ventilation strategies for Acute Respiratory Distress Syndrome (ARDS) within the Intensive Care Unit
(ICU).
My role as an Advanced Critical Care Practitioner (ACCP) involves me working with patients with
multi organ failure and more often respiratory failure following prolonged periods of ventilation and
lung insult. ARDS has been studied in a variety of settings since it was first introduced by Dr
Ashbaugh (Ashbaugh et al, 1967), at which time mortality was between 60% and 70%, however, the
most effective way to ventilate patients with ARDS is still being researched. Despite this ongoing
research into the management of ARDS, mortality remains at 35 40%. It is thought this is linked to
lack of knowledge and evidence regarding the pathological process of ARDS.
ARDS can be defined as an acute decrease in the ratio of arterial oxygen partial pressure to fractional
inspired oxygen (P/F) 30% surface area burns or any other condition which had a 50% 6 month
mortality. Although there are significant differences in each of these papers exclusion criteria they are
clearly identified, which is good practice and not always done (Van Spall et al, 2007). However, these
extensive exclusion criteria can impair generalisability of the trial results in the ICU.

no significant increase in plasma leptin levels in HFD fed mice after two hours, whereas with the CD
fed mice there was a significant increase in plasma leptin levels. This implies that leptin production
and/or secretion after refeeding is impaired in HFD fed mice. To verify that there is a decrease in
systematic leptin level, the researchers isolated primary adipocytes from HFD and CD fed mice and
then determined ISLS in ex vivo. The result was that ISLS was significantly lower in primary
adipocytes from HFD fed mice. This supports the hypothesis that impaired ISLS in HFD fed mice in
vivo and ex vivo was dependent on PI3K/Akt activation. Furthermore, there is an essential role of
PI3K/Akt in regulating ISLS in primary adipocytes as seen when blocking PI3K activity with
wortmannin. This significantly inhibits ISLS without affecting basal leptin secretion. Also, AKT 1/2

useless as well. Ohio s law requires schools to write their own bullying policies, but that s just about
