Overview of clubbing in pediatrics, grades and the clinical significance.
5 things to know about Pulse Oximeter.
What is a pulse oximeter?
A pulse oximeter is a device that is used to measure oxygen saturation levels (SPO2 levels) / oxygen levels in the blood. The pulse oximeter gives out two readings which are oxygen levels and pulse rate.
What happens when oxygen levels drop below the normal oxygen level in the body?
It is very important for the oxygen levels in the body to be in the normal range. The proper functioning of the body depends on oxygen which is carried by the blood to all the cells in the body. When the oxygen levels in the body drop, the functioning of various organs like the brain, heart, and kidneys can get affected. The condition of low oxygen levels in the body is called hypoxemia.
Importance of fingertip pulse oximeter in the times of COVID-19
According to medical experts, low oxygen levels are registered in coronavirus patients. So, it is important to monitor the oxygen levels in the blood to detect covid pneumonia in the early stage. COVID pneumonia is a deadly condition seen in severe covid cases.
But a pulse oximeter cannot be trusted solely to detect coronavirus. Low oxygen levels can be a symptom of various other conditions as well.
What is the normal oxygen level reading?
You can find the normal pulse oxygen level readings here 0:49
When the oxygen levels drop below the ideal oxygen level readings, it is a cue to consult a doctor.

Measurement of Blood Oxygen Saturation with a Pulse OximeterMalik Mehrose
Measurement of blood oxygen saturation with a pulse oximeter
PULSE OXIMETER
The pulse oximeter is a small, clip-like device that attaches to a body part, like toes or an earlobe.
It’s most commonly put on a finger, and it’s often used in a critical care setting like emergency rooms or hospitals.
BLOOD OXYGEN SATURATION
Oxygen saturation is the fraction of oxygen-saturated hemoglobin relative to total hemoglobin (unsaturated + saturated) in the blood.
When using a pulse oximeter, healthy numbers fall in the 94% to 99% range. Anything lower than 90% indicates a problem with your blood oxygen levels.
The levels of saturation are affected by a variety of agents including the amount of oxygen in the air around you and nutrient deficiencies.
Measures and displays
O2 saturation of Haemoglobin
Pulse Rate
Indicates Perfusion
Pulsate Flow
PRINCIPLES OF PULSE OXIMETERY operation
PRINCIPLE 1
Infrared absorption by oxygenated and deoxygenated haemoglobin at 2 different wavelengths.
PRINCIPLE 2
The success of pulse oximetry depends upon its ability to measure the saturation of the arterial blood by analysis of infrared absorption of vascular bed throughout the whole pulsatile pulse cycle.
PRECAUTIONS
Check battery capacity always when the system is used in the portable mode .
Apply the sensor in a manner that allows the light source (light- emitting diodes) to be directly opposite the light detector (photodetector) to determine a pulse oximetry value properly.
Gently position the sensor so that it does not cause restriction to arterial flow or venous return because the pulse oximeter is unable to distinguish between true arterial pulsations and fluid waves .
Never place the sensor on an extremity that has decreased or absent sensation .
Allow 30 seconds for self-testing procedures and for detection and analysis of waveforms before value are displayed .
Wash hands to reduce transmission of microorganisms to other patients.
SpO2 stands for peripheral capillary oxygen saturation, an estimate of the amount of oxygen in the blood. More specifically, it is the percentage of oxygenated haemoglobin (haemoglobin containing oxygen) compared to the total amount of haemoglobin in the blood (oxygenated and non-oxygenated haemoglobin).SpO2 is an estimate of arterial oxygen saturation, or SaO2, which refers to the amount of oxygenated haemoglobin in the blood.
Pulse oximetry is a noninvasive method for monitoring a person's oxygen saturation. Peripheral oxygen saturation (SpO2) readings are typically within 2% accuracy (within 4% accuracy in the worst 5% of cases) of the more desirable (and invasive) reading of arterial oxygen saturation (SaO2) from arterial blood gas analysis.

