nasal mucosa .

 Inflammatory condition of Unknown Origin
• Round, Smooth , Jelly like ,Non tender
• Translucent ,Yellow or pale glistening
structure
• Attached to nasal mucosa by relatively
narrow stalk or pedicle
• Commonly arise from Ethamoid Sinuses
• Bilateral & Multiple.
TYPES
 Common
1.Ethamoid
2.Antrochoanal
Rare
1.Sphenocoanal
2.True Choanal
Etiology-
• Combination of Allergy & Infection
• Cystic fibrosis & Salicylic acid
Hypersensitivity
• Repeated insult to nasal mucosa
• Repeated blowing of nose
ETHAMOID POLYP
 Arise from air cells of ethamoid labyrinth
 Grape like structure
 Examined by Anterior Rhinoscopy
Clinical Features-
Symptoms-
1. Nasal obstruction
2. Hyposmia or Anosmia
3. Nasal Discharge
4. Broadening of Nose
5. Headache
Signs-
1. Frog Face deformity
2. Reduced or absence of fogging
Anterior Rhinoscopy-
• Pale grayish or bluish white grape like masses
• Vascular pinkish appearance if infected
• Insensitive to touch .dont bleed on touch
• Present more anteriorly
Differential Diagnosis
1.Antrochoanal Polyp
2.Sphenochoanal Polyp
3.Rhinosporidiosis
4.Rhinoscleroma
5.Inverted Papiloma
6.Angiofibroma
Investigation-
1.Nasal Endoscopy
2.Radiography-a. X Ray PNS
b. CT-Scan –OMC
3. Allergy Test
4.Nasal Swab
5.Biopsy
Treatment-
1.Medical- Incomplete & aim at to relieve
Symptoms
2. FESS
3.Steroid Nasal Sprays
4.Antifungal
5.Nasal polypectomy
6.Ethamoidectomy
ANTROCHOANAL POLYP
 Polyp originated in the maxillary sinus
 Protruding in the meatus
 May extend posteriorly through the choana into
NPX/OPX
• Uncommon Polyp
• Good inspiratory airway with blockage on
expiration
• Three Parts
1. Antral
2. Nasal
3. Choanal
Clinical Features-
Symptoms-
• Nasal obstruction- during expiration & mostly BL
• Like antrochoanal
Signs-
• In early stage polyp may be unilateral in
middle meatus
• Post Rhinoscopy- Pale , transluscent
chonal mass
• Comes out into OPX/NPX
Investigation-
 X Ray PNS
 X Ray NPX
 CT-Scan- OMC
 Nasal Endoscopy
DD-
 Juvenile NPX Angiofibroma
 Meningocoele
 Sphenochoanal Polyp
 Rhinosporidiosis
Treatment-
 Complete Removal
 Transnasal/ Transoral
 Intranasal antrostomy
 Cald Well-Luc Operation
 Endoscopic Polypectomy
