The Supported Care Service works with over 500 voluntary and community organizations each year to provide support to patients after discharge from the hospital or those living independently. The VCS Coordinators within the Supported Care Service help connect patients to local services and organizations that can address their specific needs, such as shopping assistance, home modifications, counseling, and more. They work to develop relationships between health/social services and voluntary groups to better support patients in the community.

 Permanently or temporarily registered with
a North Norfolk/South Norfolk GP
 18 years or over
 Living independently in their own home
 Not receiving end-of-life care
 Not presenting with an unmanaged mental
health need
 Not requiring social care only whilst
awaiting a social care assessment or care
package.

4. Brundall patient admitted to hospital with a
fractured hip following a fall.
Referred to the Supported Care Service on
discharge for personal care, medication
prompting, and meal preparation.
When SCS service is completed he needs
short-term general support at home,
including shopping.
