ADEM is an uncommon inflammatory demyelinating disease of the central nervous system that usually presents with encephalopathy. It is considered a monophasic illness, though recurrence may occur in 10-18% of cases. Diagnosis is based on clinical presentation including encephalopathy, seizures, and multifocal neurological deficits along with MRI findings of widespread lesions. Treatment involves high-dose corticosteroids as first-line, with options for plasma exchange or IVIG if steroids fail. Prognosis is generally favorable, with 50-75% of patients achieving full recovery within 1-6 months.
