A 73-year-old man presented with a 3-year history of progressive memory loss, poor balance, and recent urinary incontinence. Examination showed impaired memory and difficulties with calculations and gait. Brain imaging showed enlarged ventricles and white matter changes. Laboratory tests did not reveal a treatable cause of dementia. The patient's gait improved transiently after lumbar puncture.
