De definitie van jicht is : een inflammatoire arthritis , veroorzaakt door een ophoping van urinezuurkristallen in gewrichten en/of in andere weefsels.
Het voorkomen van jicht : in 2.80 % van de mannelijke populatie en in 0.60% van de vrouwelijke populatie. Het voorkomen stijgt met de leeftijd.
Jicht is de meest voorkomende vorm van arthritis ter wereld.
De risicofactoren voor het ontwikkelen van jicht kunnen worden onderverdeeld in beïnvloedbare en niet-beïnvloedbare.
Niet-beïnvloedbaar zijn leeftijd , geslacht , ras , genetisch en chronisch nierlijden.
De beïnvloedbare risicofactoren zijn : te hoge spiegel aan urinezuur in de bloedbaan , obesitas, alcoholgebruik, voeding met veel urinezuurrijke bestanddelen zoals schaaldieren-wild-blauwe kazen etc , en bepaalde geneesmiddelen.
We gaan het hebben over co-morbiditeit , zijnde bepaalde aandoeningen die hand in hand gaan met jicht , zoals daar zijn nierinsufficientie , coronaire hartziekten en metabool syndroom. Hieronder verstaan we obesitas , hyperlipemie , hoge bloeddruk en type 2 diabetes.
Er zijn socio-economische factoren, zoals vastgesteld bij jichtpatiënten die minder uren presteren , meer afwezig zijn t.g.v. ziekte en meer hun werkschema aanpassen t.g.v pijnopstoten.
Ook zal worden besproken hoe jicht ontstaat ; hoe urinezuurkristallen worden gevormd , hoe de nieren urinezuur ‘verwerken’en wat het verband is met de urinezuurspiegel in het bloed.
We gaan natuurlijk ook in op de behandeling van jicht.
Dit houdt in de behandeling van de acute jichtaanval en de onderhoudsbehandeling.
Deze laatste is gericht op een behandeling die de urinezuurspiegel op een constante wijze laag houdt , beneden een bepaalde grens , en die aldus het optreden van schade aan de gewrichten gaat voorkomen.
Daarnaast ook het voorkomen van schade aan andere organen , vnl. schade aan de niere

1) DXA scanning is a reliable and low-radiation method to measure bone mineral density (BMD) at the lumbar spine, hip, and wrist to diagnose osteoporosis.
2) DXA can also detect vertebral fractures (VFA) and measure whole body composition, abdominal fat, and aortic calcification.
3) Interpretation of DXA results requires attention to potential variability between devices, accurate placement of regions of interest, and use of appropriate reference data since BMD can be under or overestimated in certain patients.

This systematic review analyzed 895 cases of tumor-induced osteomalacia (TIO) from case reports. TIO is caused by tumors that produce excess fibroblast growth factor 23 (FGF23), which causes hypophosphatemia and osteomalacia. The review found that TIO mostly affects adults aged 40-60 years old, with long diagnostic delays of several years on average. The tumors were located variably but most commonly in the lower limbs or head and neck region. Higher FGF23 levels correlated with larger tumor size. • Ouderen, vooral vrouwen, en hun artsen zien osteoporose nog
steeds als een natuurlijk onderdeel van het ouder worden, in
plaats van een te voorkomen of behandelbare medische

9. Wat is kenmerkend voor osteoporose
bij ouderen?
• Postmenopauzale osteoporose wordt gekenmerkt door een verhoogde botturnover
in het metabool meest actieve bot, namelijk het trabeculaire bot (zoals de wervels).
Dit leidt tot het dunner worden en verdwijnen van trabekels
• Botontkalking bij ouderen kenmerkt zich door botopbouw die achterblijft bij de
botafbraak
• Daardoor zullen de cortex en trabekels langzaamaan dunner worden
• Corticale botafbraak door secundaire hyperparathyreoïdie (o.m. door verminderde
calciumresorptie bij vitamine D-deficiëntie) ligt in belangrijke mate aan de basis
van niet-vertebrale of perifere fracturen, zoals bekken-, schouder- of heupfracturen
• Naast andere factoren zoals het verhoogde valrisico, is dit de reden waarom
perifere fracturen zoals heupfracturen vooral op hoge leeftijd voorkomen. Niet
minder dan 60% van alle perifere fracturen komt voor bij 80-plussers
10. Waarom ouderen wel behandelen?
• Bijna de helft van de SEH-behandelingen na een
valongeval bij ouderen betreft een fractuur. Het gaat
vaak om een heup- of bekkenfractuur, een polsfractuur
of soms een wervelfractuur. Meer dan de helft van deze
patiënten blijkt bij een botdichtheidsmeting osteoporose
te hebben
• Hoe ouder de patiënt, hoe hoger het absoluut risico op
een fractuur, hoe lager de NNT zal zijn bij een relatieve
fractuurrisicoreductie van rond de 50% over de hele
leeftijdsrange (50 tot 85 jaar)

