A head to toe assessment involves assessing the level of consciousness and orientation by checking the person, place, and time. It also includes comparing the bilateral upper extremities by checking hand grasps, temperature, color, pulses, capillary refill, and sensation. Additionally, the apical pulse is assessed by auscultating the heart sounds at the point of maximal impulse for one minute to check the rate and regularity. The lungs are also assessed by listening to breath sounds at various sites on the chest and counting the respirations.
