A 70-year-old lady presented to the emergency room with complaints of chest pain and difficulty breathing for 10 days. She reported generalized swelling for 1 month with fever but no decrease in urine output. On examination, she was found to have wheezing and crackles on lung auscultation. Her blood sugar was high. The impression was pulmonary edema and chronic obstructive pulmonary disease (COPD), with a history of chronic kidney disease and diabetes.
