11. A sixty fourth year old right-hand dominant
woman reported a two-year history of pain in
the left shoulder. She did not recall any
particular injury or accident, but the pain
started in severity and Range of Motion is
limited. She rested the shoulder for three
months, but the pain persisted. A magnetic
resonance imaging scan was done by her
primary care physician at MEDIC center
18. Usually, there is enough room
between the acromion and the
rotator cuff to allow the tendons
to slide easily underneath the
acromion as the arm is raised.
Each time the arms are raised,
there is rubbing on the tendons
and the bursa between the
tendons and the acromion,
which is called impingement

20. CLINICAL ISSUES
Most common signs/symptoms: Pain with abduction and external
rotation.
Clinical profile:
Athletes participating in overhead throwing sports.
Non athletes who frequently abduct and externally rotate the arm
(occupational)
TREATMENT
1. Physical therapy: Rotator cuff strengthening.
2. Arthroscopic debridement of rotator cuff and labral fraying.
3. Repair rotator cuff tear.
4. Humeral head derotational osteotomy.

22. I) MỤC TIÊU:
MR Arthrogaphy được sử dụng để đánh giá các tổn thương phức hợp sụn
viền-dây chằng bao khớp trước dưới, dây chằng ổ chảo cánh tay dưới và các
tổn thương khác đi kèm (rách bán phần gân chóp xoay, SLAP lesion, Hill
Sachs…) trong bệnh lý mất vững khớp vai trước.

23. Hippocrates đã ghi nhận các trường hợp mất vững khớp vai trước trong
y văn.
Năm 1923, Bankart mô tả những tổn thương của trật khớp vai sau chấn
thương trong tạp chí British Medical Journal
Năm 1948, Putti-Platt.
Năm 1954, Latarjet mô tả và đề ra các thức điều trị Bristow.
Ngày nay mất vững khớp vai trước được hiểu như là một chấn thương
của mô mềm hoặc xương, làm cho đầu xương cánh tay bị bán trật hoặc
trật ra khỏi ổ chảo, làm mất chức năng khớp vai.

27. MR Arthrography
Direct
Tiêm dung dịch Gado+Normal Saline
tỉ lệ 1/250
(Tuơng ứng 0,5ml Gd+50ml Normal
saline) vào khớp
Indirect
Tiêm gado đường tĩnh mạch liều
0.1mmol/kg
Sau khi tiêm sẽ khảo sát MR imaging bằng chuỗi xung T1WI fat
sat spin echo, với các látcắt Axial, Coronal, Sagittal.

3- đánh giá được các tổn thương nhỏ và kín đáo, đánh giá được thể tích bao

31. Hồi cứu 18 trường hợp chụp MR Arthrography tại Trung Tâm Y Khoa
Medic từ 08/2006 đến 8/2007. Qua đó chọn lọc được 10 case (56%) được
xác nhận lâm sàng là mất vững khớp vai trước, có tuổi từ 16 đến 70 tuổi,
trong đó có 6 nam và 4 nữ.
Các bệnh nhận này đều được khảo sát MR Arthrogaphy, với máy cộng
huởng từ GE CVI họăc Signa 1.5T.
Coil chuyên dụng là shoulder surface coil
Các lát cắt sagittal, coronal, axial T1W fat sat. TR 700, TE 16-20.
Và được tiến hành sau 15 phút tính từ lúc bơm (10-20 ml) dung dịch
Gado+Normal Saline tỉ lệ 1/250, vào bao khớp vai.
III) Phương pháp và đối tượng nghiên cứu:
32. Ghi nhận 7 ca (70%) có tổn thương phức hợp sụn viền dây chằng bao
khớp trước dưới với nhiều mức độ khác nhau (Bankart và variants
Bankart’s lesion) , 2 ca (20%) có tổn thương Hill Sachs, 4 ca (40%) có rách
bán phần gân chóp xoay (rách gân cơ dưới vai và trên gai, không ghi nhận
được ca nào rách gân cơ dưới gai), 3 ca (30%) có tổn thương SLAP, và 1
ca (10%) có tổn thương dật sứt hòan tòan dây chằng ổ chảo cánh tay dưới,
phần gắn vào sụn viền dưới. Có 1 ca (10%) duy nhất hầu như không phát
hiện được tổn thương.
IV) Kết quả và bàn luận:

