Community diagnosis generally refers to the identification and
quantification of health problems in a community as a whole in terms of
mortality and morbidity rates and ratios, and identification of their correlates
for the purpose of defining those at risk or those in need of health care

īĩ It is the foundation for improving and promoting the health
of community members.
īĩ its role is to identify factors that affect the health of a population and
determine the availability of resources within the community to
adequately address these factors.

Process of community diagnosis
1) Exploration- get permission to collect data
2) Planning of the survey-why, where, who, when, what will be
covered
3) Developing and pretesting survey tools –
questionnaire/interview schedule, focus group discussions
4) Execution of the survey
involves data cleaning, sorting/tallying, coding , data entry and
analysis. Present in the form of tables, graphs, histograms

Health status
īĩ Community diagnosis is used to determine and describe the health status of the population reflected in health
indicators in a community over a specific time period.
īĩ Health indicators are used by government to guide health care policy.
Incidence rates of any of the following in a population may be health indicators:
īĩ Low birth weight.
īĩ Diabetes.
īĩ High blood pressure.
īĩ Cancer incidence.

Interventions include:
i. Creating awareness through health education on issues such as
prevention of Communicable diseases and child spacing
ii. Action such as : Environmental clean up exercises- clearing bushes to
remove mosquito breeding places
