Rentgenogrāfija
Rentgenogrāfija ir lēta, ātri veicama, plaši pieejama
radioloģiskā izmeklēšanas metode
Izmeklējums ātri veicams
Iespējams izvērtēt augšējo elpošanas ceļu stāvokli,
retrofaringeālu/parafaringeālu abscesu, epiglotītu
5. 5
Datortomogrāfija
Standarta metode kakla dziļo infekciju izvērtēšanai
Var izvērtēt
»limfmezglu iesaisti
»fasciju telpu iesaisti kaklā un videnē
»abscesa diferenciāciju
DT ar i/v kontrastvielu nodrošina lielo asinsvadu
vizualizāciju un abscesa diferenciāciju no citiem
audiem

7. 7
Magnētiskā rezonanse
MR ir izvēles metode
MR ir labāk atdiferencējamās normālas struktūras un
labāk raksturojamas patoloģiskās struktūras
8. 8
Ultrasonogrāfija
US plaši izmanto, lai diagnosticētu labdabīgus un arī
ļaundabīgus veidojumus kaklā
Tiek izmantots Doppler signāls asinsvadu plūsmas
izvērtēšanai
US kontrolē iespējams veikt abscesa punkciju
Ierobežots redzes lauks

Kopsavilkums
Visbiežākās dziļās kakla infekcijas ir peritonsilāra infekcija
(49%), retrofaringeāla infekcija (22%), submandibulāra infekcija
(14%)
Parasti dziļās kakla infekcijas ir sekundāras infekcijas, kur
primārs infekcijas avots ir cits
Ierosinātāji visbiežāk ir no mutes mikrofloras - gan Gram-pozitīvi
gan Gram-negatīvi mikroorganismi
Pacientiem ir drudzis un vājums. Lokāli simptomi tādi kā
odinofāgija, disfāgija, sāpošs kakls, kakla stīvums, sāpes, balss
izmaiņas
Radioloģiskā diagnostika
1. Datortomogrāfija ar kontrastvielas ievadi kakla daļai, kā arī
jāveic krūšukurvja DT, ja ir aizdomas par mediastinītu
2. Magnētiskā rezonanse ir izvēles metode

Low density intratonisllar abscess (white arrows). No extension trough the capsule into PPS (white open) is present. Left tonsil is prominent and enhancing but no abscess is seen. Note sympathetic effusion in retropharyngeal space (black curved).

Fig. 1 A 41-year-old man presents with a palpable left painful neck mass, neck swelling, and fever. (A) Contrast-enhanced 16-section MDCT scan shows a large abscess in the left submandibular space, with considerable mass effect (white asterisk) and extensive lymphadenopathy (black asterisks). (B) Coronal reformatted image displays the craniocaudal extent of the fluid collection (white asterisk), in addition to soft tissue swelling, increased density and of the left suprahyoid and infrahyoid soft tissue neck spaces, and cervical lymphadenopathy (black asterisks). (C) Parasagittal reformatted image defines the relationship o f the lymphadenopathy (white asterisks) to the left jugular vein (black asterisk). (D) Parasagittal reformatted image defines the extent and location of the fluid collection (arrow) and lymphadenopathy (asterisks).
