Mutilating injuries of the upper extremity often require complex reconstruction involving soft tissue, nerve, vascular, and bone procedures. A multidisciplinary approach is needed, beginning with thorough debridement and stabilization, followed by vascular reconstruction, tendon and nerve repair, and soft tissue coverage. Replantation or revascularization may be considered for amputated parts, depending on the severity of injury, ischemia time, and likelihood of functional recovery. Close postoperative monitoring and therapy are important to optimize outcomes from these challenging injuries.
