This is a presentation of a case of fracture shaft of humerus in an adult with bad local skin conditions in the form of abrasions and CLWs which is a relative contra indication for open surgery. He was treated with closed reamed humerus nailing and the patient just came once for follow up for his hand fixator removal. He was so comfortable doing all the activities that he came for follow up only after two years that too for implant removal. Here are the function at 2 years as well as the steps of implant removal to avoid damage to the rotator cuff.

The hip joint is a ball and socket joint consisting of the femoral head and acetabulum. This articulation provides multiple planes of movement and is highly congruent. Articular cartilage, consisting of type II collagen, covers the majority of the femoral head. The acetabulum peripherally consists of articular cartilage while the central floor is non-articular and filled with a fatty layer termed the pulvinar. The ligamentum teres arises from both the transverse acetabular ligament and the central non-articular layer of the acetabulum and attaches to the central femoral head. It may play a role in stabilizing the hip joint.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

4. PORTALS
• POSTERIOR: entry point is approximately 2 to
3 cm inferior and 1 to 2 cm medial from the
posterolateral acromial angle

5. PORTALS
• Antero-supirior and antero-inferior :
• Two anterior portals are commonly used.
• Anteroinferior portal is typically the working portal and is
placed just proximal to the subscapularis tendon.
• Anterosuperior portal is an accessory portal that may be used
for viewing or shuttling of sutures and is placed directly above
or just anterior to the biceps tendon.

6. PERFORM DIAGNOSTIC ARTHROSCOPY STEP BY STEP:
FROM POSTERIOR PORTAL
1. Biceps tendon and superior labrum
2. Posterior labrum and posterior capsule recess
3. Inferior axillary recess and inferior capsular insertion to
the humeral head
4. Inferior labrum and glenoid articular surface
5. Supraspinatus tendon of rotator cuff
6. Posterior rotator cuff insertion and bare area of the
humeral head
7. Articular surface of humeral head
8. Anterior-superior labrum, superior and middle glenohumeral
ligaments, and subscapularis tendon
9. Anterior-inferior labrum
10. Anterior-inferior ligament

FROM ANTERIOR PORTAL
11. Posterior glenoid labrum and capsule insertion into the
humeral head
12. Posterior rotator cuff including infraspinatus and the
supraspinatus tendons
13. Anterior glenoid labrum and inferior glenohumeral
ligament
attachments to the humeral head
14. Subscapularis tendon and recess and middle glenohumeral
ligament attachment to the labrum
15. Anterior surface of humeral head with subscapularis
attachment and biceps tendon passage through the
rotator
