The qualities of the contributor
A person who helps someone physically, financially or
emotionally ,then he/she is a ‘Real Contributor’.
There are many contributors in my life.
The contribution of my parents, relatives and friends are
more then others.
Similarly in society there are many contributors, like
doctor, teacher, farmer, scientist, donor, etc…
Dr.P.C.JAIN is the real contributor in my life.
He is a doctor so He also contribute for the society.
Therefore I choose him as the real contributor.
DR.P.C.JAIN was born on 5th August, 1972 in Vadodara.
He graduated in MBBS (Bachelor of Medicine and Bachelor of Surgery) from
Maharaja Sayajirao Geakwad University (MSU) of Vadodara.
He joined as a medical officer on 1st May,1997 at Primary Health Center,
Sadhali, Gujrat.
He is working as a Taluka Health Officer (Class II Gazetted) at Sankheda,
Dist. Chhotaudepur.
He has worked as a Medical Officer at Primary Health Center governed by
Panchayati Raj Institute (PRI) for last 18 years.
His main job is public health which aim to provide all health services
regarding not only medical care but also prevention of diseases.

Janani Suraksha Yojna, Janani Shisu Suraksha Yojna,
Rogi Kalyan Samiti etc.

Major cholera epidemic in Sankheda Village where
approximately 200 cases of diaria, vomiting & only one death.
Cholera epidemic is controlled only in 6 days with help of all
health workers & various department. This is the taffies
epidemic in her 18 years of service.
He had not seen his daughter for a week.
