Remineralization is defined as the process whereby calcium and phosphate ions are supplied from an external source to the tooth thereby, causing ion deposition into crystal voids in demineralized enamel, thus producing net mineral gain.
Remineralization Agents - Biomimetic approaches to stabilization of bioavailable calcium, phosphate, and fluoride ions and the localization of these ions to non-cavitated caries lesions for controlled remineralization.
Fermentable carbohydrates provide plaque bacteria with substrate for acid production.
This causes a rapid drop in plaque pH and when pH becomes less than 5.5, hydroxyapatite in enamel breaks down and calcium and phosphate ions diffuse out from the enamel.
5.5 is the “critical pH”, the point where equilibrium exists. There is no mineral dissolution and no mineral precipitation.
The plaque remains acidic for about 30-60 minutes after which normal pH is restored gradually.
The calcium and phosphate ions re-enter enamel when normal pH is restored and thus remineralization occurs.

ROLE OF FLUORIDES IN ALLEVIATION OF DENTAL CARRIES
1. Tooth enamel comprises of 96% of Crystalline
Hydroxyapatite
2. Hydroxyapatite crystal structure
consists of Phosphate ions (PO4
2-),
Calcium ions (Ca2+)and hydroxy ions
(OH-) ions
3. Caries causing bacteria metabolize on the carbohydrates
provided to them through the food that we take, producing
organic acids as byproducts.
This results in change in pH of saliva.

13. Stannous fluoride is considered the best as it not only promotes re-mineralization and
impedes demineralization but also provides efficacy against bacteria.
DENTIN HYPERSENSTITIVITY
Dental pain which is sharp in character
This arises when the dentin surfaces are
exposed to thermal, electrical and chemical
stimuli.
9. When dentine are exposed to thermal stimuli, a very sharp pain is
generated.
2. Through these tubular structures a plasma like biological fluid
flows. Owing to different stimuli, changes in flow of this fluid takes
place inside the dentin.
For mild to moderate pain associated with several teeth,
OTC oral products are used
Mechanism of action: They may act by occlusion of dentin
tubules or desensitization of nerve fibres/ blocking
nueronal transmission (pottasium containing compounds).
10. 1. Oral products containing Strontium chloride (SrCl2) , Strontium acetate (C4H6SrO4) ,
Calcium carbonate (CaCO3) , Arginine, Hydroxyapatite and Calcium sodium phosphosilicate
acts by occluding dentine tubules .
2. Oral products constituting of Pottasium compounds such as pottasium chloride, pottasium
citrate and pottasium nitrate act by causing depolarization and preventing repolarization,
thereby blocking transmission of nerve signals.
CALCIUM CARBONATE
CaCO3 occurs as minerals – calcite and aragonite . Both minerals
constitute limestone.
Coral reef is a kind of limestone rich in both minerals.
The sedimentary rock (coral reef) constitutes CaCO3 shells of
molluscs, snail, oyster etc
Limestone are also formed due to chemical process by the
percipitation of calcium from sea water
11. MEDICINAL USE OF CaCO3
The action of abrasives is
aided by a toothbrush
As antacid, to relieve hyperacidity, gas ,heartburn and upset
stomach (Along with magnesium salts and simethicone)
12. As phosphate binder in patient undergoing hemodialysis and having
hyperphosphotemia.
13. Colorless solid
Odorless
Readily soluble in water
Used in municipal drinking water. Flourine in flouridated water replaces –
OH group of Hydroxyapatite in Tooth enamel to form Flouroapatite.
Flouroapatite is more resistant to demineralization process and also
resists acid formation by cariogens.
It is cheapest among all other flourine compounds and is therefore
economical.
NaF is also included in toothpaste to prevent dental caries.
14. Radioactive compound Flourine-18 labeled NaF is used as tracer
compound in Positron Emission Tomography (PET).
It is a superior radiopharmaceutical diagnostic tracer for skeletal
imaging because of its rapid adsorption on bone surface.
ZINC EUGENOL CEMENT
Zinc-Eugenol is
formulated by
combining Zinc

Eugenol from
Clove oil in
presence of
water and other
metal ions.

reaction
takes place
with the

15. MEDICINAL VALUE OF ZINC EUGENOL CEMENT
For filling of cavities, chiefly when the cavity has invaded the
dentine and is very close to the pulp chamber.
For temporary filling of dental caries
16. In case of dry sockets, zinc eugenol paste is applied over the alveolar bone.
This reduces the acute pain associated with its inflammation.
Used as impression material for the construction of dentures
