A head to toe assessment involves assessing the level of consciousness and orientation by checking the person, place, and time. It also includes comparing the bilateral upper extremities by checking hand grasps, temperature, color, pulses, capillary refill, and sensation. Additionally, the apical pulse is assessed by auscultating the heart sounds at the point of maximal impulse for one minute to check the rate and regularity. The lungs are also assessed by listening to breath sounds at various sites on the chest and counting the respirations.

This separates the mucus from the lung walls so it can be coughed out. The huff coughing technique should be done sitting up straight with the chin tilted slightly up and mouth open. A cycle of four to five huffs followed by a strong cough helps clear mucus from the lungs.