• Bisfosfonaten reduceren al na drie maanden het
botverlies en kunnen de botmassa op klinisch relevante
plaatsen verhogen, namelijk de heup en de wervels. Bij
gebruik van alendronaat en risedronaat worden zowel
wervel- als niet-wervelfracturen (o.a. heupfracturen)
gereduceerd
• In vier klinische trials bij ouderen met neurologische
ziekten, vrouwen met de ziekte van Alzheimer, vrouwen
en mannen na een CVA, en mannen met de ziekte van
Parkinson, zag men een reductie van heupfracturen na
behandeling met risedronaat

28. Atriumfibrilleren en zoledronaat
- Atriumfibrillatie was in de HORIZON-studie geassocieerd met het
toedienen van zoledronaat. De juiste betekenis van de verhoogde
incidentie van atriumfibrilleren is nog onduidelijk. Risicofactoren voor
atriumfibrillatie waren oudere leeftijd, hartfalen, vooraf bestaande
tachyaritmieeën en voorafgaande behandeling met bisfosfonaten
- In een tweede studie (HORIZON-Recurrent Fracture Trial) met
zoledronaat (jaarlijks 5 mg intraveneus toegediend gedurende twee
jaar) aan patiënten die recent een heupfractuur opliepen, vermindert de
kans op nieuwe klinische fracturen van 14 naar 9% (-35%). Dit was de
eerste studie waarin werd aangetoond dat ook de mortaliteit kan
worden verminderd met maar liefst 28 procent. In deze tweede studie
was de toediening van zoledronaat niet geassocieerd met
atriumfibrilleren. Atriumfibrillatie is in het verleden ook nooit gezien bij
het gebruik van veel hogere doses van dit geneesmiddel in de
oncologie. Algemeen wordt thans aangenomen dat het om een
toevalsbevinding ging – zelfs de FDA heeft, na inzage van de
heuptrialdata, het niet nodig geacht atriumfibrilleren in de bijsluiter op te
nemen