42. Trong đề tài của chúng tôi, cho thấy rằng tổn thương thường gặp nhất trên MR
Arthrography của bệnh lý Anterior Glenohumeral Instability (AGI), là
Bankart’s lesion (70%), điều này cũng phù hợp với y văn, Taylor and Arciero
báo cáo tỉ lệ này hơn 90%, Amin Afsari và cộng sự là 85%.
Các tổn thương khác trong báo các của chúng tôi chiếm tỉ lệ từ 10-40%,
Amin Afsari và cộng sự là từ 26-100%.
Các tác giả Chandnani et al, Palmer et al, Palmer and Caslowitz, and Tirman
et al, cho rằng độ nhạy và độ chuyên của MR Arthrography là 89% và 96%,
của chúng tôi là 90%.
BÀN LUẬN
43. MR Arthrography có thể phát hiện chính xác các tổn thương
trong bệnh lý mất vững khớp vai trước, gây trật khớp vai tái
hồi, góp phần rất lớn vào viêc hiểu rõ cơ chế bệnh sinh và
phương thức điều trị cho bệnh nhân.
MR Arthrography là phương tiện hữu ích giúp đánh giá bệnh
lý AGI trước phẫu thuật nội soi, và đặc biệt giúp chọn lọc các
bệnh nhân nên phẫu thuật mở.
V) Kết luận:
45. BN nữ 54 tuổi, đến khám bệnh vì đau vai phải, giới hạn vận
động vai phải > 3tháng, đã điều trị nhiều nơi chưa giảm.
Khám lâm sàng: HC Frozen shoulder.
Không ghi nhận tiền sử chấn thương vai, không chơi thể thao.
- CĐ sơ bộ: Td rách bán phần gân chóp xoay.
- Chẩn đóan PB: Viêm bao khớp dưới vai.
Đề nghị: MRI
CASE 1:

Hình coronal T1WI (580/22) cho thấy gân cơ
trên gai tăng tín hiệu, phù hợp với chẩn đóan
rách bán phần. Sụn viền trên bị rách kiểu
SLAP type I, có liên quan đến tổn thương
dạng nang vùng cổ xương bả vai.
Hình coronal T2WI (3500/98) fat sat cho thấy gân
cơ trên gai tăng tín hiệu, phù hợp với chẩn đóan
rách bán phần. Sụn viền trên bị rách kiểu SLAP
type I, có liên quan đến tổn thương dạng nang
vùng cổ xương bả vai.

50. BN: Nam 40tuổi, đến khám bệnh vì teo cơ vùng sau vai tình cờ phát hiện khi soi
gương, không giới hạn vận động khớp vai, đau vùng cơ dưới gai khi đè ấn.
CASE 2:
51. BN: được thăm khám để tìm nguyên nhân teo cơ.
Dấu Spurling sign (-), PXGC bình thường 2 tay.
Không có dấu yếu liệt.
Apprehension test (-).
ROM bình thường.
Ấn đau vùng cơ dưới gai.
CĐ sơ bộ: teo cơ dưới gai khu trú nghĩ đến chèn ép thần kinh tại chổ,
hoặc do bệnh lý cơ nguyên phát.
Đề nghị: MRI khớp vai

