1. The patient presents with chronic headache for 9 months associated with blurring vision and bilateral papilledema. Examination is normal except for mild obesity and papilledema. 2. The differential diagnosis includes benign intracranial hypertension (pseudotumor cerebri) given the chronic headache, papilledema and normal imaging and investigations. 3. A lumbar puncture is needed to check cerebrospinal fluid pressure to confirm the diagnosis. Treatment involves weight loss advice, stopping triggering medications, repeated lumbar punctures and/or diuretics or shunt surgery if needed to protect vision.