INTRODUCTION: Clinical fractures are associated with an increased relative risk of future fractures, but the absolute risk and timing of new clinical fractures immediately after a clinical fracture have not been reported extensively. The study objective was to determine the absolute risk of subsequent clinical fractures within 2 years after a clinical fracture. METHODS: We analyzed clinical fracture data from a university hospital recruiting all fractures in the area between January 1999 and December 2001. Subjects were 2,419 male and female patients aged 50 years and older, with a total of 2,575 fractures. There were 139 patients with more than one simultaneous fracture. Mean age was 66 years for males and 72 for females. RESULTS: The cumulative incidence of patients with new clinical fractures over 2 years was 10.8% (262/2,419). In the 262 patients with subsequent fractures, we observed a higher mean age, more females and more often multiple baseline fractures compared with the 2,157 patients without subsequent fractures. Kaplan-Meier analysis indicated that age, gender and having multiple baseline fractures contributed significantly to cumulative new fracture incidence. Cox regression showed that these variables independently contributed to a higher subsequent fracture incidence. New fracture incidence was higher with increasing age ( p <0.001; hazard ratio [HR] 1.2 per decade; confidence interval [CI] 1.1-1.3). Females had a new fracture incidence of 12.2% compared with 7.4% in males ( p =0.015; HR 1.5; CI 1.1-2.0). Patients with multiple baseline fractures had a new fracture incidence of 17.3% compared with 10.4% for subjects with one baseline fracture ( p =0.006; HR 1.8; CI 1.2-2.7). Of all clinical fractures occurring within 2 years after a clinical fracture, 60% occurred during the first year and 40% during the second year ( p =0.005). The absolute risk to develop an incident clinical fracture within 2 years after any clinical fracture was 10.8%. Increased age, female gender and the presence of multiple simultaneous fractures at baseline each independently increased the risk of incident fracture. Significantly more fractures occurred in the first year following the index fracture than in the second year. CONCLUSION: Altogether, these data support the need for early prevention of future fracture among individuals with a fracture after age 50, using interventions which have been shown to have a rapid anti-fracture benefit.
Vanaf het 50 levensjaar heeft een postmenopauzale vrouw een sterk verhoogd risico op m.n. wervelfracturen, groter dan welk ander type fracturen dan ook. Vanaf het 74e levensjaar heeft een postmenopauzale vrouw een sterk verhoogd risico op m.n. heupfracturen en andere niet wervel (perifere) fracturen. Leeftijd is daarmee naast prevalente fracturen een belangrijke (de belangrijkste) risicofactor voor OP fracturen.
Uit de literatuur blijkt een verhoogde valneiging de grootste risicofactor te zijn voor een heupfractuur, met op de tweede plaats osteoporose.
M.n. Heupfracturen zijn verantwoordelijk voor de mortaliteit van de osteoporose. De gemiddelde mortaliteit binnen 1 jaar is 20%, stijgend tot 1 op de 2 mensen in de leeftijdsgroep boven de 90 jaar.
Het kernprobleem van osteoporose wordt gekenmerkt door teveel botafbraak en te weinig botaanmaak per botombouwcyclus. Hierdoor onstaat er een negatieve balans ten gunste van botafbraak. Per botombouwcyclus treedt er netto botverlies op. Gecombineerd met een hoge botombouwsnelheid zorgt dit voor versneld botverlies wat uiteindelijk leidt tot verlies van botmassa en botstructuur en daarmee een verhoogde kans op osteoporotische fracturen.
Een vrouw met meerdere risicofactoren en een heel lage botdichtheid heeft een hoog absoluut risico op een heurpfractuur in de komende 10 jaren. Dit kan met anti-osteoporose-medicatie gerduceerd worden met 20 tot 30 procent.
Impactfactor wordt bepaald door richting, hoogte en snelheid van een val. De beschermende responsen worden gevormd door de evenwichtsreacties, uitstapreacties, opvangreacties (gestrekte versus gebogen arm) en valbreektechnieken. Anthropometrie: lage BMI.
Elderly patients have a high risk of falling and fracture, and this population is often lacking vitamin D. A 5-month, double-blind, placebo-controlled study assessed the incidence of falls among elderly patients who resided in a long-term care facility and underwent vitamin D supplementation therapy. Eligible patients were randomly assigned to placebo or 1 of 4 doses of vitamin D: 200 IU, 400 IU, 600 IU, or 800 IU. The number of falls, which was the primary outcomes measure, was documented in a computerized incident-report database. 1 Among the 124 participants in this study, 73% were women, and the mean age was 89 years (range, 68–104). The average serum 25(OH)D was 19.5 ng/mL and most (62%) of the participants had fallen within the previous year. Sixty-three percent were taking a daily multivitamin at study enrollment. 1 Among the patients who received 800 IU of vitamin D, the mean 25(OH)D concentration was 29.95 ng/mL; the mean concentration in the other groups was lower, ranging from 22 to 24 ng/mL. Falls occurred in 44% of those given placebo, 58% of those given 200 IU of vitamin D, 60% of those given 400 or 600 IU, and 20% of those given 800 IU. METHODS: In a factorial-design trial, 5292 people aged 70 years or older (4481 [85%] of whom were women) who were mobile before developing a low-trauma fracture were randomly assigned 800 IU daily oral vitamin D3, 1000 mg calcium, oral vitamin D3 (800 IU per day) combined with calcium (1000 mg per day), or placebo. Participants who were recruited in 21 UK hospitals were followed up for between 24 months and 62 months. Analysis was by intention-to-treat and the primary outcome was new low-energy fractures. FINDINGS: 698 (13%) of 5292 participants had a new low-trauma fracture, 183 (26%) of which were of the hip. The incidence of new, low-trauma fractures did not differ significantly between participants allocated calcium and those who were not (331 [12.6%] of 2617 vs 367 [13.7%] of 2675; hazard ratio (HR) 0.94 [95% CI 0.81-1.09]); between participants allocated vitamin D3 and those who were not (353 [13.3%] of 2649 vs 345 [13.1%] of 2643; 1.02 [0.88-1.19]); or between those allocated combination treatment and those assigned placebo (165 [12.6%] of 1306 vs 179 [13.4%] of 1332; HR for interaction term 1.01 [0.75-1.36]). The groups did not differ in the incidence of all-new fractures, fractures confirmed by radiography, hip fractures, death, number of falls, or quality of life. By 24 months, 2886 (54.5%) of 5292 were still taking tablets, 451 (8.5%) had died, 58 (1.1%) had withdrawn, and 1897 (35.8%) had stopped taking tablets but were still providing data for at least the main outcomes. Compliance with tablets containing calcium was significantly lower (difference: 9.4% [95% CI 6.6-12.2]), partly because of gastrointestinal symptoms. However, potentially serious adverse events were rare and did not differ between groups. INTERPRETATION: The findings do not support routine oral supplementation with calcium and vitamin D3, either alone or in combination, for the prevention of further fractures in previously mobile elderly people.
Alendronate significantly decreased both NTx (1 month) and PINP (3 months). The suppression of bone turnover with alendronate was maintained through month 12. Conversely, teriparatide significantly increased both markers of bone remodeling. The increases of the bone formation marker, PINP, were vigorous and rapid (113% above baseline at 1 month), peaking at 6 months of treatment (251% increase above baseline. The increases in the bone resorption marker, NTx, were of far smaller magnitude (58% at peak), reached significance only after 3 months of treatment, and lagged behind those of PINP. Thus, the different effects of the two drugs on bone remodeling were evident after 1 month of treatment, and there were significant differences (P<0.001) between the treatment groups in each marker at each time point (months 1,3,6,12). ___________________ McClung, et al. What is strontium ranelate? Strontium ranelate is made up of 2 atoms of stable strontium –therefore not radioactive- and 1 molecule of ranelic acid, the organic part permitting the best compromise in terms of molecular weight, pharmacokinetics and acceptability of the medicinal product. Ranelate is important to transport strontium through the blood, ranelate is not absorbed in the body. Strontium ranelate is a new and innovative drug that has been synthesized by Servier research. The strontium content in strontium ranelate is 34.1 % The absolute bio availability of strontium is about 25% (range 19-27%) after an oral dose of 2 g strontium ranelate. Maximum plasma concentrations are reached 3-5 hours after a single dose of 2 g. Steady state is reached after 2 weeks of treatment. Intake of strontium ranelate with calcium or food reduces the bio availability of strontium by approximately 60-70%, compared with administration 3 hours after a meal. Deze humane botbiopten tonen aan dat SR de botmatrix synthese stimuleert: het aantal trabekels neemt toe, de dikte neemt toe, de tussenruimte neemt af en de corticale dikte neemt toe.
Ook het minerale gedeelte van het bot neemt toe.

