Systems Oriented Design
What is healthcare in the future? Design itself is an important power to make the healthcare system change a lot.
This project is aimed at exploring the future healthcare service in system thinking to make a holistic process and assumption of future system. Cardiovascular patients is a biggest group in the world. I hope working with this group can bring a lot of inspiration for other healthcare issue. In this project, I use strategy design, service design and interaction design method to create a hotlist service for cardiovascular patients out of the hospital. Nowadays a lot of people are suffering from longterm diseases and having high risk of cardiovascular. The food is a good way to address this problem. The meaning of the food and medicine are changing. The longterm medicine cause a mental problem at the beginning. The industry of medicine are struggling with their development, The goal of this project is to discuss the touchpoint food and medicine to address all the problems here, using system design methodology.

The Prevalence of Pressure ulcers in hospitals & Healthcare Facilities among the elderly & decreased mobility patients
an overview
Pressure ulcers are responsible for 70% of the significant issues affecting older individuals in hospitals.
The yearly healthcare cost of treating pressure ulcers in the United States is projected to be more than $20 billion( Saleh et al., 2019).
Pressure ulcers cause patients to stay in the hospital for longer periods of time, which has a significant economic impact on both the patients and the facility.
My capstone project aims to implement certain method to prevent pressure ulcers among the elderly above 60 years and decreased mobility patients in the hospital and healthcare facilities through the use of Braden scale, applying mepilex foam dressing to bony prominence areas, and repositioning (Saleh et al., 2019)
The majority of pressure ulcers or sores originate near the joints (Norman et al., 2020). They can cause infections in joints and lead to amputation or dangerous bone infections if not properly cared for or if they take a long time to close and heal. They can cause cancer and sepsis, a disease marked by internal organ inflammation caused by the body’s production of chemicals to fight infections.

Background significance
Elderly people with pressure ulcers may be unable to care for them, and as a result, they take longer to heal, exposing the wounds to infections that can be detrimental to the body and even fatal.
Pressure ulcers can lead to cancer and sepsis, an inflammatory illness defined by the production of chemicals to fight infections in the body (Papp, 2019).
People who are over 60 years are at risk of having pressure wounds due to their immobile state and they are likely to have multiple pressure wounds or recurring ones, depending on their activities and postures.
This study aims to implement certain method to prevent pressure ulcers among the elderly above 60 years and decreased mobility patients in the hospital and healthcare facilities through the use of Braden scale, applying mepilex foam dressing to bony prominence areas, and repositioning (Saleh , et al., 2019)

Background and significance cont.
Despite several methods for avoiding pressure ulcers, this problem persists in the American healthcare system, where it is both severe and embarrassing. My recommendation is that institutions institute a policy of use of Braden scale, rotating and repositioning critically sick patients every two hours, and use of mepilex border silicon foam dressing to protect the bony prominences since this is the best evidence-based method for avoiding pressure ulcers in elderly people and patient.

PICOT QUESTION: Does the required education on the Braden scale increase nursing interventions for Adult patients on an acute care floor at risk of developing pressure ulcers during hospitalization?
Currently, most hospitals are faced with a clinical problem of acquired pressure ulcers. According to Pittman et al (2015), hospital-acquired pressure ulcers remain one of the persistent and relevant issues that need to be addressed in long-term hospital stay patients. Health care is attempting to implement evidence-based protocols, though patients continue to suffer from this prevalent and preventable injury. Health care institutions are facing a big challenge for the patients with this acquired condition because hospital bills continue to balloon and at the same time insurance companies stopped paying for this condition. Research shows that pressure ulcer is preventable; though, in spite of hospitals striving to integrate evidence-based approaches to curb the issue, it continues to remain a serious issue for long-term hospital stay patients. This PICOT statement this research paper is proposing to use is a Braden Scale which can be used by nurses in their practice to reduce hospital-acquired pressure injuries which will reduce the patient stay in the hospital as well as the bills burden in the hospital.

According to Engels et al (2016), “the importance of using evidence-based practice in long-term care hospitals to reduce prevalent pressure ulcers is to promote a safe cost-effective outcome for our patients, families, and the healthcare group. Research needs to be conducted and qualitative data collected when designing an evidence-based solution to hospital-acquired injuries”. Despite a lot of research being conducted from the past years concerning acquired pressure ulcers, many patients continue to get the disease. Evidence-based practice allows the nurse to get pooled in a team of experts where interdisciplinary collaboration becomes the ultimate objective for nurses to practice autonomy that enhances change in the nursing field based on data. “The nursing research utilizes qualitative and quantitative logical methods and an EBP approach aimed at around the study and change of patient consideration, understanding consideration frameworks, and patient results” (Mervis & Phillips, 2019). This PICOT question will effectively apply the Braden Scale to see how it can positively impact long-term hospital in reducing pressure ulcers injuries.

A 54-year-old man, Mr. Stephen Smith, was brought by ambulance to the Emergency Department. He had woken up from sleep with slurring of speech and weakness of his right arm and leg. His wife was extremely distressed as Mr Smith had been perfectly well the previous night when he went to sleep. Within 20 minutes after the initial call was made Mr. Smith was admitted to the Emergency Department and was reviewed by the SpR covering the Regional Specialist Stroke Unit. Mr. Smith had been on regular antihypertensive medication (lisinopril) for 8 years. He smoked 5-8 cigarettes a day and was a social drinker consuming about 6 units of alcohol a week. He was not diabetic.
His Serum lipids were checked and was advised to reduce weight and started on a Statin (Simvastatin). There was no family history of hyperlipidaemia but his grandfather died after a Stroke. Mr. Smith had an urgent appendectomy 1 week and made an uneventful recovery. He lives with his wife in a 4-bedroom detached house.
Neurological examination showed that Mr. Smith was fully conscious and alert. He had an upper motor neuron facial palsy on the right side. He had expressive dysphasia but appeared to comprehend speech. He was just able to lift his right arm off the bed for a short period but had no grip. His right leg was weak.
Reflexes on the right side were exaggerated and his right plantar was extensor. He responded to touch and pin prick equally on both sides. He either had visual inattention or a visual field defect on the right side. He had no papilloedema His blood pressure was 164/96, pulse 84 per min, regular. Other systems were entirely normal.
ECG and all routine blood tests were performed. An emergency CT scan was requested and even though the scan was normal the SpR ruled out emergency thrombolysis in this instance. Mr. Smith was admitted to the Acute Stroke Unit and was seen by the Stroke Consultant. Mr. Smith remained fully conscious and alert but had some difficulty in swallowing. Hence an intravenous infusion was commenced and an alternate strategy was adopted for providing his nutritional requirements. A carotid Doppler scan was requested.
After 3 weeks, Mr. Smith seemed to make good progress and the MDT meeting recorded a consistent improvement in his Barthel Index. The Stroke Team met him with his wife and discussed arrangements for discharge home. His wife was keen to know what support measures were available to them when Mr. Smith returned home. She also wanted to know about the risk of a future stroke and how this could be cut down.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo.
