Precautions
Mohsin Raza
Fire And Evacuation
Activate the building fire alarm system.
If the fire is small enough, use fire extinguisher
If the first attempts to put out the fire do not succeed
Doors, and if possible, windows, should be closed as the
last person leaves a room or area.
Do not use elevators, use building stairwells.
all should proceed to a designated meeting area where
head can counts
Mohsin Raza
Do not fight a fire when
• You don't know what's burning.
• The fire is spreading rapidly.
• You don't have the proper equipment.
• You can't do so with your back to an exit.
• The fire might block your means of escape.
• You might inhale toxic smoke.

Sub Topics
Inspecting Condition
What is an safety inspections?
Types of safety Inspections?
Planning an safety inspections.
How to conduct safety Inspections?
How to write the safety inspection report and time-lines?
Effective Implementation of Inspections
Mohsin Raza
Inspecting Condition
Safety and Health qualified personnel
Inspections shall not disrupt the operations
Company assign someone to walk with inspector, for
taking notes
A separate deficiency report for each violation should be
written and RAC’S
Respond within 30 days
Mohsin Raza
What Is Safety Inspections?
An inspection is part of workplace monitoring
Checking of substandard acts and conditions
Checking the PEPME: people, environment, physical
conditions, machinery/materials and equipment.
An audit of the whole health and safety system
Establish a good abatement plan
Mohsin Raza
Types Of Inspections
Informal
Formal
Mohsin Raza
Informal Inspections
Performed everyday: checking the worksites for hazards prior
to beginning work
Walking around by management, supervisors, workers to
check/monitor conditions informally (not written down, but
noted).
Mohsin Raza
Formal Inspections
 This is an example of regular inspections.
 Inspections can be conducted periodically with no defined timelines so
people cannot prepare also known as surprise inspections
Always inform someone higher up that you will be conducting the inspections so
you do not create a hazard for yourself or the inspection team
Mohsin Raza
Planning an Inspection
 Determine what you wish to inspect with your team
 Who will be on the team
 Why you wish to inspect
 When you wish to inspect
 How you will go about it i.e. How to rate/rank hazards, what checklist to use
 Allocate enough time so you can be effective
 Ensure enough resources are provided so the corrective actions can occur in a
timely manner
 Assemble the checklist, pen and paper, materials, and the team and begin
Mohsin Raza
Conducting The Inspection
 Wear the necessary PPE: set a good example
 Do not operate equipment you are not competent to do so
 Inform the supervisor of the area if this is a regular inspection, and
be sure you are aware of the health and safety hazards in the area
 Get off the beaten path, look on, under, up and around, check for
hazards and prioritize them either high, medium, low…”A, B, C or
1-3
 High hazards (A, 3) dealt with immediately, medium (B, 2) hazards
24 hours, (C, 1) low hazards 48 to 72 hours
Mohsin Raza
Writing The Inspection Report
 Use KISS principle
 Rank the worst hazards first
 Create an action plan
 Assign people responsible for corrective actions
 Record the implementation date of the corrective actions
 Assign a follow up person to check that these have been done
 Do not refer to people by name if they had an unsafe act
 Preserve the privacy of individuals this is the law
 Complete within 72 hours after the inspection (s)
 Explain why some corrective actions have not been done
 Post the inspection reportMohsin Raza

Issues May Contribute Towards A Major
Accident Or Hazard:
Failing of the site safety management system
Failure to recognize a hazard before and during
maintenance
Failure to fulfill with the work permit system in hazardous
environments
Communication failure during the use of a work permit
system.
Mohsin Raza
Contributory Factors For An Assessor
 Whether staff have been sufficiently informed, instructed, trained and
supervised to minimize a potential human failing during operation of the
work permit system

maintenance instructions, correct PPE and equipment for use
 Whether the work permit contains sufficient information about the type of
work required
 Whether the employees responsible for control of the maintenance work
are identified within the work permit system and that the work is properly
authorized by a responsible person
Mohsin Raza
 Whether the work permit system is managed, regularly inspected
 Whether all work permits are kept on file
 Whether sufficient precautions are taken prior to initiating a work
permit
 Whether staff are aware of the type of environment they are working
in during the operation
 Whether the work permit system involves a formal procedure
whereby the maintained plant or equipment is handed back to
operation.
Mohsin Raza
Major Hazards Could Arise
 Wrong type of work permit used
 Wrong information about work on the work permit;
 Failure to recognize the hazards where work is carried out
 Terms of work permit not observed to
 Failure to hand-over plant in safe condition on completion of