Tổn thương dạng nang nằm sau trên cơ dưới gai
53. Hình coronal T1WI (580/22) cho thấy 1 tổn thương dạng nang (tín hiệu thấp), nằm
ở vị trí cổ xương bả vai, trên cơ dưới gai và dưới cơ trên gai.
Hình sagittal PWI Fat sat (3000/41), cũng cho hình ảnh tương tự về vị trí của tổn
thương, vị trí này cũng chính là vị trí của nhánh thần kinh trên vai (suprascapular)
54. Hình axial T1 (580/22), T2WI (3500/90) fat sat cho thấy cơ dưới gai bị viêm teo
và thóai hóa mỡ.
55. DISCUSSION
- Qua 2 cases ghi nhận tổn thương cyst có liên quan đến rách sụn viền
(Labral tear), hoặc rách bao khớp- sụn viền (glenoid labralcapsular tear).
- Cơ chế sinh lý bệnh chưa được biết rõ. Giả thuyết được chấp nhận là
do tràn dịch khớp xuyên qua vị trí rách bao khớp vào mô kế cận.***
- Cyst có thể gây đau, hoặc đau là do nguyên nhân chèn ép nhánh thần
kinh trên vai gây hậu quả viêm teo cơ thoái hóa của cơ dưới gai.
- Labral cyst có thể đi kèm nguyên nhân khác (Partial rotator cuff tear,
SLAP Lesion), làm mất vững khớp vai (Instability shoulder)

56. MRI là phương tiện tốt nhất để chẩn đóan.
SA có thể thấy được nang sụn viền, nhưng thường khi đánh giá chóp
xoay hay bỏ sót vùng ngách gai vai ổ chảo.
MỘT SỐ NHẬN XÉT VỀ CÁC DẤU HIỆU TRÊN MRI CỦA LABRAL CYST:
1- Vị trí: thường là ở ngách gai vai và ổ chảo (spinoglenoid notch), chổ
tiếp giáp của gân trên gai và dưới gai, gây chèn ép nhánh xa của thần
kinh trên vai.
2- Hình dạng có thể dẹt hoặc tròn thường là có nhiều cyst
(multiloculated), kích thước thay đổi từ vài mm- vài cm.
3- Tín hiệu cao trên T2WI, PWI Fat sat và thấp trên T1WI. Các dấu hiệu
kèm theo có thể là rách sụn viền, rách chóp xoay, teo cơ dưới gai và
họăc trên gai.
57. Biểu hiện lâm sàng của bệnh labral cyst đa dạng thường hay nhầm lẫn với
bệnh lý chóp xoay.
86% trường hợp có biểu hiện TC của chèn ép nhánh thần kinh trên vai***
Vì vậy TC teo cơ vùng cơ trên gai và dưới gai là rất thường gặp, do đó bệnh
có thể gặp ở nhiều chuyên khoa khác nhau (thần kinh, cột sống, cơ xương
khớp …).
Nếu chẩn đóan sớm và chính xác khi chưa diễn tiến teo cơ thì khả năng
phục hồi sau phẫu thuật nội soi là rất tốt

68. Intramuscular injection can induce local hemorrhage,
ischemia, necrosis, and chemical myositis. Repeated
intramuscular injections, however, can lead to fibrosis
of the injection sites
DISCUSSION
69. Among the three deltoid portions, the intermediate
portion is the preferable site for intramuscular
injection
70. However, the intermediate portion with short
muscle fibers is most susceptible because even
fibrosis of only a few short muscles easily can
lead to coalition of the interspersed tendons and
subsequent development of DC
71. DC can lead to decreased muscle strength in
flexion, extension, and limited adduction of the
shoulder.
Distal release and partial resection of the fibrotic
masses, if necessary, result in excellent recovery.
MRI has been reported as the best technique to
delineate the hypointense fibrous cords in the
deltoid muscle

73. History and clinical:
A 43-year-old man’s right knee gave out while he was playing tennis.
There was immediate pain, swelling, and effusion. Initial clinical
evaluation revealed no definite instability. Radiographs of the knee was
normal. MR imaging revealed tears of the anterior cruciate ligament
and posterior horn of the medial meniscus, a moderate joint effusion,
and focal bone derangement at the insertion the central tendon of the
semimembranosus.
75. Bone abnormalities involving the posteromedial tibial plateau are
uncommon. The fracture is very difficult to detect when the avulsed fragment
is not displaced.The frequency of this type of fracture is probably greater,
owing to the diagnostic limitations of conventional radiographs in its
detection.
Posterolateral tibial plateau fractures, which are very often associated with
avulsion fractures of the posterior cruciate ligament.
Avulsion fractures noted at the posteromedial corner of the tibia may be
dismissed as capsular in origin. Furthermore small avulsion fractures about
the knee may be missed if oblique projections are omitted.

