PEG is a surgical procedure where a feeding tube is placed through the abdomen into the stomach using an endoscope, avoiding open abdominal surgery. It is used for patients unable to eat for a prolonged period. A gastrostomy is created through the skin and stomach using the endoscope to guide placement of the feeding tube. PEG is performed to provide nutrition to patients who cannot swallow food and is done by physicians like surgeons, ENT specialists, radiologists or gastroenterologists, often in outpatient settings. It has advantages over surgical gastrostomy in taking less time, carrying less risk and costing less.

PRINCIPLE OF SUTURE REMOVAL
Suture area is first clean with normal saline. The suture is then pulled towards the incision line to prevent dehiscence. If suture entrapped in a scab, application of Hydrogen peroxide/normal saline is necessary
If pieces of suture left are left out infection may occur. RECOMMENDED

How Injuries Occur
An upper GI endoscopy or EGD (esophagogastroduodenoscopy) is a procedure to diagnose and treat problems in your upper gastrointestinal tract( esophagus, stomach and first half of duodenum).. A patient may be fully conscious or sedated during the procedure.This procedure is done using a long, flexible tube called an endoscope.
The tube has a LED light and video camera on one end. The tube is put into your mouth and throat. Then it is slowly pushed through your esophagus and stomach, and into your duodenum. Video images from the tube are seen on a monitor.

POEM is a minimally invasive procedure that uses a high definition endoscope (a flexible tube with a mounted camera at end) to help treat certain swallowing disorders and stands for Peroral Endoscopic Myotomy.
Who cannot undergo this procedure?
Although POEM is a safe and effective procedure, it may not be a suitable option in certain cases. People with any of the following conditions are generally not recommended to undergo POEM for achalasia:
1. Disorders of coagulation
2. History of any kind of therapy that is likely to compromise the integrity of the mucosa of the esophagus or lead to fibrosis of the submucosa for e.g. radiation for cancer treatment, endoscopic mucosal resection, or radiofrequency ablation
3. Liver cirrhosis with portal hypertension
4. Severe erosive esophagitis
When should it be done?
POEM is generally indicated for treating the following disease:
1. Achalasia: It is a medical condition that affects the muscles of the esophagus. The lower esophageal sphincter (LES), a muscular valve present between the esophagus and the stomach fails to relax during swallowing in this condition.
2. Diffuse esophageal spasm: Esophageal disorders of muscle spasm that are unresponsive to medical therapies
3. Nutcracker esophagus also known as hypertensive peristalsis is a muscle spasm disorder which needs surgical correction.
PEG tubes are becoming increasingly in demand for alternative enteral feeding options. Thus, BBS is a common complication that one should be aware of and how to manage it. Hope this helps..

Percutaneous endoscopic gastrostomy (PEG) is a surgical
procedure for placing a tube for feeding without having to
perform an open operation on the abdomen (laparotomy). It
is used in patients who will be unable to take in food by
mouth for a prolonged period of time. A gastrostomy, or
surgical opening into the stomach, is made through the skin
using an a flexible, lighted instrument (endoscope) passed
orally into the stomach to assist with the placement of the
tube and secure it in place.
3. The purpose of PEG
The purpose of a percutaneous endoscopic gastronomy is to
feed those patients who cannot swallow food. Irrespective of
the age of the patient or their medical condition, the
purpose of percutaneous endoscopic gastronomy is to
provide fluids and nutrition directly into the stomach.
4. The purpose of PEG
Percutaneous endoscopic gastronomy is done by a physician.
The physician may be a general surgeon, an otolaryngologist
(ENT specialist), radiologist, or a gastroenterologist
(gastrointestinal specialist).
5. The purpose of PEG
PEG is performed in a hospital or outpatient surgical facility.
It is not necessary to perform a percutaneous endoscopic
gastronomy in an operating room.
6. Advantages
Percutaneous endoscopic gastronomy takes less time, carries
less risk and costs less than a surgical gastrostomy which
requires opening the abdomen. Percutaneous endoscopic
gastronomy is a commonly-performed so there are many
physicians with experience in performing the procedure.
When feasible, percutaneous endoscopic gastronomy is
preferable to a surgical gastrostomy.
