Low back pain
12/07/15 Progress Report described a follow up visit. The patient continued to report
adequate pain with the use of Vicodin and Flexeril. Review of utilization review
report indicated that these medications were no longer certified. However, TPI trial
with anesthetic has been certified. The patient noted 5/10 scale level pain in the
lumbar region without radiation. He denied any side effects of the medication.
Examination revealed trigger points with a jump response on palpation of the lumbar
paraspinal muscles. The ROM is restricted in the lumbar region secondary topain and
stiffness. Faber sign is positive on the left. SI joints are non tender. SLR and sitting
SLR are negative. Treatment plan included Vicodin and Flexeril. Work status: Return
to full duty; avoid lifting 20 lbs and frequent bending activities.
11/19/15 UR report showed modified certification. TPI 3 X trial was certified and
Vicodin and Flexeril were denied.

infection in the United States and is associated with high mortality and morbidity
(Seymann, 2008). This paper is a case study of a 52 year old female who was in the
hospital for a scheduled gastric bypass surgery. During a post op test she aspirated
dye thus beginning the process of her developing nosocomial pneumonia. The patient
was discharged only to return to the emergency department the following day

Ms. G. was admitted to the hospital for scheduled gastric bypass surgery on 9/14
/2010. On 9/15/2010 she aspirated dye during a post operative test. This began the
process of aspiration caused pneumonia for Ms. G. Signs and symptoms of
respiratory problems began immediately. However she was released on 9/17/2010.
She was admitted for pneumonia on 9/18/2010. Ms. G. stated that the two
radiologists began arguing because the first radiologist was forcing me to drink
faster than I could tolerate due to my gastric bypass surgery. She began choking on
the dye and aspirated it into her lungs.
Being a single parent to a teenager, Ms. G. has severe anxiety related to being sent
home without any type of oxygen supplementation and not being able to breathe,
which is exactly what happened after her gastric bypass surgery. Ms. G also was
very concerned about the fact that she had planned out her gastric bypass surgery
knowing she had only a certain amount of sick pay to cover the missed time from
her job. As a result of acquiring the nosocomial pneumonia she is now faced with
losing additional time from work with no pay. Recovering from the severity of her
illness could possibly take weeks during which time she will have no income.

Gleaners Teamwork
What I liked most about the volunteer work at Gleaners was the result of the work
itself. It was satisfying to hear the supervisor give us the total of 15 tons sorted
through. It put a number on the amount of help we were doing, and it made the
work feel even more rewarding to know how much we accomplished. It also was
amazing to hear how, if not for our presence, all of said work would ve fallen on that
one supervisor. I cannot fathom how he could manage to do that much work in a
decent amount of time, so it only added to that feeling of doing something good. I
also enjoyed the teamwork in the work itself because it always feels good to be in
sync with other people. It gives a sense of comradery, and in this type of volunteer
work it also establishes a base of trust. The team effort helped for the project to move
quicker, and it made it much more enjoyable.
I would have to say what I liked least about the volunteer project was the labor, in
and of itself. It was a lot to manage at once and it was especially difficult

This debate on whether women should be in military combat roles or not, has no in

should not go into combat. The two extremes sides of this argument offer numerous

Ticket scalping is when event tickets purchased from primary markets (such as box
offices) are resold on secondary markets. Tickets are purchased at official prices set
by ticketing agents (P), and then sold at higher prices, to those who are willing to
buy it (Ps). Therefore, profits drive scalpers to initiate such activities, which leads to
excess demand, and ultimately not allowing the market to clear.
When tickets (the official fixed amount, therefore having perfectly inelastic supply)
is sold out, there is a shortage and excess demand is present, as Qd is more than Qs.
Scalpers then increase their ticket selling price from P to Ps, reducing demand closer
to Qs, until it reaches Qs=Ps.

As seen from the surplus diagram above, scalping does not affect producer surplus
(ticket office) and total surplus. This is because the supply curve is perfectly
inelastic, which leads to scalpers gaining a surplus, as they are able to sell at a
higher price than expected. This causes consumer surplus to decrease, as consumers
have to pay more for scalpers tickets.

the Pentium 4 HT, which meant that the CPU could be fooled into thinking it had
two CPUs for each one that it actually had. Using hyper threading along with
additional cores has enhanced performance and speed because some cores are
utilized for programs while others perform background jobs (Hoffman, 2014,
paras. 6,7). Another way CPUs have been able to increase speed is by raising the
number of cores per CPU socket, and utilizing an I/O Hub called QuickPath

Caring for Populations: Part I Huntington, WV was named by the Center for Disease
Control in 2010 as the least healthy city in the least healthy state of America in the
most overweight nation in the developed world (Kilmer,et.al, 2008). Jamie Oliver,
known as the Naked Chef, came to Huntington in the fall of 2010 to try to teach the
school cooks how to prepare a healthier menu that the school children would eat. He
was not met with open arms by the school cooks, the school system or children.
Huntington, WV is a dying town in the rust belt of the United States that has lost its
manufacturing and railroad employment. Unemployment is worse than the national
average and, poverty hovers around 19% much greater than the national average...
But when you drive around Huntington, the majority of the homes are in disrepair.
A significant portion are rented out to college students. Then there is another
significant portion around the old industries that are in disrepair. Once the railroad
tracks are crossed to go around the city park, there are older, architectural
masterpieces where the wealthy live. There are very few yards of any size in town.
People can get around in town by bus normally. There is a bike rack at the college
but there are rarely any in it. Residents have to travel out of Huntington to shop at the
mall, but when they want to hang out there is a large city block downtown that has
restaurants, theaters, comedy club, etc. Street people hang out behind this on the
river front behind the flood wall out of site. There is a tent city allowed there by the
city. There are multiple churches throughout the city, but most of them reflect the city
and are in disrepair as well. There are several hospitals in the city. One affiliated with
the medical school that rotates trauma call with a Catholic Hospital that specializes in
heart disease and meeting the needs of the community. There are two more within a
fifteen minute drive.
Problem and Significance
Childhood obesity leads to many health problems in adult hood. Obese children
experience an increase in asthma (all time high now), skin diseases, joint pain and
there is an increase in diabetes
