metabolic syndrome; or increased
reactive C-protein

PPOK dan penyebab kedua kematian (sekitar 20-25%) pada

• Penelitian lung health berhasil menemukan korelasi antara
FEV1 dengan morbiditas, dikatakan bahwa penurunan 10%
dari fungsi paru (FEV1) pada pasien PPOK terkait dengan
peningkatan resiko kematian penyakit kardiovaskuler
sebesar 30%

• Hubungan antara PPOK dan PJK didasari oleh
adanya inflamasi sistemik, yang ditandai dengan
peningkatan penanda inflamasi sistemik seperti
CRP dan TNF alpha. Peningkatan derajat PPOK
berbanding lurus dengan peningkatan inflamasi
sistemik yang terjadi dan inflamasi tersebut
diperkirakan menyebabkan peningkatan
aterosklerosis

• Mekanisme lain yang menghubungkan PPOK dan
penyakit kardiovaskuler adalah hipoksia
• Hipoksia dapat menyebabkan respon inflamasi
abnormal. CRP juga meningkat pada kondisi
hipoksia. Selain itu hipoksia dapat menyebabkan
stres oksidatif. Hipoksia menyebabkan
aterosklerosis dan terjadi peningkatan
peroksidasi lipid, sebuah penanda stres oksidatif.

• Komorbiditas PJK merupakan penyebab utama
mortalitas dan morbiditas pada pasien PPOK. Atas
dasar itu maka pengobatan terhadap keduanya wajib
dilakukan secara simultan dan komperhensif
• Saat ini pengobatan PPOK efektif terhadap eksersebasi
tetapi tidak mengubah riwayat perjalanan alamiah
penyakit ini. Sedangkan pengobatan pada PJK terbukti
lebih berhasil dibandingkan dengan pengobatan pada
PPOK

• Terdapat suatu analisis retrospektif yang
membuktikan bahwa pengobatan penyakit
penyerta kardiovaskular pada PPOK, dapat
menurunkan morbiditas, rawat inap dan
kematian pada pasien dengan PPOK

• Statin memiliki efek pleiotropik selain efek penurunan
lipid, termasuk anti-inflamasi, antioksidan,
antithrombogenik dan kemampuan pemulihan fungsi
pembuluh darah. Statin memiliki efek yang
menguntungkan pada pasien PPOK dengan mengurangi
komplikasi kardiovaskular. Tinjauan sistematik dari
berbagai penelitian menyimpulkan bahwa pengobatan
statin pada pasien dengan PPOK dapat mengurangi
morbiditas dan/ atau kematian

• Beta bloker merupakan terapi lini pertama pada pasien
dengan PJK. Sayangnya, diagnosis PPOK menyebabkan
rendahnya pemakaian beta bloker pada pasien yang
membutuhkannya karena pertimbangan kontraindikasi
dan kekuatiran timbulnya efek samping dan
bronkospasme. Sebenarnya, beta bloker ditoleransi
dengan baik pada pasien penyakit jantung dengan
komorbiditas PPOK, dan pada mayoritas tidak ada bukti
terjadinya pemburukan gejala respirasi ataupun
penurunan FEV1

underdiagnosed and associated with poor health status
and prognosis.
• ECLIPSE, Depression was more prevalent among
subjects with COPD (26 percent), than smokers without

associated with worse health and functional status.

• Penurunan massa sel tubuh merupakan
manifestasi sistemik yang penting pada PPOK.
Perubahan massa sel tubuh diketahui melalui
penurunan berat badan dan penurunan massa
bebas lemak. Massa bebas lemak yang hilang
mempengaruhi proses pernapasan, fungsi otot
perifer, kapasitas latihan, dan status kesehatan.
Penurunan berat badan mempunyai efek negatif
terhadap prognosis penderita PPOK.

• Kaheksia sering ditemukan pada penderita PPOK dan
angka kejadiannya meningkat sesuai derajat PPOK.
Pada PPOK berat dengan gagal napas didapatkan angka
kejadian kaheksia mencapai 50%, sedangkan pada
penderita dengan PPOK ringan sampai sedang
prevalensinya mencapai 10-15%. Secara keseluruhan
angka kejadian kaheksia pada PPOK sekitar 30-40% dan
dihubungkan dengan prognostik yang buruk pada
pasien

• Serious infections, especially respiratory infections, are
frequently seen in patients with COPD.
• There is no evidence that infections should be treated
differently in the presence of COPD. However, repeat
courses of antibiotics for exacerbations may increase
the risk for the presence of antibiotic resistant bacterial
strains and more extensive cultures of serious
infections may be warranted.

khususnya sitokin proinflamasi TNF-α dan IL-6
dapat menimbulkan resistensi insulin serta
meningkatkan risiko DM dan Sindroma

31. Kesimpulan
• Kebanyakan panduan mengabaikan fakta bahwa
kebanyakan individu dengan penyakit kronis
memiliki satu atau lebih komorbiditas. PPOK,
gagal jantung kronis, peripheral artery disease,
diabetes atau kanker dapat memiliki dampak kuat
terhadap penderita penyakit kronis, terutama
pada geriatri. Sehingga dibutuhkan pendekatan
yang lebih menyeluruh terhadap pasien-pasien
ini dengan memperhatikan adanya CSIS

Systemic effects and comorbidities of chronic obstructive pulmonary disease (COPD). Peripheral lung inflammation may cause a ‘‘spill-over’’ of cytokines, such as interlukin (IL)-6, IL-1b and tumour necrosis factor (TNF)-a, into the systemic circulation, which may increase acute-phase proteins such as C-reactive protein (CRP). Systemic inflammation may then lead to skeletal muscle atrophy and cachexia and may initiate and worsen comorbid conditions. Systemic inflammation may also accelerate lung cancer. An alternative view is that systemic inflammation causes several inflammatory diseases, including COPD. CS1
