S3 and S4 heart sounds are abnormal mid-diastolic and late diastolic/presystolic sounds respectively. An S3 is heard during rapid ventricular filling and can be normal in children/young adults, though it becomes less common with age. It is pathological in cases of ventricular dysfunction, excessive rapid early diastolic filling, or restrictive diseases. An S4 coincides with atrial systole and is sometimes normal in healthy old adults, but is pathological in cases of severe left ventricular hypertrophy, ischemic heart disease, aneurysms, or valvular issues. A pericardial knock occurs very early in diastole in constrictive pericarditis.
