Dated 05-03-2023 at 12:50 Hrs. Fire cached from outside area of the plant which was Observed by
team during patrolling, and we Responded quickly and Mitigated the Fire, 500 meters 6 Sqmm cable
was damage, Security Bike was Damaged partially, and the area fire occurred was public accessible
area and we are suspecting Local people roaming in that road area. and enquiring the nearby people

so, We easily Used MSC Pipeline water to mitigate the Fire.

Dated 10-03-2023 at 10:15 hrs . At Gate No-2 Fire cached from outside area of the plant which was
Observed by team during patrolling, and we Responded quickly and Mitigated the Fire. Local Villagers
Cleaned the two sides of Road which was passing through by our plant & also they are fired the dry
grass other waste thing, so that fire entered into our plant premises through the bottom site of
Boundary wall. There is no Equipment damaged due to fire

6. On 21-03-2023 at around 10:15 am, one first aid incident was ocuured ,the details of this
incident is as follows:
Helper boy MR. Amit Mishra was attending the gardening work near MCR area. During
gardening, while connecting flexible hose pipe to metallic pipe for watering plants he got
injured and resulted in small cut on right hand.
Due to unavailability of pipes connecting arrangement flexible pipe slipped and due to force
full action got injured.
PVC pipe extension with connecting nozzle will be connected to metallic pipe to which flexible
pipe will be connected safely.

1. A revised SoP was drafted and training was given to workers on correct handling, operation
and maintenance of Robot Batteries
2. Aegeus technologies have been instructed on the BMS and Battery casing upgrades to
immobilise battery pack movement within its casing during handling.
3. Handle design have been proposed and tested for easy and safe handling of battery.
4. Scheduled monthly battery health checkups proposed and initiated for all available robot
batteries in Teqo portfolio
5. New charging station with improved electrical routing, grounding and safety procedures
being planned to avoid any impact due to ambient temperature increase expected in H1
Battery Incident report
Incident Brief
Dry cleaning worker picked up charged battery for second shift of cleaning from charging room
and progressed towards Block-17 and suddenly felt rise in temperature of battery casing. He
dropped the battery on ground and informed the supervisor, during the conversation fumes
were observed out of battery and later lead to Flash and Fire in the battery pack
Immediate action taken
Immediately after, Emergency siren was blown at MCR and the quick response team
approached the location and extinguished the flames with fire extinguisher. No personnel
injury happened
Further action planned
Initial investigation was done and battery was transported to vendor premises in Banglore for
detailed investigation. Battery has been replaced under warranty
Incident root cause analysis
Initially internal shorting was reported via Aegeus and report received from Micronics states
that "External Physical pressure on battery lead to internal short circuit between lithium cells
and caused fire on battery "
