Pathological evaluation of melanocytic lesionsHisashi Uhara
In this lecture, the following basic steps by which I routinely scan specimens in our hospital will be presented with examples.
1. Evaluate the specimen preparation.
1) Is the incision for the specimen made perpendicular to the skin surface?
2) Is the slice of tissue from volar skin made perpendicular to the furrows of skin?
2. Estimate the specimen size and location.
1) Estimate the size of the lesion from the magnification of the objective lens.
2) Estimate the specimen location.
3. Precaution before evaluation
1) Observe the specimens without clinical information as much as possible.
2) Obtain as much information as possible at low magnification.
4. The steps for observation
1) At low magnification: Check the symmetric properties and circumscription of the lesion based on the following points.
a. Distance from the densest area of the lesion to both ends.
b. Variation of the thickness of epidermis from the center to both ends.
c. Distribution of melanin in the coronoid layer, epidermis, and dermis.
d. Distribution of nests and distance between each nest.
e. Density of solitary distributed melanocytes.
f. Existence of inflammatory infiltration in the dermis and its distribution.
g. Continuity of the spread of nests and tumor cells in both ends.
h. Is the bottom of the lesion smooth or not?
2) At high magnification: Check the details of tumor cells.
a. Tumor cells in the epidermis: Existence of necrosis, atypia (large nucleolus), or mitosis.
b. Other findings in the epidermis: Distribution of melanin in the cornified layer, the existence of tumor cells in the upper epidermis, the polymorphism of tumor cells, the relationship between tumor cells and keratinocytes.
c. In the dermis: An overlapping, crowded, or sheet-like gathering of tumor cells, maturation of tumor cells, mitotic figures, or melanin of tumor cells at the bottom of the lesion.
d. In the adnexal area: The existence of tumor cells in adnexal walls.
5. After provisionally giving a pathological diagnosis, check discrepancies between the pathological diagnosis and clinical findings. Return to the pathological evaluation if necessary.

Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. Published classroom materials form the basis of syllabuses, drive teacher professional development, and have a potentially huge influence on learners, teachers and education systems. All teachers also create their own materials, whether a few sentences on a blackboard, a highly-structured fully-realised online course, or anything in between. Despite this, the knowledge and skills needed to create effective language learning materials are rarely part of teacher training, and are mostly learnt by trial and error.
Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research. It will also show how anybody involved in language teaching (any language, not just English!), teacher training, managing schools or developing language learning materials can benefit from using the framework.

2. Oral and perioral pigmentation may be physiologic
or pathologic in origin. Physiologic pigmentation is
typically brown in appearance. However, in the
course of disease, the oral mucosa and perioral
tissues can assume a variety of discolorations,
including brown, blue, gray, and black. Such color
changes are often attributed to the deposition,
production, or increased accumulation of various
endogenous and exogenous pigmented
substances.

3. Hemoglobin, hemosiderin, and melanin represent
the most common endogenous sources of
mucosal color change. A sub mucosal collection
of hemoglobin or hemosiderin, produced by lysis
of red blood cells, may impart a red, blue, or
brown appearance. In contrast, melanin, which is
synthesized by melanocytes, may appear brown,
blue, or black, and is often dependant on the
amount of melanin and it’s location within the
tissue.

4. Pigmented lesions that are of exogenous origin
may usually traumatically deposited directly into
the sub mucosal tissues. In some cases, the
substances may be ingested, absorbed, and
distributed hematogenously, to be precipitated
in the connective tissues, particularly in areas
subject to chronic inflammation, such as the
gingiva. In other instances, these ingested
substances can actually stimulate melanin
production, thus precipitating the color change.

7. ENDOGENOUS PIGMENTATION
Melanin is found universally in nature. Melanin is
the pigment derative of tyrosine and is
synthesized by melanocytes, which typically
reside in basal cell layer of the epithelium. Their
presence in the skin is thought to protect
against the damaging effects of actinic ir-
radiations as well as to act as scavengers in
protecting against various cytotoxic
intermediates.

8. Melanin is synthesized within specialized
structures known as melanosomes. Melanin is
actually composed of Eumelanin, which is
brown-black pigment, and pheomelanin, which
has red-yellow color. Melanin pigmentation
maybe physiologic or pathologic and focal
multifocal of diffuse in its presentation. The term
melanosis is frequently used to describe diffuse
hyperpigmentation.

10. Over production of melanin maybe caused by a
variety of mechanisms, the most common of
which is related to increased sun exposure.
However, intraorally, hyper pigmentation is
more commonly a consequence of physiologic
or idiopathic sources, neoplasia, medication or
oral contraceptive use, high concentrations of
pituitary adrinocorticotropic hormone ( ACTH ),
post inflammatory changes, and genetic or auto
immune diseases.

11. In addition to biopsy and histologic study, various
laboratory and clinical tests, including dioscopy,
radiography and blood tests, maybe necessary for
definitive diagnosis of oral pigmentation. Dermascopy,
also known as epiluminescence microscopy is another
increasing employed clinical test. A more advanced
method makes use of binocular stereo microscopes with
or without the assistance of digital technology and
imaging software. This diagnostic test has been shown
to be effective in discriminating melanocytic from non
melanocytic lesions and benign versus malignent
melanocytic lesions.

13. Freckle/ Ephelis
The cutaneous freckle, or ephelis, is a commonly
occuring, asymptomatic, small (1-3 mm), well
circumscribed, tan- or brown- colored macule that
is often seen on the sun exposed regions of the
facial and perioral skin. Most commonly observed
in light skinned individuals and are quite prevalent
in red- or light blond- haired individuals. Although
the pigmentation itself is focal in nature, most
patients have multiple freckles. These are thought
to developmental in origin.

15. Ephelides are usually more abundant in
number and darker in intensity in childhood
and adolesence. Freckles tend to become
darker periods of prolonged sun exposure
(spring, summer) and less intense during
the autumn and winters. With increase in
age the number and color intensity tends to
diminish.

16. ORAL/LABIAL MELANOTIC
MACULE
The melanotic macule is a unique, benign,
pigmented lesion that has no known dermal
counterpart. Melanotic macules are the most
common oral lesions of melanocytic origin.
C/F:- females, lower lip and gingiva,
adulthood.Congenital melanotic macules
have also been described occuring in
tongue.Small, circumscribed, oval or
irregular in outline and uniformly pigmented.

17. Once the lesion reaches a certain size, it
does not tend to enlarge further. Does not
become dark with sun exposure. Usually
