2. Depressie en zwangerschap
• Depressie komt vaak voor
• Lifetime prevalentie varieert van 5-17%
• Komt vaker voor in vrouwen (2x zo vaak)
• Onset op 20-30 jaar met een piek tussen 30-40 jaar
• Ook bij zwangere vrouwen komt depressie regelmatig voor
• Prevalentie ± 10%
3. Depressie en zwangerschap
• Belangrijk om te voorkomen of te behandelen:
• Inadequate prenatale zorg
• Slechte voeding
• Obstetrische complicaties
• Postpartum depressie
• Mogelijke behandelingen:
• Psychoeducatie
• Cognitieve gedragstherapie/ interpersoonlijke psychotherapie
• Gebruik van medicatie

4. SSRI’s en zwangerschap
• Selectieve serotonine
heropnameremmers
• 1.5% in 1996
• 6.2 % in 2005
• In Nederland: 1-2%
• Zorgen over mogelijke gevolgen op het ongeboren kind
• Abstinentiesyndroom bij pasgeborene
• Verstoorde slaappatronen bij pasgeborene
• Vertraagde motorische ontwikkeling (subtiele effecten)
Huidige studies zijn niet eenduidig, kleine groepen, verschillende uitkomstmaten,
bias/vertekening, retrospectief, etc.
5. • Afweging voordelen van behandeling t.o.v. nadelen mogelijke gevolgen
• Richtlijnen: landelijk kenniscentrum psychiatrie en zwangerschap
• Meer informatie nodig om ‘evidence-based’ beslissingen te nemen

• Een experimenteel gerandomiseerd onderzoek is niet ethisch

• Prospectieve dataverzameling

• Extra controle groep: depressieve symptomen zonder SSRI’s

• In utero SSRI blootstelling is
• Gerelateerd aan een verhoogde mate van autistische kenmerken
• Niet gerelateerd aan IQ op 6 jaar [-0.48 (-4.13 to 3.17), p=.79]
10. Samenvatting
• Er zijn aanwijzingen dat SSRI blootstelling mogelijk van invloed is op de
hersenontwikkeling
• Er is meer onderzoek nodig naar SSRI blootstelling en de ontwikkeling van
het kind
• Uit meerdere systematische reviews blijkt dat de literatuur nog steeds
inconsistent is
• Belangrijk om depressie bij jonge vrouwen te voorkomen

Meta-analyses of case-control studies # showed significant associations between SSRI use
during preconception, pregnancy, first and second trimester, while the qualitative review of

15. MRI
Children aged between 6-8 years were more likely to drop out:
• Of the 60 SSRIs exposed children contacted, 14 refused to participate, and of the
44 children participating only 21 children were scanned successfully.
• Also later (second MRI wave) at 9-10 years, only 34 children exposed to SSRIs
were scanned.
