Discussion 4
Prepare a 350 word draft of a clinical problem that you would like to use for your Research project. There should be an introduction to the problem (as to why it is a problem) that is documented with literature, a clear problem statement (one declarative sentence that begins with "The problem is...", and a purpose to your project. Post this as your Initial response. (Essential I-IX).
The problem is falls among elderly and its relation with their health problems and surrounding environmental factors. Falls are one of the most common problems in the elderly around the world. A fall is defined as an event which results in a person coming to rest inadvertently on the ground or floor or at another lower level. Currently, there is no numerical criterion that classifies people as "elderly." However, the United Nations has determined that the age of 60+ should be used to refer to people as being "elderly. This is in spite of the fact that most developed countries take the chronological age of 65 years to define the ‘elderly. In the United States, falls are a leading cause of morbidity and mortality among mature adults. It is the second leading cause of accidental or unintentional injury/death after road traffic injuries. For example, of the 11 million adults aged 65 years and over living in the UK, around 30% of community-dwelling mature adults fall at least once in their lifetime. It was found in a study conducted in the Eastern Mediterranean Region that 30%–40% of adults older than 65 years residing in the community fall each year. The rates were higher in hospitalized patients and nursing home residents. Moreover, the incidence of falls rose steadily from middle-age onward peaking in persons older than 80 years.
Falls may be associated with various contiguous environmental hazards such as carpets and rugs. Most falls (72.8%) occur at home. Women represented 80.2% of fall injury victims. Not surprisingly, perhaps, the most common location for fall injuries in the home is the bathroom (35.7%). Other environmental hazards include poor stairway design and disrepair, inadequate lighting, clutter, slippery floors, unsecured mats, and the lack of nonskid surfaces in bathtubs. There is a paucity of literature on the important topic of falls amongst the elderly in the Kingdom of Saudi Arabia. However, our study will focus on estimating the true extent of falls among the elderly within the community alone and examine the relationship between the health status of elderly people as well as the impact of the environment on their propensity to experience falls.

Hepato-splenic abnormalities in vitiligo - do they exist? VR Foundation
Vitiligo is a skin disease of still unclear, multifactorial aetiology. Numerous autoimmunological disturbances observed in vitiligo may be suggestive of a systemic nature of the disease. Since the research into immunological and metabolic abnormalities gives evidence that they may also be present in vitiligo, it seems essential that any disturbances or abnormalities should also be searched for in the patients affected with the condition. Metabolic changes which take place in both the liver and spleen are also worth the interest.
Aim & Scope: The aim of the study was to assess the liver and spleen function parameters in the children hospitalized in Department of Dermatology Medical University of Lublin.
Material and Methods: The study included 34 vitiligo patients and 35 healthy individuals, aged 7-15 years old. We examined those children who had the active phase of vitiligo, in which exacerbation of skin lesions had a history of 6 months. Liver and spleen ultrasound examination included their size and parenchyma assesment. In order to assess the liver and spleen functioning the following accessory examinations were performed: blood test, transaminases, electrophoresis of proteins, lipid profile, autoantibodies, HCV antibodies.
Results: The values of the liver size were not significantly different between the study and control groups. The ultrasonografic pictures of the spleen did not revealed any abnormalities in the size and structure of the organ. The concentration of HDL-cholesterol was significantly lower whereas the concentration of LDL-cholesterol was significantly higher in vitiligo patient in comparison with the healthy group. The value of the LDL/HDL ratio was significantly higher in vitiligo patients.
Comments:
• Performed investigations are indicative of lipid disturbances in children affected with vitiligo.
• However, the ultrasound examination did not reveal any abnormalities with reference to the size and structure of both the liver and spleen. Neither did biochemical investigations reveal any disfunctions of the liver and spleen.
• Since there were no structural and functional abnormalities in the liver and spleen it seems plausible that lipid disturbances in vitiligo may result from genetic defect or some auto-cytotoxic events taking part in the condition pathogenesis.
• The nature of the conducted research is preliminary and it requires further long-term studies, which would include a wider variety of studied subjects as well as more numerous groups.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

25. Take home message
Osteogenesis Imperfecta patients appear to have almost three-fold higher hospitalization rate,
when compared to the general Dutch population.
Hospitalizations seem to be due to both skeletal and extraskeletal problems, which could have
been due to complications related to OI.
Due to the seemingly higher rate of hospital admissions, more awareness about the impact of OI
on patients’ life is needed to improve and implement prevention and follow-up guidelines.
