Κα Σπανάκη,πρόεδρε της Παγκρήτιας Παιδιατρικής Εταιρείας, Κα Ραϊσάκη, Κα Μανιαδάκη σας ευχαριστώ για την ευγενική σας πρόσκληση.Βασίζομαι σε δύοπαραδοχές: Το ακροατήριο αποτελείται κυρίως από παιδιάτρους. Η νοσολογία που μας ενδιαφέρει αφορά τη γυναικεία εφηβική πύελο.
Δημογραφικά στοιχεία.7 δισ. είναι αυτή τη στιγμή οι κάτοικοι του πλανήτη, και 600 περίπου εκ. από αυτούς είναι κορίτσια και έφηβοι ηλικίας 10-19 ετών.Σε αυτό το 8% του ανθρώπινου πληθυσμού εστιάζω σήμερα την πυελική απεικόνιση μας.
Οι κατευθυντήριες γραμμές της Αμερικανικής Παιδιατρικής Ακαδημίας για τη γυναικολογική εξέταση της εφήβου στο παιδιατρικό ιατρείο:Δημοσιεύθηκαν το Σεπτέμβριο του 2010, και απευθύνονται στον παιδίατρο που ασκεί την κλινική του πρακτική στο ιατρείο.Σύμφωνα με τους συγγραφείς, ο παιδίατρος βρίσκεται σε ιδανική θέση να ελέγξει την έφηβο για συνήθη γυναικολογικά προβλήματα, πολλές φορές χωρίς καν να προβεί σε πυελική εξέταση. Βασιζόμενος δηλαδή στο ιστορικό της εμμήνου ρύσεως και των αναφερόμενων γυναικολογικών συμπτωμάτων, την εξέταση των μαστών και την επισκόπηση των έξω γεννητικών οργάνων.
Σύμφωνα με αυτό το άρθρο, πυελική εξέταση της εφήβου στο παιδιατρικό ιατρείο απαιτείται σε συγκεκριμένεςενδείξεις.Παραθέτωτον πίνακα των ενδείξεων για πυελική εξέταση όπως τον έχω μεταφράσει με ελαφρές τροποποιήσεις. Θεωρώ ότιμας παρέχει ταυτόχρονα και τις κύριες ενδείξεις απεικόνισης της εφηβικής πυέλου.
Απεικόνιση της εφηβικής πυέλου μπορεί να επιτευχθεί με τρεις μεθόδους: Υπερηχογραφία Μαγνητική τομογραφία Υπολογιστική τομογραφίαΥπερηχογραφία: η κύρια μέθοδος απεικόνισης.Πλεονεκτήματα: Ευρεία διαθεσιμότητα, ανώδυνη & ακίνδυνη,χαμηλό κόστος, απουσία ιοντίζουσας ακτινοβολίας, υψηλή χωροχρονική ανάλυση και σκιαγραφική αντίθεση.Μη χρησιμοποιείται άλλες μεθόδους χωρίς να έχετε εκτιμήσει τις πληροφορίες που παρέχει ένα αξιόπιστο υπερηχογράφημα. Μαγνητική τομογραφίαΣυμπληρωματική μέθοδος απεικόνισης, ιδίως για την εκτίμηση μεγάλων εξαρτηματικών όγκων και συγγενών ανωμαλιών. Και εδώ, σημαντικό πλεονέκτημα η απουσία ιοντίζουσας ακτινοβολίας. Επιτρέπει δυναμική ενίσχυση των εικόνων με χρήση ειδικών σκιαγραφικών υλικών.Χρονοβόρα. Πρόβλημα το κόστος. Θέματα κλειστοφοβίας. Λιγότερο κατάλληλη για επείγοντα περιστατικά. Προτιμότερο να κατευθύνεται η χρήση της από τον ιατρό που θα αναλάβει την τελική διαχείριση της ασθενούς.Υπολογιστική τομογραφίαΤαχεία ταυτόχρονη απεικόνιση κοιλίας και πυέλου, ιδίως όταν το περιστατικό έχει επείγοντα χειρουργικά χαρακτηριστικά.Χρήσιμη και στην σταδιοποίηση ωοθηκικών όγκων.Σημαντικότατη η ακτινική επιβάρυνση.Προτείνω να χρησιμοποιείται μόνο σε νοσοκομειακό χώρο.

The normal ovary in a woman of reproductive age has a varying appearance throughout the menstrual cycle, which may include multiple developing follicles, one or more dominant follicles, and a corpus luteum.At US the follicles appear as multiple, thin and smooth walled, round or oval, anechoic spaces with no flow by means of color Doppler US (ie, appearing as simple cysts). The size of the dominant follicle at ovulation averages 2.0–2.4 cm, with a range of 1.7–2.8 cm. For simplicity, the panel felt that follicles or simple cysts up to 3 cm in maximal diameter should be considered normal physiologic findings.After ovulation takes place, the dominant follicle turns into a corpus luteum.The corpus luteum is typically a cyst with diffusely thick walls and crenulated inner margins, measuring less than 3 cm in maximal diameter. It usually has internal echoes and a ring of vascularity at the periphery at color Doppler US.
Polycystic ovary syndrome is the commonest endocrine disorder in women and typically presents during adolescence.PCOS in adolescents arises as a result of a genetically determined disorder of ovarian function that results in hyper-secretion of androgens.The criteria fulfilling sufficient specificity and sensitivity to define the PCO should have at least one of the following: either 12 or more follicles measuring 2–9 mm in diameter, or increased ovarian volume (>10 cc).The presence of a single PCO is sufficient to provide the diagnosis.
