Background:
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) emerged in China in the city of
Wuhan in December of 2019.

Transmission of the disease occurs through direct contact or droplets through coughing or sneezing.2
COVID-19 has been declared by WHO as a global public health emergency on March 11, 2020. 1
Infected patients had presented with dry cough, dyspnea, fever and bilateral
pulmonary infiltrates in imaging examinations.3

Pathogenesis:
A- Immune mediated tissue damage
Virus spike proteins interact heavily with the receptors of
angiotensin-converting enzyme 2 (ACE2). ACE2 is highly
concentrated in the lung, heart, renal, ocular tissue and
intestinal tissue.5
ACE2 expression occurs in the cornea and in the conjunctiva
epithelial cells.6
The characteristic tissue damage in COVID-19 patients is
mediated by T-lymphocytes, antigen-presenting cells and
macrophages, which have ACE2 receptors.7

B- COVID-19 associated coagulopathy
The COVID 19 infection is characterized by
more thrombotic than hemorrhagic events
that are associated with coagulopathy. 8
COVID-19 associated coagulopathy caused by
endothelial dysfunction of sepsis induced
coagulopathy with elevated levels of
fibrinogen. 9, 10

Eye as a gateway to COVID-19
infection
Respiratory infection transmission through the eye and lacrimal-
nasal pathway was evident during the 1918 influenza pandemic.11
Ocular surface droplet deposition is greatly underestimated as a
probable, common gateway to SARS-CoV-2 transmission. 11
The virus may bind to corneal and conjunctival (ACE2) receptors and
superficial tear film.12

Eye as a route of COVID-19 infection
transmission
There are various studies that proves that viral shedding
in tears and conjunctival secretions can occur in patients
of SARS-CoV-2.13, 14

Prevalence of Ophthalmic symptoms in Covid 19
A large meta- analysis evaluating the prevalence of
ophthalmic manifestations found that 4.3% reported
ocular symptoms and 0.9% presented with ophthalmic
symptoms as their initial complaint. 15
Another recent meta-analysis of 12 studies in 1,930
patients reported that conjunctivitis or conjunctival
congestion was found in up to 8% of COVID-19
patients.16
The prevalence of ocular manifestations was found to
vary from 2% to 32% in another study.17

Follicular conjunctivitis:
Conjunctivitis is the most common ophthalmic
manifestation reported in COVID-19 patients. 18
It was suggested that ocular manifestations are
more common in the middle phase of the disease.

However; delayed onset of follicular conjunctivitis
was reported four weeks after severe COVID-19
infection. 20

Viral keratoconjunctivitis
Keratoconjunctivitis is the initial presentation
in a positive swab patient with mild
respiratory symptoms with redness,
discharge, and photophobia has been
reported. 21
Relapsing viral keratoconjunctivitis in COVID-
19 was also reported, Although SARS-CoV-2
could not be detected in conjunctival
secretions. 22

Hemorrhagic and pseudomembranous conjunctivitis
It was reported in a case of a 63-year-old male patient with severe COVID-19 infection.23
Conjunctival congestion was found to be an early sign of COVID-19 infection even before the
development of systemic symptoms in 2.26% of the patients. 24

Episcleritis
Most of cases are idiopathic and self-
limiting.
It was presented with nasal
conjunctival and episcleral congestion
which was followed by the systemic

Blepharitis and dry eye
Blepharitis was positively correlated with the COVID-19
disease duration.
It is associated with meibomian orifice abnormalities and
lid margin hyperemia or telangiectasia. 27

Posterior Segment Manifestations:
It includes retinal vascular occlusions, retinal abnormalities and uveal
abnormalities.
Retinal vascular occlusion
Patients of COVID-19 are in a procoagulant state.
Central retinal vein occlusion was reported in many
reports 28, 29 while other reports 30,31 documenting
central retinal artery occlusion.
Bilateral central retinal vein occlusion in a 40-Year-old
man with severe coronavirus pneumonia. 32

Mucormycosis
Mucormycosis is a life-threatening, opportunistic infection.
it’s caused by environmental molds that we’re breathing all the time.
Patients with moderate to severe COVID-19 are more susceptible to it because of the compromised
immune system by the virus and steroids therapy. 47, 48

Mucormycosis
Mortality rate is as high as 50% even with treatment:
developed as late as 30-42 days after the diagnosis of
COVID-19.18
High index of suspicion, early diagnosis and treatment
are mandatory to improve survival.49

Treatment ( according to the presentation)
Conjunctivitis resolved with lubricants and preservative-free moxifloxacin eye drops.20
Viral keratoconjunctivitis treated as herpetic keratoconjunctivitis. 21
Hemorrhagic and pseudomembranous conjunctivitis treated with azithromycin and
dexamethasone drops and daily debridement of pseudomembrane. 23
Central retinal vein occlusion is managed by using anti-vascular endothelial growth
factor (anti-VEGF) in the established phase. Early anticoagulant prophylaxis should be
considered.28
Mucormycosis is managed with antifungals and aggressive surgical debridement
(functional endoscopic sinus surgery and orbital exenteration) can improve survival.49
Guidelines on providing ophthalmic care50
Use available slit-lamp barriers or breath shields.
Inform the patients to wear masks and to speak as
little as possible during the slit-lamp examination.
The ophthalmologist is better protected by
wearing an N95 mask instead of a surgical mask
and to wear eye protection.
Gowns and gloves should be worn.
N95 masks and eye protection are mandatory in
Ophthalmology operating room.

Summary
As the pandemic continues to affect the world, it is crucial for physicians to
understand the manifestations of COVID-19.
The main pathogenesis of Covid eye manifestations are due to Immune mediated
tissue damage and COVID-19 associated coagulopathy.
Conjunctivitis is the most common ophthalmic manifestation reported in COVID-19
patients.
Other manifestations include posterior segment manifestations, neuro-ophthalmic
manifestations, and Orbital manifestations.
Mucormycosis is a life-threatening orbital opportunistic infection which necessitates
early diagnosis and treatment.

Treatment ( according to the presentation) Conjunctivitis resolved with lubricants and preservative-free moxifloxacin eye drops.20 Viral keratoconjunctivitis treated as herpetic keratoconjunctivitis. 21 Hemorrhagic and pseudomembranous conjunctivitis treated with azithromycin and dexamethasone drops and daily debridement of pseudomembrane. 23 Central retinal vein occlusion is managed by using anti-vascular endothelial growth factor (anti-VEGF) in the established phase. Early anticoagulant prophylaxis should be considered.28 Mucormycosis is managed with antifungals and aggressive surgical debridement (functional endoscopic sinus surgery and orbital exenteration) can improve survival.49
  • 29. Guidelines on providing ophthalmic care50 Use available slit-lamp barriers or breath shields. Inform the patients to wear masks and to speak as little as possible during the slit-lamp examination. The ophthalmologist is better protected by wearing an N95 mask instead of a surgical mask and to wear eye protection. Gowns and gloves should be worn.   • 30. Summary As the pandemic continues to affect the world, it is crucial for physicians to understand the manifestations of COVID-19. The main pathogenesis of Covid eye manifestations are due to Immune mediated tissue damage and COVID-19 associated coagulopathy. Conjunctivitis is the most common ophthalmic manifestation reported in COVID-19 patients. Other manifestations include posterior segment manifestations, neuro-ophthalmic manifestations, and Orbital manifestations. Mucormycosis is a life-threatening orbital opportunistic infection which necessitates early diagnosis and treatment.