Introduction
Regular and non-invasive assessments of cardiovascular function are important insurveillance for cardiovascular catastrophes and treatment therapies of chronic diseases.Resting heart rate, one of the simplest cardiovascular parameters, has been identified as anindependent risk factor (comparable with smoking, dyslipidemia or hypertension) for cardiovascular disease [1]. Currently, the gold standard techniques for measurement of thecardiac pulse such as the electrocardiogram (ECG) require patients to wear adhesive gel patches or chest straps that can cause skin irritation and discomfort. Commercial pulseoximetry sensors that attach to the fingertips or earlobes are also inconvenient for patientsand the spring-loaded clips can cause pain if worn over a long period of time. The ability to monitor a patient¶s physiological signals by a remote, non-contactmeans is a tantalizing prospect that would enhance the delivery of primary healthcare. For example, the idea of performing physiological measurements on the face was first postulated by Pavlidis and associates [2] and later demonstrated through analysis of facial thermalvideos. Although non-contact methods may not be able to provide details concerning cardiacelectrical conduction that ECG offers, these methods can now enable long-term monitoring of other physiological signals such as heart rate or respiratory rate by acquiring themcontinuously in an unobtrusive and comfortable manner.Beyond that, such a technology would also minimize the amount of cabling andclutter associated with neonatal ICU monitoring, long-term epilepsy monitoring, burn or trauma patient monitoring, sleep studies, and other cases where a continuous measure of heart-rate is important. The use of photoplethysmography (PPG), a low cost and non-invasivemeans of sensing the cardiovascular pulse wave (also called the blood volume pulse) throughvariations in transmitted or reflected light, for non-contact physiological measurements has been investigated recently. This electro-optic technique can provide valuable informationabout the cardiovascular system such as heart rate, arterial blood oxygen saturation, blood pressure, cardiac output and autonomic function.

Pulse oximeter – how does it work and how can it be used in home care?
1. Pulse Oximeter
How does it work and how can it be used in home care?
www.silverlinemeditech.com
2. Pulse oximeter is used to measure the oxygen level in the blood and also check your heart rate. A fing
er pulse oxymeter for home use is small, lightweight and painlessly attaches to a finger tip so that it ca
n monitor the amount of oxygen that is carried in the body.
The device should show an oxygen level greater than 95% to be considered normal.
Oxygen levels lower than 92% are considered as low. In addition to this, the device also displays the p
ulse rate. Around 60-80 beats per minute is considered as normal. It is common for heart beat to incre
ase with some exercise and the oxygen saturation may decrease – although it should remain at 90%
or more to be normal.
Introduction
3. The pulse oximeter sends a repeated series of two distinct pulses – a red pulse and an infrared pulse
through the patient’s finger. The oxygenated hemoglobin in the blood transmits a different amount of r
ed and infrared light than non-oxygenated hemoglobin. By measuring and caliberating the relative abs
orption of both the red and infrared light, the amount of oxygen is determined.
How does it work?
4. As compared to drawing blood and testing in a lab, the home pulse oximeter definitely has some adva
ntages.
• The very first advantage is that it is non-invasive and hence all the potential risks of an invasive type
test (such as drawing blood for testing) are eliminated. Infections caused due to drawing blood from th
e finger can be avoided.
• Secondly, since the light pulses are extremely fast, you get the results instantly. In around 10 second
s, you know what the results are. This kind of speed is extremely important in case of emergencies. E
specially for high performance athletes and pilots flying over a 10000 feet where oxygen becomes dilu
ted.
Advantages of a Home Oximeter
5. One of the main disadvantages of the oximeter that uses red and infrared light is that – just by sendin
g light pulses through the fingertip, it is not possible to determine the exactl volume of blood that is sa
mpled by the light and hence may not be an accurate estimate.
The measurement of oxygen saturation in the blood is only relative. It gives the relative ratio of the oxy
genated hemoglobin concentration vs the non-oxygenated hemoglobin concentration.
If you get a lower reading on the oximeter, one factor could be the oxygenated hemoglobin definiency
and the other reason could be the deficiency of hemoglobin itself.
Disadvantages of a pulse oximeter
6. However, this home care device is a good place to begin with. If you suspect greater reasons for the l
ower reading, you always have some solid backing to base your blood tests on. Other than that, for ho
me care, the finger pulse oximeter should suffice.