Ultrasonography continues to be the primary modality used to identify and characterize adnexal masses.Adequate characterization of an adnexal mass is important both to determine which patients need surgery and to help define the type of surgery.The critical clinical questions to the radiologist in the setting of adnexal lesions are the site of origin, benign versus malignant features, and presence of infection or abscess.Pairing clinical presentation and imaging findings will direct appropriate management, whether it is reassurance, follow-up imaging, or surgery.
The subjective approach, also called a pattern recognition approach, has been shown to be superior to other methods, with a sensitivity of 88%–100% and specificity of 62%–96% for predicting malignancy. The high accuracy of the subjective approach was obtained in specialized centers with experienced imagers. The quality of US scans may not be as consistent in less specialized centers, although the knowledge and skill set needed to reproduce this degree of performance should be attainable by anyone who routinely performs gynecologic US.
US demonstration of a solid component within a cystic mass is the most important predictor of malignancy, and conversely, malignancy is very unlikely in the absence of a solid component.Terminology to describe the solid component varies and also includes papillary projection, excrescence, vegetation, and nodule.Color or power Doppler US can be helpful in this distinction, with absent flow characteristic for clot and definite visible flow typical for neoplasm.
Ultrasound image of a multilocular cyst in a 52-year-old patient with a mucinous cystadenoma. 10% of multilocular cysts proved to be malignant.Septa in a cystic ovarian mass are strong evidence of a neoplasm and are more likely to indicate malignancy if they are greater than 2–3 mm in thickness or have detectable flow on Doppler US scans. A cystic ovarian mass with septa (particularly when thin) but without a solid component is likely to be a benign neoplasm, though occasionally may be malignant when there are a very large number of septa. A cystic mass with multiple, smooth, thin septa and no nodularity is suggestive of a mucinous cystadenoma.
A simple cyst is a round or oval anechoic space with smooth thin walls/ posterior acoustic enhancement/ no solid component or septation/ and no internal flow at color Doppler US.Most simple ovarian cysts are follicular cysts, occur in premenopausal patients, and will resolve within 1–2 months. A few simple ovarian cysts, particularly larger ones or those in older women, are serous cystadenomas.In women of reproductive age:1. Cysts ≤3 cm: Normal physiologic findings; do not need follow-up. 2. Cysts >3 and ≤5 cm: do not need follow-up. 3. Cysts >5 and ≤7 cm: they are almost certainly benign; yearly follow-up with US recommended. 4. Cysts >7 cm: Since these may be difficult to assess completely with US, further imaging with magnetic resonance (MR) or surgical evaluation should be considered.
Hemorrhagic ovarian cyst. Transverse transvaginal US image of the adnexa shows a complex hemorrhagic cyst with the characteristic lacelike echogenic pattern of fibrin strands that form as blood clots and retracts.

Κατά τη διάρκεια της περιόδου ο ιστός αυτός αποπίπτει, όπως και το φυσιολογικό ενδομήτριο, χωρίς όμως να βρίσκει δίοδο, με αποτέλεσμα την εσωτερική αιμορραγία, τη φλεγμονή των γύρω περιοχών και το σχηματισμό ουλώδους ιστού.Endometriosis can occur in adolescents and this patient group presents particular challenges in terms of differential diagnosis, variable presentation and symptoms, and choice of treatment. Early diagnosis is essential in order to decrease pain and hopefully prevent disease progression and preserve future fertility.Οι πιο κοινές θέσεις όπου αναπτύσσεται η νόσος είναι κυρίως η πυελική περιοχή: the ovary (OV), pouch of Douglas-rectosigmoid (D), uterine ligaments (Lig), uterus (U), and fallopian tube (Tube); less common sites are the vagina (V), cervix (Cx), and bladder (B).
Sonographically, many endometriomas demonstrate internal homogeneous ground-glass or low-level echoes, without internal color Doppler flow, wall nodules, or other neoplastic features; in such masses, the additional features of multilocularity and/or tiny echogenic wall foci can help distinguish them from an acute hemorrhagic cyst. When these additional features are not present, an initial short-interval follow-up with US (6–12 weeks) in a woman of menstrual age is helpful to ensure that an acute hemorrhagic cyst has not been mistaken for an endometrioma.Cystic masses with classic features of an endometrioma should be followed with US if they are not removed surgically. The frequency of follow-up is variable, and should be based on factors such as the patient’s age and clinical symptoms, such as pain. In general follow-up should be at least yearly, to ensure that the cyst is not progressively enlarging and not changing in internal architecture (for example, new development of a solid element).
Endometriomas may contain a small solid area in 4%–20% of cases, and these can simulate the mural nodule of malignant neoplasm. While this most likely is due to clot or endometrial tissue, Doppler US can sometimes depict flow when the solid area is endometrial tissue, and the reliability of Doppler US in this setting is unclear.
Any ovarian or tubal mass predisposes a patient to ovarian torsion. Of course, ovarian torsion can occur without a lead point secondary to excessive mobility of the ovary or fallopian tube. In either situation, torsion is a high-stakes imaging diagnosis and surgical emergency. The diagnosis may be particularly challenging if torsion is intermittent.
The other adnexal emergency, which occurs in postpubertal girls, is ectopic pregnancy—hence, the importance of the pregnancy test in the clinical evaluation. Any complex adnexal mass with an empty uterus and positive pregnancy test is highly concerning. The presence of complex ascites strengthens the case.Risk factors include pelvic inflammatory disease, previous ectopic pregnancy, intrauterine device, in vitro fertilization, and tubal surgery. 1.3—2% of all reported pregnancies are extrauterine.