Het werkingsmechanisme van mn de botafbraakremmers (mn bisfosfonaten: het remmen van botafbraak en botaanmaak en de mate van botombouw waardoor er geen aanmaak van botmatrix plaatsvindt maar slecht accumulatie van mineralen) doet vermoeden dat lange termijn effectiviteit moeilijk te verwezenlijken zal zijn. Het is ook nooit aangetoond.

Strontium ranelate produces an early and sustained reduction of both vertebral and nonvertebral fractures in patients > or = 80 years of age. INTRODUCTION: About 25-30% of the population burden of all fragility fractures in the community arise from women > or = 80 years of age, because this population is at high risk for all types of fracture, particularly nonvertebral fractures. Despite this, evidence that therapies reduce the risk of both vertebral and nonvertebral fractures in this group is lacking. The aim of this study was to determine whether strontium ranelate, an agent that reduces the risk of vertebral and nonvertebral fractures in postmenopausal women >50 years of age, also reduces fractures in the elderly. MATERIALS AND METHODS: An analysis based on preplanned pooling of data from two international, phase III, randomized, placebo-controlled, double-blind studies (the Spinal Osteoporosis Therapeutic Intervention [SOTI] and TReatment Of Peripheral OSteoporosis [TROPOS]) included 1488 women between 80 and 100 years of age followed for 3 years. Yearly spinal X-rays were performed in 895 patients. Only radiographically confirmed nonvertebral fractures were included. RESULTS: Baseline characteristics did not differ in placebo and treatment arms. In the intent-to-treat analysis, the risk of vertebral, nonvertebral, and clinical (symptomatic vertebral and nonvertebral) fractures was reduced within 1 year by 59% (p = 0.002), 41% (p = 0.027), and 37% (p = 0.012), respectively. At the end of 3 years, vertebral, nonvertebral, and clinical fracture risks were reduced by 32% (p = 0.013), 31% (p = 0.011), and 22% (p = 0.040), respectively. The medication was well tolerated, and the safety profile was similar to that in younger patients. CONCLUSIONS: Treatment with strontium ranelate safely reduces the risk of vertebral and nonvertebral fractures in women with osteoporosis > or = 80 years of age. Even in the oldest old, it is not too late to reduce fracture risk.
