PORTFOLIO DAY ACCOMPLISHMENT REPORT
S.Y. 2021-2022 1ST
GRADING PERIOD
I. INTRODUCTION
The Buraburon National High School followed modalities and General Guidelines and
Considerations of Distance Learning based on Memorandum DM-CI-2020-00162 and the
DepEd Order No. 29, s. 2021, re: School Calendar and Activities for School Year 2021-2022.
The school together with the faculty and staff organized the conduct of the said activity
and followed the minimum safety and health protocols set by the local IATF.
II. OBJECTIVES
The following activities were suggested to be carried out:
a) Parent-Teacher-Adviser Conference and Feedback Giving
b) Showcase of Learners’ Outputs and Performances
c) Distribution of Learner’s Progress Report Cards
III. HIGHLIGHTS OF ACCOMPLISHMENTS

Focus Areas Issues and Challenges Resolutions
1. Parent-Teacher -Adviser
Conference and
Feedback-giving
Some parents asked about the
performance and progress of
their children and the lacking
grades of students.
 Teachers checked the
records of the learners
who have incomplete
grades and found out
that some learners lack
outputs in certain
subjects hence the
incomplete grades.
 The parents were given
the list of the lacking
outputs/projects of
their children in
particular subjects.
2. Showcase of Learner’s
Outputs and
Performance
Some parents were unable to
find/see the outputs/projects
of their children on the display
area.
 The teacher explained
to the parents that their
children were not able
to submit the required
outputs consequently
their children have no
outputs on the display
area.
3. Distribution of Learner’s
Progress Report Cards
Other parents were hurrying to
go home to attend to some
errands and other matters.
 The teacher required the
parents to attend the
whole activity.

IV. CONCLUSION/RECOMMENDATIONS
The conduct of the Quarterly Portfolio Day is one of the most important activities of the
school. It aims to showcase the various accomplishments of the students within the quarter
through their outputs, projects and other performance tasks. It intends to show the student’s
performance and progress within the quarter. The school, the teachers, the parents and other
stakeholders must work hand in hand to ensure that the learners are able to perform at their best
and will be able to achieve the quality education they deserve.

Tagadtaran, Burauen, Leyte PORTFOLIO DAY ACCOMPLISHMENT REPORT S.Y. 2021-2022 1ST GRADING PERIOD I. INTRODUCTION The Buraburon National High School followed modalities and General Guidelines and Considerations of Distance Learning based on Memorandum DM-CI-2020-00162 and the DepEd Order No. 29, s. 2021, re: School Calendar and Activities for School Year 2021-2022. The school together with the faculty and staff organized the conduct of the said activity and followed the minimum safety and health protocols set by the local IATF. II. OBJECTIVES The following activities were suggested to be carried out: a) Parent-Teacher-Adviser Conference and Feedback Giving b) Showcase of Learners’ Outputs and Performances c) Distribution of Learner’s Progress Report Cards III. HIGHLIGHTS OF ACCOMPLISHMENTS A. Accomplishments Date of Conduct: NOVEMBER 27, 2021 Percentage of Parents who Participated in the Conduct of the Activity via: Percentage a. Parent-Teacher -Adviser Conference and Feedback-giving Some parents asked about the performance and progress of their children and the lacking grades of students.  Teachers checked the records of the learners who have incomplete grades and found out that some learners lack outputs in certain
  • 2. subjects hence the incomplete grades.  The parents were given the list of the lacking outputs/projects of their children in particular subjects. 2. Showcase of Learner’s Outputs and Performance Some parents were unable to find/see the outputs/projects of their children on the display area.  The teacher explained to the parents that their children were not able to submit the required outputs consequently their children have no outputs on the display area. 3. Distribution of Learner’s Progress Report Cards Other parents were hurrying to go home to attend to some errands and other matters.  The teacher required the parents to attend the whole activity. 4.   • 4. IV. CONCLUSION/RECOMMENDATIONS The conduct of the Quarterly Portfolio Day is one of the most important activities of the school. It aims to showcase the various accomplishments of the students within the quarter through their outputs, projects and other performance tasks. It intends to show the student’s performance and progress within the quarter. The school, the teachers, the parents and other stakeholders must work hand in hand to ensure that the learners are able to perform at their best and will be able to achieve the quality education they deserve.
