Gram positive
Non-acid fast
Non-motile
Carynebacterium diphtheria
Bacterial biotypes:
 Mitis
 Intermedius
 Gravis
 Belfanti
 The period between exposure to
an infection and the appearance
of the first symptoms is called
“incubation period”.
 Incubation period is 3-4 days.
TRANSMISSION:
Transmitted mainly
 By droplet infection.
 From infected skin lesions.
 By cuts, wounds of affected people who
are not treated properly.
 By objects.
The organism usually infects the
epithelium of the skin and mucosa of
the upper respiratory tract, leading to
inflammation.
Nose and larynx are common for
infection.
Corynebacterium
diphtheria
Exotoxin
Interferes with cellular
protein synthesis
Tissue necrosis
Chain A
Chain B
Attempts to remove pseudomembrane results in
Bleeding
Paralysis of palate and hypopharnynx
Paralysis in combination with inflammation and
edema of upper respiratory tract can leads to
“AIRWAY OBSTRUCTION”
Systematic spread of the toxin can cause injury
to heart, kidney and nervous tissue.
Symptoms before the pseudomembrane appears include:
 Fever and weakness
 Enlarged lymph nodes on the neck
 Swelling of soft tissue on neck
 Nasal discharge
 High heart rate
 Sore throat
 Difficulty swallowing
 Difficulty breathing
Symptoms after pseudomembrane formation
 Nausea and vomiting
 Chills, headache
 fever
Doctors may suspect diphtheria in a sick child who has a sore
throat with pseudomembrane covering the tonsils and throat.
In suspection of diphtheria treatment begins immediately even
before the results of bacterial tests available.
Diphtheria treatment today involves:
 Using diphtheria antitoxin to stop the poison
(toxin) produced by the bacteria from
damaging the body.
 Using medicines (called antibiotics) to kill and
get rid of the bacteria.
 Antitoxin used is horse serum.
 Antibiotics used are:
• PENICILLIN
• ERYTHROMYCIN
 The best way to prevent diphtheria is to get ”vaccinated”
The diphtheria vaccine is combined with
vaccines for tetanus and whooping cough
known as “DPT VACCINE” .
DTaP vaccine - children
 TdaP vaccine - adults.
Vaccination consists of a series of five shots
2 months
4 months
6 months
15 to 18 months
4 to 6 years
Side effects of DPT vaccine
Soreness
Redness or swelling at the injection site
Slight fever
Allergic reaction