DISCUSSION
84. DISCUSSION
Vanek's cadaveric study findings showed that
the ACL was torn before the medial tibial
plateau translated anteriorly, which led to a
posteromedial tibial plateau fracture. Therefore,
the presence of a fracture of the posteromedial
tibial plateau appears to be predictive of an
associated ACL tear.
The mechanism leading to this type of injury,
however, is not clear.
We believe that a valgus force applied to the
tibia most likely causes external rotation and,
possibly, anterior subluxation of the tibia in a
mechanism similar to the one that leads to a
tear of the ACL.
A torque force on the posteromedial aspect of
the knee, which is may lead to an avulsion
fracture or a contusion of the posteromedial
tibial plateau.
85. Joint fluid that has leaked through resultant osteochondral defects.
When the ACL is torn, the resultant rotary instability causes both traction
to be applied to the medial aspect of the knee and impaction of its lateral
aspect. As a result, injuries occur to both the medial tibial plateau and the
lateral femorotibial compartment.
An abnormally deep lateral condylopatellar sulcus has been attributed to
an impacted osteochondral fracture and the other authors concluded that a
lateral femoral notch greater than 2.0 mm in depth was highly suggestive
of an ACL injury (inconstant finding)
DISCUSSION
86. In summary:
“that a fracture of the posteromedial tibial plateau is predictive of
an associated ACL tear, although the mechanism leading to this
type of bone injury is not clear”.

88. A 29-year-old female who was otherwise healthy
presented with a recent history of right knee pain,
radiation calf ( right calf pain when walking
approximately 500m), and normal arterial pulses were
present bilaterally in the femoral, popliteal, dorsalis
pedis, and posterior tibial arteries. There was no
evidence of ischemia

96. On the basis of the clinical and imaging findings, a
diagnosis of adventitial cystic disease (ACD) of the
popliteal arteries was made.
Diagnosis
97. We found it curious that despite the patient’s
smptoms, none of the imaging examinations revealed
any evidence of luminal stenosis in either popliteal
artery.
This was most likely due to the fact that ACD causes
a dynamic exercise dependent flow obstruction, and
all of the imaging examinations in this patient were
performed at rest.
Discussion
98. Discussion
ACD is a rare disease, representing 0.1% of vascular
disorders and accounting for one of every 1200 cases
of calf claudication
The clinical diagnosis of ACD is often difficult to
make because of misleading clinical features that can
suggest a musculoskeletal rather than a vascular
disorder.
Compression of the popliteal artery produced by the
cysts can be seen on axial images and on MR
angiographic.
99. ACD is a rare disease, but it should be suspected
in male patients aged 40–50 years who do not
have evidence of atherosclerotic disease and who
present with intermittent claudication.
US and CT or MR imaging may be used to arrive
at the correct diagnosis, avoiding the
interventional procedure of conventional
angiography.

102. KẾT QUẢ:
SA: Viêm gân gập ngón IV bàn chân phải
XN: BT, VS: h1: 20, h2: 40.
Cấp toa: 7ngày
1. Mobic 7,5mg uống ngày 2 lần, lần 1 viên
2. Losec 20mg uống ngày 1lần, lần 1viên.
BN: tái khám có giảm T/c >70%. Cho SA lần II,
KQ: Viêmgân gập ngón IV bàn chân phải, kích thước phù
nề giảm so với lần I.
Cấp toa: 7 ngày như lần 1.
1 tháng sau BN quay trở lại: đau trở lại.
Đn: MRI bàn chân phải. KQ: Morton’s neuroma.

