BACKGROUND: Sequential Epstein-Barr virus (EBV)–positive B cell lymphoma to the initial diagnosis of angioimmunoblastic T cell lymphoma (AITL) is very rare, the exact mechanism and standard therapy of which is still being explored. CASE: A 50-year-old man was admitted to our hospital in January 2014 with a three-week history of enlargement of multiple lymph nodes. His initial pathological evaluation indicated AILT. The reactivation of EBV was observed during the immunosuppression therapy for AITL, accompanied by onset of subcutaneous nodules proven to be EBV-positive diffuse large B cell lymphoma (DLBCL) based on the pathological findings of rebiopsy. The patient was successfully treated with chidamide, a histone deacetylase (HDAC) inhibitor, and rituximab.
Conclusion: The sufficient surveillance for serum EBV and repeat biopsy is necessary for patients with AITL, and this treatment modality may become an active option.

The course of hepatitis B virus (HBV) infection is variable depending on many factors. In this study,
we investigated single nucleotide polymorphisms of interleukin-28B and interleukin-16 as possible host factors
which may determine the occurrence of hepatocellular carcinoma (HCC) in Saudi patients. Chronic hepatitis B
(CHB) patients (75), HCC patients (42), and healthy controls (70) were analyzed for polymorphisms of the IL16
and IL28B genes using PCR and restriction fragment length polymorphism (RFLP). Results showed that HCC
and chronic HBV patients had higher prevalence of rs11556218TG genotype than controls. The rs11556218GG
genotype was higher among HCC (14.4%) compared to chronic HBV (2.7%) patients. The IL-16 genotype
rs4072111CT was higher among HCC (47.6%) and chronic HBV (46.7%) patients than controls (28.6%). The
rs4072111TT genotype was higher among HCC patients compared to the other two groups. The T allele
frequency was higher among HCC patients than controls. The CT and TT of the IL-28B rs12979860 genotype
were significantly less frequent in chronic HBV and HCC patients. The IL-28B rs8099917 TG genotype was
more frequent among HCC (19%) compared to chronic HBV (8%) patients. However, no significant difference
was detected in the allele distribution.

5. Introduction
HIV is transmitted most efficiently from cell to cell and productive infection occurs mainly in
activated CD4 T cells.
It is postulated that HIV exploits immunological synapses formed between CD4 T cells and
antigen-presenting cells to facilitate the targeting and infection of activated CD4 T cells.
The results showed that the HIV Env did not disrupt immunological synapse formation.
Instead, the HIV Env accumulated with TCR at the center of the synapse, altered the kinetics
of TCR recruitment to the synapse, and affected the synapse morphology over time

Virus replication after stimulation with OKT3 + HIV gp120 was comparable to or even
higher than that seen in CD4 T-cells stimulated with OKT3 and anti-CD28 mAb, indicating
the HIV gp120’s potent co-stimulatory activity.
All together these results show that in the CD4 T cell immunological synapse HIV Env may
act as a co-stimulator that heightens TCR activation of the CD4 T cells and imparts the cells
with the capacity to support higher levels of virus replication.
17. Conclusion
•This study demonstrated the ability of HIV
Env gp120 to alter the kinetics of TCR
recruitment and the activation of TCR-
proximal signaling at the CD4 T cell
immunological synapse.
•HIV, via its Env glycoproteins, alters the
immunologic synapse and the activation state
of CD4 T cells undergoing TCR activation in
a way that is likely to facilitate viral
replication and spread.
•Without TCR activation HIV cannot attach
on the Cell and Complete viral replication.