ANATOMY
104. In 1876, Morton believed that the etiology was
related to compression of the nerve.
In 1948, Nissen believed that an ischemic.
In 1980, Bossley and Cairney believed the etiology was secondary to
an intermetatarsal bursa distal to the transverse metatarsal ligament
undergoing inflammatory changes that caused fibrosis around the
nerve.
In 1997, Mulder agreed with Bossley and Cairney and determined the
Mulder sign.
PATHOPHYSIOLOGY
105. A neuroma is a benign tumor of a nerve. Morton's
neuroma is not actually a tumor.
106. SYMPTOMS OF A NEUROMA
Begins with numbness or tenderness in the foot,
just behind the 3rd and 4th toes.
At a later stage, pain, numbness, burning and
tingling sensations can radiate around the foot.
Severe pain may be present at weight bearing.
The patient may experience spontaneous shooting
pains, which is often referred to as an "electric
shock".
This can affect patients when are sleeping at night.
107. Diagnosis
During the examination, your physician will feel
for a palpable mass or a "click" between the bones.
Range of motion tests will rule out arthritis or joint
inflammations.
He or she will put pressure on the spaces between
the toe bones to try to replicate the pain and look
for calluses or evidence of stress fractures in the
bones that might be the cause of the pain.
110. Treatment may include:
1. Wearing properly fitting shoes.
2. Taking anti-inflammatory drugs.
3. Wearing a pad below one of the bones in your

Conventional radiography (images not shown) depicted a soft-tissue mass,
without any soft-tissue calcification or skeletal abnormality. Magnetic
resonance (MR) imaging was performed.

126. The keys to assigning the correct diagnosis were patient age, history of recent
trauma, and a rapidly growing, painful solitary soft-tissue mass located in the
substance of the biceps muscle, together with MR findings. These clinical and
imaging findings were indicative of proliferative myositis (PM).
MR images of PM have been reported in several cases, with a hypo- or isointense
T1 signal compared with that of muscle and homogeneous enhancement. T2-
weighted MR images typically demonstrate a hyperintense soft-tissue mass
127. Differential considerations for a mass involving the skeletal muscle include soft-
tissue tumors, traumatic injury, myositis ossificans, muscle sarcoidosis, and various
causes of infectious and inflammatory processes.
The most common benign and malignant intramuscular soft-tissue tumors are
angiomatous lesions, lipoma, myxoma, malignant fibrous histiocytoma, and various
types of sarcomas
128. Soft-tissue angiomatous lesions and lipomas can be diagnosed confidently on the
basis of their specific signal intensity and morphologic characteristics on MR images.
Typically, T1-weighted MR images demonstrate soft-tissue masses of intermediate
signal intensity with interspersed areas of high signal intensity, as well as almost
homogeneous high-signal-intensity masses that correspond to fatty areas
129. Soft-tissue myxomas often display characteristic MR findings, including an
intramuscular location, a surrounding rim of tissue similar to fat, and high water
content resulting in signal intensity lower than that of skeletal muscle on T1-weighted
MR images and markedly high signal intensity on T2-weighted MR images (10). These
features made myxoma an unlikely diagnosis in this case.

130. The aggressive course of the clinical presentation may suggest malignant fibrous
histiocytoma and sarcomas in the differential diagnosis. However, the presence of
a painful mass following trauma; well-defined lesion margins; lack of infiltration in
the adjacent soft tissues; and, specifically, identification of the preserved muscle
fibers inside the lesion on MR images made these diagnoses less likely.
Homogeneous contrast enhancement of the intramuscular lesion on MR images
enabled us to easily exclude different categories of traumatic muscle injuries,
although the lesion developed after trauma. Rhabdomyolysis (Rhabdomyolysis
can also be ruled out by determining levels of muscle enzymes in the blood).
131. Myositis ossificans is another main cause of an intramuscular soft-tissue mass. It is
not an inflammatory myopathy. There is usually a history of local trauma, but it may be
seen in patients with paralysis, burns, tetanus, or an intramuscular hematoma. It may
also develop spontaneously (15). Several MR appearances corresponding to the
stages of maturation of myositis ossificans are noted (16,17). The area that may
ossify peripherally within 6–8 weeks is swollen and painful during the 1st days or
weeks of the acute phase, and it shows rim enhancement after contrast agent
injection at this early stage (17). Although the clinical features and unenhanced MR
imaging findings indicated a diagnosis of myositis ossificans, the enhancement
pattern enabled us to rule this out as the most likely diagnosis.
